摘要
我科室收治一例单肺通气下行左胸小切口冠状动脉旁路移植术的男性患者在术后12小时内出现急性肺损伤的患者,总结其气道护理方法。患者持续有创通气,氧合指数<300mmHg,双肺弥漫性浸润,超声心动示心功能良好,诊断急性肺损伤,通过给予充分的镇静镇痛治疗并根据2006版急性肺损伤/急性呼吸窘迫综合征诊断和治疗指南和2006版机械通气临床应用指南给予相应的保护性通气,肺复张,气道温湿化等相应的气道治疗护理后,患者于术后第5天成功拔出气管插管。
A male patient with acute lung injure undergoing left minimal incision coronary artery bypass grafting under one-lung ventilation within 12 hours after operation was admitted to our department,summarize the methods of airway nursing.Invasive ventilation for the patient,PaO2/FiO2<300 mmHg,diffuse infiltration of both lungs,diagnosis of acute lung injury.On the basis of adequate sedation and analgesia,protective ventilation,lung recruitment,airway humidification and other airway nursing are provided through the guidelines for the diagnosis and treatment of acute lung injury/acute respiratory distress syndrome(2006)and the practical guidelines for mechanical ventilation(2006),the patient was extubated seventh days after the operation.
