摘要
目的探究外周血氨基酸水平在阻塞性睡眠呼吸暂停(obstructive sleep apnea syndrome,OSAS)患者中的临床应用价值。方法选择2015年1月-2020年1月于本院接受治疗的60例OSAS患者为研究对象,将其按照是否合并心脑血管疾病区分为并发组(21例,OSAS并发心脑血管疾病)和OSAS组(39例),另选取同期于本院接受体格检查的50例健康个体为对照组,采集三组个体外周血样并检测游离氨基酸(苏氨酸、色氨酸和谷氨酸)水平,同时检测血管内皮生长因子(VEGF)和缺氧诱导因子-1α(HIF-1α)水平,对比三组个体上述指标差异,分析上述指标的相关性,对60例OSAS患者开展为期6个月的临床治疗,对比治疗前后入组患者上述指标变化情况。结果(1)并发组患者外周血中苏氨酸和色氨酸水平明显低于OSAS组和对照组,谷氨酸水平明显高于OSAS组和对照组(P<0.05),OSAS组患者外周血中苏氨酸和色氨酸水平明显低于对照组,谷氨酸水平明显高于对照组(P<0.05);(2)并发组患者外周血中HIF-1α和VEGF浓度明显高于OSAS组,OSAS组明显高于对照组(P<0.05);(3)苏氨酸同HIF-1α、VEGF浓度均呈现正相关联系(r=0.781、0.449,P<0.05),色氨酸同HIF-1α、VEGF浓度均呈现正相关联系(r=0.671、0.723,P<0.05),谷氨酸同HIF-1α、VEGF浓度均呈现负相关联系(r=-0.443、-0.525,P<0.05);(4)干预后OSAS患者苏氨酸、色氨酸水平较干预前出现了明显的升高,谷氨酸水平较干预前出现了明显的降低(P<0.05)。结论外周血氨基酸水平同OSAS患者的病情呈现一定的相关性,可以考虑将其作为OSAS诊断治疗的重要依据之一。
Objective To investigate the clinical application value of peripheral blood amino acid levels in patients with obstructive sleep apnea syndrome(OSAS).Methods 60 cases of OSAS who were treated in our hospital from January 2015 to January 2020 were selectas subjects.They were divided into the concurrent group(21 cases,OSAS complicated by cardiovascular and cerebrovascular diseases)and the OSAS group(39 cases)according to whether they were combined with cardiovascular and cerebrovascular diseases.In addition,50 healthy individuals who received physical examination in our hospital during the same period were selectedascontrol group.Three groups of peripheral blood samples were collected and the levels of free amino acids(threonine,tryptophan and glutamate)were measured,as well as vascular endothelial growth factor(VEGF)and hypoxia-inducible factor-1α(HIF-1α)Level,the differences of the above indicators in the three groups were compared,the correlation of the above indicators were analyzed,6-month clinical treatment on 60 OSAS patients were carry out,and compare the changes of the above indicators before and after the treatment.Results(1)The levels of threonine and tryptophan in the peripheral blood of the concurrent group patients were significantly lower than those in the OSAS group and control group,and the levels of glutamate were significantly higher than those in the OSAS group and control group(P<0.05).The levels of threonine and tryptophan in the peripheral blood of patients in the OSAS group were significantly lower than those in thecontrol group,and the level of glutamate was significantly higher than that of the control group(P<0.05);(2)The levels of HIF-1αand VEGF in the peripheral blood of patients in the concurrent group were significantly higher than those in the OSAS group,the OSAS group were significantly higher than those in the Control group(P<0.05);(3)Threonine was positively correlated with HIF-1αand VEGF concentrations(r=0.781,0.449,P<0.05).Tryptophan was positively correlated with HIF-1αand
