MRI及CT在菌阴肺结核诊断中的应用体会 被引量:4

摘要 目的:探讨MRI和CT这两种影像学检查方法在菌阴肺结核患者中的应用价值。方法:收集60例结核菌培养阴性肺结核患者,分别进行CT和MRI检查,采用单盲法对其影像学表现进行分类及影像学分析,并对这两种影像学检查方法诊断菌阴肺结核的准确率进行统计学分析。结果:CT和MRI对菌阴肺结核的诊断准确率分别为85.0%和68.3%,CT诊断准确率明显高于MRI,差异具有统计学意义(P<0.05)。MRI在对结核球的显示方面优于CT,差异有统计学意义(P<0.05),CT在钙化显示方面优于MRI,差异有统计学意义(P<0.05);但二者在干酪样病变、空洞型病变及结节或肿块型病变显示方面差异无统计学意义(P>0.05)。结论:对于菌阴肺结核患者结核球病灶的检出,MRI明显优于CT;对于菌阴肺结核的干酪样、空洞样、肿块样及结节样病变,两者的诊断价值相当,影像诊断作用可相互补充。 Purpose:To explore the application value of MRI and CT imaging examination methods in patients with bacteriological negative pulmonary tuberculosis.Methods:Sixty patients with sputum culture negative pulmonary tuberculosis were collected.And CT,MRI examinations were processed.Their imaging manifestations were classified and analyzed by single-blind method,and the accuracy of the two imaging examination methods in the diagnosis of sputum culture negative tuberculosis was analyzed.Results:The diagnostic accuracy of CT and MRI for bacillary-negative tuberculosis was 85.0%and 68.3%respectively,and the diagnostic accuracy of CT was significantly higher than that of MRI,with statistically significant differences(P<0.05).MRI was significantly better than CT in the display of tuberculous bulbs,with statistically significant differences(P<0.05).CT was significantly better than MRI in the display of calcification(P<0.05),but there was no significant difference in the display of caseous lesions,cavitary lesions and nodular or mass lesions(P>0.05).Conclusion:For patients with sputum culture negative pulmonary tuberculosis,MRI was significantly better than CT in the detection of tuberculous bulbs.CT and MRI have similar diagnostic value for caseous,cavernous,mass-like and nodular lesions in bacillary-negative tuberculosis and the diagnostic imaging can complement each other.
