摘要
本文报告78例慢性肺脓肿的外科治疗。手术方式为肺叶切除45例,全肺切除22例,双叶或叶段切除10例,肺段切除1例。术后并发脓胸6例,胸腔出血5例,肺瘘3例,炎性分泌物播散3例,其他2例,术后并发症发生率24.4%。近期疗效为治愈74例,未愈2例,死亡2例,手术治愈率94.9%,手术死亡率2.6%.有66例随访1~21年,无症状者51例,症状好转者5例,死亡9例。作者还对慢性肺脓肿的诊断、手术指征及防止术后并发症等问题进行了讨论。
During 1960-1979,78 cases of chronic lung abscess were treated by lobectomy(45 cases),pneumonectomy(22 cases),bilobectomy or iobe-segmentectomy(10 cases)and segmentectomy(1 case).Their postoperative complications included empyema in 6 cases,intrathoracie hemorrhage in 5 cases,alveopleural fistula in 3 cases,dissemination of secretion to bronchi in 3 cases and others.The operative complication rate was 24.4%.The short-term results were 74(94.9%)cases cured,2 not cured and 2(2.6%)died.Sixty-five patients were followed for 1-21 years,51 are living and we11,5 had some symptoms persisted and 9 died.The diagnosis,indications for operation and prevention of postoperative complications are discussed,
