食管癌放疗引起放射性肺损伤的研究进展 被引量:1

摘要 食管癌是一种原发于食管黏膜上皮的恶性肿瘤,也是最常见的消化道恶性肿瘤之一,其发病率和死亡率位居前列。放射治疗是食管癌综合治疗的重要组成部分,也是食管癌最常用、最有效的治疗方法。由于食管与肺在解剖结构中紧密相连,食管癌患者接受放疗时易导致放射性肺损伤的发生。本研究介绍食管癌患者的放射性肺损伤,分析其发病机制、食管癌放疗对放射性肺损伤的影响及治疗。利用中国知网和PubMed中英文数据库,以“食管癌”“放射治疗”和“放射性肺损伤”为关键词,检索建库至2022-04-30相关文献。纳入标准:主题涉及食管癌、放射治疗和放射性肺损伤的文献。剔除标准:年份较远的综述类文献。结果表明,放射性肺损伤的发病机制尚未明确,食管癌放射治疗与放射性肺损伤的发展密切相关,临床常用糖皮质激素、HGMB1拮抗剂、谷胱甘肽S-转移酶P1(GSTP1)抑制剂、乌司他丁、尼达尼布、藻蓝蛋白以及新型纳米材料等进行干预治疗,但缺乏可靠有效的治疗方案,继而影响了肿瘤局部控制和患者的生活质量。因此,阐明放射性肺损伤的确切发生机制,减少放射性肺损伤的发生,对其进行更有效的预防和治疗,对保障患者的生活质量具有重要意义。 Esophageal cancer is identified as a malignant tumor originating from the epithelium of the esophageal mucosa,and also known as one of the most common malignant tumors of the digestive tract.Its morbidity and mortality are among the highest.Radiotherapy is an important part of comprehensive treatment of esophageal cancer,and it is also the most commonly used and most effective treatment method for esophageal cancer.Since the esophagus and lung are closely connected in the anatomical structure,patients with esophageal cancer are prone to radiation damage when receiving radiation therapy.In this study,radiation lung injury in patients with esophageal cancer was introduced,and its pathogenesis,the effect of radiotherapy for esophageal cancer on radiation lung injury and its treatment were analyzed.Using CNKI and PubMed with Chinese and English databases,and taking"esophageal cancer","radiotherapy"and"radiation-induced lung injury"as keywords,the relevant literature from the establishment of the database to April 30,2022 was searched.Inclusion criteria:Literature on topics related to esophageal cancer,radiation therapy and radiation lung injury.Exclusion criteria:Reviews with longer vintages.The results show that the pathogenesis of radiation lung injury is not yet clear,radiation therapy for esophageal cancer is closely related to the development of radiation lung injury,glucocorticoids,hgmb1 antagonists,glutathione S-transferase P1(GSTP1)inhibitors,ulinastatin,nitanib,phycocyanin and new nano materials are commonly used in clinical intervention treatment,but there is a lack of reliable and effective treatment plan,which affects the local control of tumor and the quality of life of patients.Therefore,it is of great significance to elucidate the exact mechanism of radiation lung injury,reduce the occurrence of radiation lung injury,and prevent and treat it more effectively to ensure the quality of life of patients.
