摘要
目的探究超声小探头气管镜对肺外周结节病变的诊断价值。方法选取广东省高州市人民医院2018年3月至2020年8月30例肺外周结节病变患者作为研究对象,采用前瞻性研究方法予以分析。以病理检测结果作为"金标准",整理超声小探头引导下经支气管镜肺活检的诊断结果,判断超声小探头引导下经支气管镜肺活检的诊断价值。结果经手术病理诊断,30例肺外周结节病变患者均为肺癌患者。经超声小探头引导下经支气管镜肺活检诊断后,21例患者为肺癌患者,检出率为70.00%;7例患者为良性病变患者,检出率为23.33%;2例未能确诊患者,占比为6.67%。在21例肺癌患者中,有1例小细胞肺癌患者,占比为4.76%;有14例肺腺癌患者,占比为66.67%;有6例鳞状上皮细胞癌患者,占比为28.57%。在7例良性病变患者中,有4例肺结核患者,占比为57.14%;有3例肺脓肿患者,占比为42.86%。结论在肺外周结节病变的检查诊断中,超声小探头气管镜具有较高的临床诊断价值,能够准确诊断不同类型的肺癌,并制定不同类型肺癌患者的临床治疗方案,实现肺癌患者的早诊断、早治疗目的。
Objective To explore diagnostic value of ultrasonic small probe bronchoscopy in peripheral pulmonary nodules.Methods The paper chose 30 patients with peripheral pulmonary nodules from March 2018 to August 2020 as research objects,and analyzed them with prospective study method.with"gold standard"of pathological examination results,diagnostic results of small probe ultrasound-guided bronchoscopic lung biopsy were sorted out to determine diagnostic value of small probe ultrasound-guided bronchoscopic lung biopsy.Results All 30 patients with peripheral pulmonary nodules were diagnosed as lung cancer by operation and pathology.21 cases of lung cancer were diagnosed with bronchoscopic lung biopsy under guidance of small ultrasonic probe,with detection rate of 70.00%;positive rate was 23.33%;2 cases were not diagnosed,accounting for 6.67%.Among 21 cases of lung cancer patients,1 case was small cell lung cancer patients,accounting for 4.76%;there were 14 cases of lung adenocarcinoma,accounting for 66.67%;there were 6 cases of squamous cell carcinoma,accounting for 28.57%.Among 7 cases of benign lesions,4 cases were pulmonary tuberculosis,accounting for 57.14%;there were 3 cases of lung abscess,accounting for 42.86%.Conclusion Ultrasonic small probe tracheoscopy has high clinical diagnostic value in examination and diagnosis of peripheral pulmonary nodules,can diagnose different types of lung cancer accurately,formulate clinical treatment schemes for different types of lung cancer patients,to realize early diagnosis and treatment of lung cancer patients.
