摘要
目的分析多排螺旋CT(MSCT)联合多平面重建技术(MPR)在磨玻璃密度结节样(GGN)肺癌患者的诊断效能。方法选取2020—2022年本院疑似GGN肺癌患者90例,均进行MSCT联合MPR检查,以病理结果为“金标准”,统计MSCT联合MPR检查的诊断结果和诊断效能,并分析肺癌病灶征象显示变化。结果90例疑似GGN肺癌患者中,经病理诊断出恶性30例、良性60例,经MSCT联合MPR检查诊断出恶性28例、良性56例;病理诊断和MSCT联合MPR检查诊断一致性程度很好(Kappa=0.853,95%CI:0.646~1.059)。经MSCT联合MPR检查灵敏度为93.33%(28/30)、特异度为93.33%(56/60)、准确度为93.33%(84/90)、漏诊率为6.67%(2/30)、误诊率为6.67%(4/60)、阳性预测值为87.50%(28/32)、阴性预测值为96.55%(56/58)。30例确诊GGN肺癌患者中,经MSCT联合MPR检查:分叶征24例(80.00%)、血管集束征6例(20.00%)、支气管充气征21例(70.00%)、毛刺或胸膜拉扯征14例(46.67%)。结论MSCT联合MPR检查诊断GGN肺癌可全面显示病灶情况,提高诊断效果与诊断效能,值得临床进一步推广应用。
Objective To analyze the diagnostic accuracy of multi⁃slice spiral CT(MSCT)combined with multi⁃plane reconstruction(MPR)in ground⁃glass density nodular lung cancer(GGN)patients.Methods Ninety patients with sus⁃pected GGN lung cancer in our hospital from 2020 to 2022 were selected.All patients were examined by MSCT combined with MPR.With the pathological results as the“gold standard”,the diagnostic results and diagnostic efficacy of MSCT com⁃bined with MPR were counted,and the changes in lung cancer focus signs were analyzed.Results Among the 90 suspected GGN lung cancer patients,30 cases were malignant and 60 cases were benign by pathology,while 28 cases were malignant and 56 cases were benign by MSCT combined with MPR.There was good consistency between pathological diagnosis and MSCT combined with MPR(Kappa=0.853,95%CI:0.646-1.059).The sensitivity of MSCT combined with MPR was 93.33%(28/30),the specificity was 93.33%(56/60),the accuracy was 93.33%(84/90),the missed diagnosis rate was 6.67%(2/30),the misdiagnosis rate was 6.67%(4/60),the positive predictive value was 87.50%(28/32),and the negative predictive value was 96.55%(56/58).Among the 30 patients diagnosed with GGN lung cancer,MSCT combined with MPR examination showed that there were 24 cases of lobular sign(80.00%),6 cases of vascular bunching sign(20.00%),21 cases of air bron⁃chial sign(70.00%),and 14 cases of burr or pleural pulling sign(46.67%).Conclusion MSCT combined with MPR exami⁃nation for the diagnosis of GGN lung cancer can comprehensively show the lesions,improve the diagnostic effect and diagnos⁃tic efficiency,and is worthy of further clinical application.
