摘要
目的探讨彩色多普勒超声对人工关节置换术后小腿肌间静脉血栓(MCVT)的诊断价值。方法选取2018年10月至2020年10月于我院行人工关节置换术的64例疑似MCVT患者作为研究对象。人工关节置换术后3~5 d,采用彩色多普勒超声诊断仪检查全方位多切面重点探查双侧小腿肌间静脉,以下肢静脉造影为金标准,分析彩色多普勒超声诊断价值及其与多排螺旋CT、MRI和手术探查结果的一致性。结果以下肢静脉造影为金标准,64例疑似患者中确诊为MCVT有60例(93.75%)。彩色多普勒超声检查的敏感度为95.00%,特异度为50.00%,准确性为92.19%,阳性预测值为96.61%,阴性预测值为40.00%。Kappa检验显示,彩色多普勒超声与下肢静脉造影检查结果一致性尚可(kappa值0.403,P=0.001)。结论人工关节置换术后彩色多普勒超声对MCVT具有较高的诊断价值,具有较高的灵敏度、准确度及阳性预测值,可为临床治疗提供可靠的依据。
Objective This paper aims to investigate the diagnostic value of color Doppler ultrasound on muscular calf vein thrombosis(MCVT)after artificial joint replacement.Methods 64 suspected MCVT patients were selected from October 2018 to October 2020.On 3-5 d after artificial joint replacement,color Doppler ultrasound was used as diagnostic instruments to examine bilateral calf intermuscle vein,with lower limb angiography as the gold standard,the diagnostic value of color Doppler ultrasound was analyzed,including sensitivity,specificity,accuracy,negative and positive predictors,and the consistency of multiple spiral CT and MRI and surgical exploration results.Results Lower limb venous angiography was gold and 60 of 64 suspected patients(93.75%).After color Doppler ultrasound,sensitivity was 95.00%,specificity of 50.00%,accuracy of 92.19%,positive prediction of 96.61%and negative prediction of 40.00%.Kappa tests showed consistent color Doppler ultrasound and lower limb angiography(kappa 0.403,P=0.001).Conclusion Color Doppler ultrasound has high diagnostic value for MCVT,high sensitivity,accuracy and positive prediction to provide reliable basis for clinical treatment.
