摘要
目的讨论多发产气性肝脓肿并脓毒性肺栓塞临床表现及治疗手段。方法回顾性分析2022年11月入住本院的1例多发产气性肝脓肿并脓毒性肺栓塞患者的救治过程。结果患者经急诊肝脓肿穿刺引流术,持续有创呼吸机辅助呼吸,抗感染、输血,CVVH清除炎症因子,去甲肾上腺素+间羟胺持续升压等治疗后,于第23天转外院行康复治疗。结论多发产气性肝脓肿并脓毒性肺栓塞时应及时行肝脓肿穿刺引流术,采用呼吸机辅助呼吸,并进行抗感染、升压等治疗。
Objective To discuss the clinical manifestations and treatment of multiple gas-producing liver abscess with septic pulmonary embolism.Methods The treatment process of a patient with multiple gas-producing liver abscess and septic pulmonary embolism admitted to our hospital in November 200 was analyzed retrospectively.Results After hospitalization,the patient were underwent of liver abscess puncture and drainage,continuous invasive ventilator assisted breathing,anti-infection,blood transfusion,inflammatory factors clearance,and boost presure by noradrenaline combined with M-hydroxyamine.At 23rd day,the patient was recovery and transferred to another hospital for rehabilitation treatment.Conclusion Puncture and drainage of liver abscess together with assisted breathing with ventilator,anti-infection and pressor treatment should be performed in time when multiple gas-producing liver abscess and septic pulmonary embolism occurred.
