摘要
目的探讨基于倾向得分匹配法的家庭医生签约服务对医患信任的影响。方法2019年1月以多阶段随机抽样方法,在天津市4个市辖区8个社区卫生服务中心抽取394名居民作为受访者,采用问卷调查的方式对受访者的个人基本情况、家庭医生签约服务情况、医患信任等情况进行调查。以签约居民作为处理组,未签约居民作为控制组,利用倾向得分匹配方法估计平均处理效应。结果倾向得分匹配结果表明,签约家庭医生可以使医患信任水平提高0.27分,差异有统计学意义(P<0.01)。异质性检验结果表明,享受家庭医生签约服务的普通人群医患信任水平比未签约居民高0.46分,差异有统计学意义(P<0.01),而重点人群的平均处理效应比较,差异无统计学意义(P>0.05)。结论家庭医生签约居民的医患信任水平高于未签约居民。相比于重点人群,普通人群更容易由于享受家庭医生签约服务而提高医患信任水平。
Objective To explore the impact of family physician contracting services on doctor-patient trust.Methods In January 2019,394 residents were selected from 8 community medical service centers in four districts of Tianjin by multistage random sampling,and they were investigated by a questionnaire survey on their personal basic situation,utilization of family physician contracting services,and the level of doctor-patient trust.Propensity score matching was used to study the impact of receiving family physician contracting services on healthcare seeking behaviors by comparing such behaviors between contracted residents(treatment group)and those non-contracted(control group).Results Propensity score matching results showed that the level of doctor-patient trust of contracted residents was 0.27 scores higher than that of non-contracted residents.The level of doctor-patient trust of the general population who enjoyed the family physician contracting services was 0.46 scores higher than that of non-contracted residents(P<0.01).However,no significant differences were found in doctorpatient trust between contracted and non-contracted priority populations(P>0.05).Conclusion The level of doctor-patient trust of contracted residents is higher than that of non-contracted residents.Compared with the priority population,the general population is more likely to improve the level of doctor-patient trust because of the enjoyment of family physician contracting services.
