摘要
目的探讨输尿管软镜钬激光碎石术在孤立肾结石治疗中的安全性及有效性。方法回顾性分析本院自2016年9月-2020年6月应用输尿管软镜钬激光碎石术治疗28例孤立肾结石患者的临床资料。男21例,女7例。年龄26岁-58岁,平均46岁。分析清石率、术后住院天数、并发症及血清肌酐变化。结果本组患者输尿管软镜均一次碎石成功,手术时间为32-75min,平均(46±12)min;结石大小为0.8~2.5 cm,平均(1.5±0.3)cm;一期结石清除率为85.71%,其中4例行二次输尿管软镜。术后二个月结石清除率为100%。术后平均住院2-4d(平均2.6±0.7)d,手术相关并发症为17.85%,均为ClavienⅠ级。患者术后2个月血肌酐为58-146 umol/L,平均(69.5±24.8)umol/L,与术前比较差异无统计学意义(P>0.05)。结论输尿管软镜钬激光碎石术治疗孤立肾结石几乎不影响肾功,具有创伤小、恢复快等优点,是安全、有效治疗方式。
Objective To evaluate the safety and efficacy of flexible ureteroscopy(RIRS)combined with holmium laser in the treatment of calculi of solitary kidney.Methods 28 cases of solitary kidney after using RIRS with holmium laser lithotripsy from September 2016 to June 2020 were reviewed.There were 21 male and 7 female patients and the mean age was 46 years(26~58years).And the stone-free rate,postoperative hospitalization time,complication rate and changes in serum creatinine levels were review.Results Soft ureteroscopic lithotripsy was successful in all patients in this group,operation time was 32~75min,mean(46±12)min.The stone size was 0.8~2.5cm,mean(1.5±0.3)cm.The primary stone free rate was 85.71%,4 patient received RIRS twice.The total stone free rate was 100%two months after operation.The postoperative hospital stay was 2-4days(2.6±0.7days).Complication occurrence was 17.85%.All were ClavienⅠgrade.The Serum creatinine was 58-146μmol/L 2 months after operation,mean(69.5±24.8)μmol/L.There was no significant difference between before and after operation(P>0.05).Conclusions Soft ureteroscopic holmium laser lithotripsy has little effect on renal function in the treatment of isolated kidney stones,and is a safe and effective treatment with the advantages of small trauma and quick recovery.
