摘要
本文报道在8只猫和5名重度脑损伤患者中,用过度通气法(每分钟通气量较实验前约增加50%)降低颅内压的效果。8只猫共进行16次实验,过度通气后5分钟,硬脑膜外压力平均下降16毫米汞柱,相当于原水平的33%(P<0.01).停止通气后即回升,一般在5分钟后恢复到原有水平。5例重度脑损伤中有2例最终存活者共进行6次过度通气治疗,其中5次有明显降压效果,3例最终死亡者各进行一次过度通气治疗,降颅内压作用不明显.
The effects of controlled hyperventilation(HV)on increased intracranial pressure(ICP)were studied both experimentally and clinically.Intracranial hypertension in 8cats was created by extradural balloon technic.When the ICP was maintained above 30 mmHg,HV minute volume increased by 50%for 15 min was carried out altogether16 times.There was 33%drop in ICP after 15 min.With stoppage of HV the ICPreturned to the pre-HV level in 5 min.There was a decrease in PaCOfrom 36.53±6.08 to 21.99±3.35 mmHg,and increase in PaOfrom 79.44±17.24 to 119.63±11.93mmHg after 15 min HV.The results were statistically significant.HV was used in 5head injury cases with deep coma.HV(tidal volume 0.6~1.0 L)lasted for 15~60min was used 9 times.In 3 cases there were no response to HV,and patients diedeventually.In the remaining 2 cases,HV was given 6 times,and the ICP dropped about10.9 mmHg in 5 instances,they survived inspire of severe injury.It is concluded thatunder appropriate HV,considerable drop in ICP could occur.
