高功率短时程与低功率长时程射频消融治疗心房颤动有效性与安全性的Meta分析

Meta analysis of efficacy and safety of high-power-short-duration and low-power-longduration radiofrequency ablation in the treatment of atrial fibrillation

摘要 目的通过Meta分析系统评价高功率短时程(HPSD)与低功率长时程(LPLD)射频消融治疗心房颤动的有效性和安全性。方法检索PubMed、Embase、Web of Science、Cochrane Library、Medline、VIP、WanFang Data及CNKI数据库,搜集HPSD与LPLD治疗心房颤动有效性和安全性的研究,采用RevMan 5.4版统计软件进行Meta分析。结果共纳入15个研究,与LPLD相比,HPSD可以降低术后心房颤动复发的风险(RR=0.77,95%CI=0.66~0.90,P<0.01)、减少手术并发症发生率(RR=0.61,95%CI=0.39~0.96,P<0.05)以及缩短手术时间(MD=-45.41,95%CI=-59.11~-31.72,P<0.01)。结论与LPLD相比,HPSD可以降低心房颤动消融术后房性心律失常的复发率及手术并发症的发生率,并可以缩短手术时间。 Objective To evaluate the efficacy and safety of high-power-short-duration(HPSD)and low-power-long-duration(LPLD)radiofrequency ablation in the treatment of atrial fibrillation by Meta analysis system.Methods PubMed,Embase,Web of Science,Cochrane Library,Medline,VIP,Wanfang and CNKI databases were searched to collect the researches on the efficacy and safety of HPSD and LPLD in the treatment of atrial fibrillation.Revman 5.4 software was used for Meta analysis.Results A total of 15 studies were included.Compared with LPLD,HPSD can reduce the risk of postoperative recurrence of atrial fibrillation(RR=0.77,95%CI=0.66-0.90,P<0.01),the incidence of surgical complications(RR=0.61,95%CI=0.39-0.96,P<0.05)and the operation time(MD=-45.41,95%CI=-59.11--31.72,P<0.01).Conclusion Existing evidence shows that compared with LPLD,HPSD can reduce the recurrence rate of atrial arrhythmia after atrial fibrillation ablation,shorten the operation time,and reduce the incidence of surgical complications.
