摘要
目的探讨支气管扩张症患者临床评估指标与住院经济负担指标之间的相关性。方法回顾广东省中西医结合医院2018年1—12月收治的173例支气管扩张症患者的病历资料,观察临床评估指标[支气管扩张症严重度指数(BSI)、合并病种数、伴随症状四项计分、血清白蛋白计量、血红蛋白计量等]与住院经济负担指标(住院时间和住院费用),并对两组资料进行典型相关分析。结果两组资料经典型相关分析示,第1典型相关系数(r1)为0.409(P<0.05)。伴随症状四项计分、合并病种数与住院费用呈正相关(P<0.05),血清白蛋白、血红蛋白与住院费用呈负相关(P<0.05)。结论支气管扩张症患者的临床评估指标与经济负担指标有一定的相关性。运用BSI临床评估支气管扩张症患者的同时,还要尽全面的评估,合理用药并加强监护及营养支持,减少患者住院经济负担。
Objective To investigate the clinical evaluation and economic index of inpatient bronchiectasis cases.Method A total of 173 patients with bronchiectasis treated in Guangdong Province Hospital of Integrated Traditional Chinese and Western Medicine from January to December 2018 were retrospectively analyzed.The data including the clinical evaluation indexes including the severity index of bronchiectasis(BSI),comorbidities,accompanying symptom score,serum albumin count,serum hemoglobin count,and the hospitalization economic burden index(hospitalization days and hospitalization costs)were collected and canonical correlation analyzed.Result The canonical correlation analysis showed that clinical evaluation indexes(x)and economic burden indexes(y)in the set 1 canonical correlation coefficient was 0.409(P<0.05).The accompanying symptom score,comorbidities exist positive correlation with hospitalization cost(P<0.05),and the serum albumin/hemoglobin count exist negative correlation with hospitalization cost(P<0.05).Conclusion The clinical evaluation indexes have correlation with the hospitalization economic burden indeses.We should use the BSI tool in the clinical evaluation of bronchiectasis,focus on the accompanying symptoms,comorbidities and serum albumin/hemoglobin count,upgrade the nursing level and nutritional support therapies,in order to reduce the hospitalization economic burden of patients.
