摘要
目的探讨超声引导下静脉留置针穿刺术对老年心肺复苏患者一次穿刺成功率、并发症的影响。方法选取60例老年心肺复苏患者作为受试对象,分为研究组与对照组各30例。对照组采取传统穿刺中心静脉置管,研究组采用超声引导下静脉留置针穿刺术。比较两组患者手术时长、出血量、穿刺时间、穿刺成功率评估差异;比较术后3d内,两组并发症(血肿、静脉血栓、感染、管路阻塞)发生情况差异。结果研究组患者手术时长、出血量、穿刺时间明显少于对照组(P<0.05);研究组患者一次穿刺成功率明显高于对照组(P<0.05),两组患者两次穿刺成功率及多次穿刺成功率比较无统计学意义(P>0.05);术后3d内,研究组患者血肿发生率明显低于对照组(P<0.05),两组患者静脉血栓、感染、管路阻塞发生率比较无统计学意义(P>0.05)。结论超声引导下静脉留置针穿刺术应用于老年心肺复苏患者,可有效改善穿刺效果,提高穿刺成功率,降低术后并发症风险。
Objective To investigate the effect of ultrasound-guided venous indwelling needle puncture on the success rate and complications of one puncture in elderly cardiopulmonary resuscitation patients.Methods 60 elderly patients with cardiopulmonary resuscitation were selected as the study subjects,and 30 patients in the study group and 30 patients in the control group were selected according to the random number table method.The control group was treated with conventional central venous catheterization,and the study group was treated with ultrasound-guided venous indwelling needle puncture.The differences in operation time,intraoperative blood loss,puncture exploration time and puncture success rate between the two groups were compared;and the differences in the occurrence of complications(local hematoma,venous thrombosis,infection,pipeline blockage)between the two groups within 3 days after operation were compared.Results The operation time,intraoperative bleeding volume and puncture exploration time of the study group were significantly less than those of the control group(P<0.05);the success rate of one puncture in the study group was significantly higher than that of the control group(P<0.05),and there was no significant difference in the success rate of two punctures and multiple punctures between the two groups(P>0.05);within 3 days after operation,the incidence of local hematoma in the study group was significantly lower than that in the control group.(P<0.05),there was no significant difference in the incidence of venous thrombosis,infection and pipeline blockage between the two groups(P>0.05).Conclusion Ultrasound-guided venous indwelling needle puncture in elderly patients with cardiopulmonary resuscitation can effectively improve the puncture effect,improve the success rate of puncture,and reduce the incidence of postoperative complications.
