摘要
目的分析柔红霉素治疗小儿急性白血病心脏毒性的效果。方法选取2019年5月至2020年5月广东医科大学顺德妇女儿童医院收治的100例急性白血病患儿作为研究对象,开展回顾性分析,以是否出现心脏毒性进行分组,对照组(n=70)未出现心脏毒性,观察组(n=30)出现心脏毒性,对比两组肝酶升高例数及柔红霉素累积量。针对出现心脏毒性的患儿,按照柔红霉素累积量进行分组,A组柔红霉素累积量<100mg/m2,B组柔红霉素累积量101~200mg/m2,C组柔红霉素累积量201~3000mg/m2,D柔红霉素累积量301~400mg/m2,E组柔红霉素累积量>400mg/m2,分析柔红霉素累积量和心脏毒性发生之间的相关性。结果D组、E组两组心脏毒性发生率,无统计学差异(P>0.05);D组、E组心脏毒性发生率明显高于A、B、C组,数据有统计学差异(P<0.05)。观察组肝酶升高例数、柔红霉素累积量明显高于对照组,数据有统计学差异(P<0.05)。结论针对接受柔红霉素治疗的急性白血病患儿应密切关注其柔红霉素累积量以及心脏毒性情况,需定期进行心脏检查,尽早发现心脏毒性并采取针对性的措施,并酌情考虑减少柔红霉素剂量,尽可能降低心脏毒性的发生。
Objective To analyze effect of daunorubicin on cardiotoxicity of acute leukemia children.Methods The paper chose 100 cases acute leukemia children in our hospital from May 2019 to may 2020 as research objects for review and analysis,and divided them into groups based on cardiotoxicity.Control group(n=70)was without cardiotoxicity and observation group(n=30)with cardiotoxicity.Cases with liver enzymes increase and cumulative amount of daunorubicin was compared between two groups.According to daunorubicin accumulation,children with cardiotoxicity were divided them into groups,daunorubicin accumulation of groups A was less than 100mg/m2,that of B was 101-200mg/m2,that of group C was 201~3000mg/m2,that of groups D was 301-400mg/m2,and that of group E was more than 400mg/m2.Correlation between accumulation of daunorubicin and cardiotoxicity was analyzed.Results There was no significant difference in cardiotoxicity incidence between group D and E(P>0.05).Cardiotoxicity incidence in group D and E was significantly higher than group A,B and C(P<0.05),data showed statistically significant difference.Cases with liver enzyme increase and daunorubicin accumulation in observation group were significantly higher than control group,data showed statistically significant difference,(P<0.05).Conclusion It is necessary to pay close attention to accumulation of daunorubicin and cardiotoxicity of acute leukemia children receiving daunorubicin treatment,carry out regular cardiac examination,detect cardiotoxicity as early as possible and take targeted measures,reduce dosage of daunorubicin as appropriate,and lower cardiotoxicity incidence as far as possible.
