白质高信号与颅内外动脉粥样硬化病变相关性的研究进展

摘要 白质高信号(white matter lesions,WML)为脑小血管病中的一类特殊分型,常见于皮层下白质,偶见于小脑及脑干。影像学表现为CT中皮质下白质异常密度,头部MRI中T2WI及FLAIR序列相对对称斑片状高信号。临床表现为步态障碍、尿便障碍、认知障碍及痴呆等,严重影响患者生活质量。其重要发病机制为小动脉及微动脉等小血管病变。人体血供由大、小血管系统构成,两者之间存在结构及功能的极大相关性。近来随着小血管病变研究深度不断被突破,人们逐渐倾向对于大、小血管之间的联系及交互作用的研究。其中,动脉粥样硬化为动脉损害最典型标志,同一个体各部位动脉粥样硬化损害可存在较大差异。现本文就WML与颅内外动脉粥样硬化病变相关性做以简要综述。 White matter lesions(WML)are a special type of cerebral small vessel disease,which is common in the subcortical white matter and occasionally in the cerebellum and brainstem.The imaging findings were the abnormal density of subcortical white matter in CT,and the relatively symmetric patchy high signal of T2 WI and FLAIR sequences in the head MRI[1].Clinical manifestations of gait disorders,urinary dysfunction,cognitive disorders and dementia,seriously affect the quality of life of patients.Its important patho-genesis is small vessel disease such as small arteries and arterioles.Human blood supply consists of large and small vascular systems,and there is a great correlation between structure and function.Recently,as the depth of research on small vessel disease has been continuously broken,people gradually tend to study the connection and interaction between large and small blood vessels.Among them,atherosclerosis is the most typical marker of arterial damage,and there may be a large difference in atherosclerotic lesions in different parts of the same individual.This article briefly reviews the relationship between WML and intracranial and extracranial atherosclerotic lesions.
