摘要
[背景]分析前列腺脓肿(PA)的临床特点.[病例报告]回顾性分析自2005年3月至2015年6月间就诊的23例PA患者的临床资料,比较朝鲜族(n=7)与汉族(n=16)PA患者的临床特点.所有病例均行经尿道前列腺脓切开引流术.23例患者年龄为52~81岁,平均为(66.4±8.0)岁,其中11例(47.8%)患有糖尿病,7例(30.4%)合并高血压.常见的主要临床症状依次为发热(18例,78.3%)、会阴部不适(15例,65.2%)、尿失禁(14例,60.9%)及急性尿潴留(13例,56.5%).朝鲜族与汉族PA患者年龄、合并糖尿病、嗜酒、脓肿大小及IIEF-5评分间差异均有统计学意义(P<0.05),朝鲜族和汉族PA患者住院时间及细菌培养结果间差异均无统计学意义(P>0.05).治疗后所有患者均未见复发.[讨论]朝鲜族PA患者发病年龄较汉族患者明显低,且合并糖尿病及嗜酒的病例更多,脓肿体积更大,重度ED病例更多.
BACKGROUND To analyze the clinical characteristics of prostatic abscess(PA).CASE REPORTS The clinical data of 23 patients with PA who were admitted between March 2005 and June 2015 were retrospectively analyzed.The clinical characteristics of the Korean nationality(n=7)and Han nationality(n=16)PA patients were compared.All patients were underwent transurethral resection of the prostate.The 23 patients ranged in age from 52 to 81 years,with an average age of(66.4±8.0)years.Among of them,11(47.8%)had diabetes mellitus,and 7(30.4%)were combined with hypertension.The most common clinical symptoms were fever(18,78.3%),perineal discomfort(15,65.2%),urinary incontinence(14,60.9%),and acute urinary retention(13,56.5%).There were statistically significant differences between the Korean and Han nationality PA patients in age,diabetes mellitus,alcohol addiction,abscess size and IIEF-5 score(P<0.05),while there was no significant difference in length of hospital stay and results of bacterial culture between them(P>0.05).No recurrence was observed in all patients after treatment.DISCUSSION The onset age of the Korean nationality PA patients was significantly younger than that of the Han nationality PA patients,and there were more cases combined with diabetes and alcohol addiction,with larger abscess volume,and more cases of severe ED.
