摘要
目的探讨紫癜合剂对原发免疫性血小板减少症(ITP)患者Treg、Th17细胞及其相关转录因子表达的影响。方法选取60例ITP患者,随机分为中药组和激素组,每组各30例。中药组患者予紫癜合剂口服治疗,激素组患者予强的松口服治疗,治疗周期为3个月。比较两组患者的外周血血小板计数、出血评分及生活质量评分;检测患者的血清中Treg、Th17细胞表达,比较两组患者的Treg/Th17比值;检测患者的RORγt mRNA、Foxp3 mRNA表达,比较两组患者的Foxp3/RORγt比值。结果①治疗后,两组患者的血小板计数较治疗前明显上升(P<0.05),出血评分及生活质量评分较治疗前明显降低(P<0.05),且中药组患者的出血评分、生活质量评分明显低于激素组(P<0.05)。②治疗后,中药组患者的Treg细胞表达及Treg/Th17比值较治疗前明显升高(P<0.05),激素组患者的Treg/Th17比值较治疗前升高(P<0.05),中药组患者的Treg细胞表达及Treg/Th17比值高于激素组(P<0.05)。③治疗后,中药组患者的Foxp3 mRNA表达及Foxp3/RORγt比值较治疗前明显上升(P<0.05),且中药组患者的Foxp3 mRNA表达及Foxp3/RORγt比值明显高于激素组(P<0.05)。结论紫癜合剂能有效提升ITP患者的血小板计数,改善出血症状及生活质量,调整体内Treg/Th17免疫失衡状态。
Objective To investigate the effects of Zidian Mixture on Treg and Th17 cells and expressions of related transcription factors in patients with primary immune thrombocytopenia(ITP).Methods Sixty patients with ITP were randomly divided into the Chinese medicine group and hormone group,30 cases in each group.The Chinese medicine group was orally treated with Zidian Mixture,while the hormone group was orally treated with prednisone,with a course of three months.The platelet count in peripheral blood,bleeding score and life quality score were compared between the two groups.The expressions of Treg and Th17 cells in serum were detected,and the Treg/Th17 ratio of the two groups was compared.The mRNA expressions of RORγt and Foxp3 were detected,and the Foxp3/RORγt ratio of the two groups was compared.Results①After treatment,the platelet count of the two groups was increased significantly compared with treatment before(P<0.05),and the bleeding score and life quality score were decreased significantly(P<0.05).The bleeding score and life quality score in the Chinese medicine group were obviously lower than those in the hormone group(P<0.05).②After treatment,the Treg cell expression and Treg/Th17 ratio in the Chinese medicine group were significantly increased compared with treatment before(P<0.05),and the Treg/Th17 ratio in the hormone group was also increased compared with treatment before(P<0.05).The Treg cell expression and Treg/Th17 ratio in the Chinese medicine group were higher than those in the hormone group(P<0.05).③After treatment,the expression of Foxp3 mRNA and Foxp3/RORγt ratio in the Chinese medicine group were significantly increased compared with treatment before(P<0.05),and the expression of Foxp3 mRNA and Foxp3/RORγt ratio in the Chinese medicine group were obviously higher than those in the hormone group(P<0.05).Conclusion Zidian Mixture can effectively increase platelet count,improve bleeding symptoms and quality of life,and adjust Treg/Th17 immune imbalance in patients with ITP.
