摘要
目的探讨来曲唑联合尿促性腺激素(HMG)、来曲唑联合氯米芬治疗多囊卵巢综合征(PCOS)的临床效果,为临床治疗提供参考。方法选取聊城市高唐县人民医院2018年12月—2019年12月收治的92例PCOS患者,随机分为观察A组和观察B组,各46例。观察A组接受HMG联合来曲唑治疗,观察B组接受氯米芬联合来曲唑治疗,均持续治疗1~3个促排卵周期。治疗前及注射绒毛膜促性激素(HCG)当天,检测并比较两组卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)水平,及单卵泡排卵率、排卵率,记录并比较两组治疗期间不良反应发生情况。结果注射HCG当天,两组LH/FSH水平均低于本组治疗前,E2水平高于本组治疗前,且组间E2水平计较,差异均有统计学意义(P<0.05);但两组LH/FSH水平比较,差异无统计学意义(P>0.05)。观察B组单卵泡排卵者率为89.13%,高于观察A组的69.57%,差异有统计学意义(P<0.05);两组排卵率比较,差异无统计学意义(P>0.05)。观察A组不良反应总发生率为15.22%,与观察B组的19.57%比较,差异无统计学意义(P=0.582)。结论PCOS患者应用来曲唑联合HMG或氯米芬治疗均具有较好的效果,其中氯米芬可促进单卵泡排卵,HMG有助于E2水平的调节。
Objective To investigate the intervention effect of letrozole combined with human menopausal gonadotrophin(HMG)and letrozole combined with clomiphene on patients with polycystic ovarian syndrome(PCOS).Methods The clinical data of 92 patients with PCOS admitted in the Gaotang County People’s Hospital of Liaocheng from December 2018 to December 2019 were collected,and they were divided into two groups,46 cases in each group.Group A was treated with HMG combined with letrozole for 1-3 ovulation induction cycles,and group B was treated with clomiphene combined with letrozole for 1-3 ovulation induction cycles.Before treatment and on the day injected with human chorionic gonadotropin(HCG),the sex hormone indexes including follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),ovulation rate and adverse reactions were compared between two groups.Results On the day of HCG injection,compared with before treatment,the levels of LH/FSH decreased and the levels of E2 increased in both groups,and comparison of the level of E2 between groups has significantly difference(P<0.05).The proportion of single follicle ovulation in group A was lower than that in group B(P<0.05).There was no statistical difference in the proportion of ovulation between the two groups(P>0.05).The incidence of adverse reactions in group A(15.22%)was slightly lower than that in group B(19.57%),but the difference was not statistically significant(P>0.05).Conclusion Letrozole combined with HMG and letrozole combined with clomiphene in treatment of patients with PCOS all have better effects.Clomiphene can promote single follicle ovulation better,and HMG can regulate the level of E2 more.
