摘要
目的分析高血压基底节区脑出血疾病采用小骨窗开颅微创手术治疗的临床效果,以及对患者生活质量及预后的影响。方法选入东莞市樟木头医院于2016年9月至2019年10月进行治疗的高血压基底节区脑出血疾病患者50例,以随机数字表法的形式将其分为研究组和参照组,分别为25例。参照组行传统骨瓣开颅血肿清除手术治疗,研究组行小骨窗开颅微创手术治疗,对比和分析两组临床治疗效果、两组生活能力恢复情况、手术时间、住院时间、手术中的出血量、血肿清除程度以及两组并发症。结果研究组的总有效率高于参照组,组间差异有统计学意义(P<0.05);研究组的生活能力恢复情况、血肿清除程度均高于参照组,且研究组手术时间、住院时间、手术中的出血量、并发率均低于参照组,组间差异有统计学意义(P<0.05)。结论对于高血压基底节区脑出血疾病采用小骨窗开颅微创手术进行治疗,能够减少手术中的出血量,缩短手术时间,较好的清除血肿,有利于神经功能的恢复,降低颅内感染。
Objective To analyze clinical effect of small bone window craniotomy in treatment of hypertensive basal ganglia intracerebral hemorrhage,and effect on life quality and prognosis of patients.Methods The paper chose 50 patients with hypertensive basal ganglia intracerebral hemorrhage in our hospital from September 2016 to October 2019,and divided them into study group and reference group randomly,with 25 cases in each group.Control group was treated with traditional bone flap craniotomy and hematoma clearance surgery,while study group with small bone window craniotomy and minimally invasive surgery.Clinical treatment effect,recovery of life ability,operation time,length of stay,blood loss during operation,hematoma clearance degree and complications was compared and analyzed between two groups.Results Total effective rate of study group was higher than control group,with difference between two groups(P<0.05);recovery of life ability and hematoma clearance degree of study group was higher than reference group,operation time,length of stay,intraoperative blood loss and complication rate of study group was lower than reference group,with significant difference between two groups(P>0.05).Conclusion Small bone window craniotomy minimally invasive surgery treatment can reduce amount of blood loss,shorten operation time,remove hematoma better for hypertensive basal ganglia cerebral hemorrhage disease,help recovery of neurological function,reduce intracranial infection,which is worthy of wide range of clinical promotion and application.
