不同剂量盐酸异丙嗪治疗药源性帕金森综合征的临床疗效

Comparison of the clinical efficacy of different doses of promethazine hydrochloride in the treatment of drug-induced Parkinson’s syndrome

摘要 目的探讨不同剂量盐酸异丙嗪治疗药源性帕金森综合征的临床疗效。方法选取2018年8月至2021年5月在新郑市公立人民医院院诊治的药源性帕金森综合征患者68例作为研究对象,根据随机信封抽签原则分为研究组与对照组,每组34例。研究组与对照组均给予盐酸异丙嗪治疗,盐酸异丙嗪用剂量分别为25、50 mg,连续应用7 d。观察两组治疗后总有效率、治疗期间不良反应发生率,比较两组治疗前后全血黏度、血浆比黏度、血清超氧化物歧化酶(superoxide dismutase,SOD)活力以及丙二醛(malondialdehyde,MDA)含量的变化。结果治疗后,研究组与对照组总有效率分别为97.06%和100.00%,差异无统计学意义(P>0.05)。研究组治疗期间眩晕、盗汗、恶心呕吐、耳聋等不良反应总发生率为5.88%,低于对照组的29.41%,差异有统计学意义(P<0.05)。两组治疗前全血黏度和血浆比黏度比较,差异均无统计学意义(P>0.05);两组治疗后全血黏度和血浆比黏度均低于本组治疗前,研究组均低于对照组,差异均有统计学意义(P<0.05)。两组治疗前血清SOD活力和MDA含量比较,差异均无统计学意义(P>0.05);两组治疗后血清SOD活力均高于本组治疗前,MDA含量均低于本组治疗前,差异均有统计学意义(P<0.05),两组治疗后SOD活力、MDA含量比较,差异均无统计学意义(P>0.05)。结论25 mg盐酸异丙嗪治疗药源性帕金森综合征也能提高患者的抗氧化能力与总体治疗效果,且能更有效地改善机体血液流变学状况,减少不良反应的发生。 Objective To determine the clinical efficacy of different doses of promethazine hydrochloride in the treatment of drug-induced Parkinson’s syndrome.Methods From August 2018 to May 2021,68 patients with drug-induced Parkinson’s syndrome were diagnosed and treated in our hospital.All the patients were divided into study and control groups with 34 patients in each group by a random envelope drawing.Both the study and control groups were treated with promethazine hydrochloride(25 and 50 mg)for 7 days.The total effective rate,incidence of adverse reactions,whole blood viscosity,plasma-specific viscosity,superoxide dismutase(SOD)and malondialdehyde(MDA)levels were determined in the two groups.Results After treatment the total effective rates of the study and control groups were 97.06%and 100.00%,respectively;there were no statistically significant differences between the two groups(P>0.05).The incidence of adverse reactions,such as dizziness,night sweats,nausea,vomiting,and deafness,in the study group during treatment was 5.88%,which was less than the 29.41%in the control group(P<0.05).The whole blood viscosity and plasma-specific viscosity of the two groups after treatment were lower than before treatment,and lower in the study group compared to the control group(P<0.05).The SOD levels after treatment were higher in the two groups than before treatment,and MDA levels were lower than before treatment(P<0.05);the differences within the group were not statistically significant(P>0.05).Conclusion Lowdose promethazine hydrochloride in the treatment of drug-induced Parkinson’s syndrome improved the patient’s antioxidant capacity and overall therapeutic effect,and more effectively improved the body’s hemorheology and reduced the incidence of adverse reactions.
