摘要
目的分析替诺福韦酯治疗妊娠慢性乙肝患者的临床效果。方法将甘肃医学院附属医院于2019年1月至2020年12月接受治疗的妊娠慢性乙肝患者50例作为本次研究对象,分为观察组和对照组,对比分析两组患者分别治疗前后ALT、HBV-DNA水平变化,ALT复常率、HBV-DNA转阴率、不良反应发生率、新生儿HBsAg阳性率、HBV-DNA阳性率、新生儿Apgar评分。结果治疗后相较于对照组患者,观察组ALT、HBV-DNA水平明显更低,对比统计学差异明显(P<0.05);相较于对照组患者,观察组服药后5周、20周、35周ALT复常率、HBV-DNA转阴率明显更高,对比统计学差异明显(P<0.05);与对照组患者相比较,观察组不良反应发生率明显更低,对比统计学差异明显(P<0.05);观察组患者新生儿HBsAg阳性率、HBV-DNA阳性率明显更低,新生儿Apgar评分明显更高,对比统计学差异明显(P<0.05)。结论在妊娠慢性乙肝患者的治疗中采用替诺福韦酯治疗可明显提高患者治疗效果,提高ALT复常率、HBV-DNA转阴率,降低新生儿HBsAg阳性率、HBV-DNA阳性率,提高新生儿出生质量,值得临床广泛应用。
Objective To analyze the clinical effect of tenofovirate in the treatment of chronic hepatitis B in pregnancy.Methods From January 2019 to December 2020,50 patients with chronic hepatitis B were treated in affiliated hospital of Gansu medical college,divided into observation group and control group,comparative analysis of the changes of ALT,HBV-DNA levels before and after treatment,ALT recurrence rate,HBV-DNA negative rate,adverse reaction rate,neonatal HBsAg positive rate,HBV-DNA positive rate,neonatal Apgar score.Results After treatment,compared with the control group,ALT,HBV-DNA levels in the observation group were significantly lower,comparative statistical difference was significant(P<0.05).Compared to the control group,the recurrence rate and HBV-DNA negative rate were higher ALT 5 weeks,20 weeks and 35 weeks after administration,comparative statistical difference was significant(P<0.05).Compared with the control group,the incidence of adverse reactions in the observation group was significantly lower,comparative statistical difference was significant(P<0.05).The positive rate of neonatal HBsAg and HBV-DNA of the observation group was significantly lower,and the Apgar score of neonates was significantly higher,with statistically significant differences(P<0.05).Conclusion Tenofoviride in the treatment of chronic hepatitis B patients in pregnancy can significantly improve the treatment effect,to increase ALT recurrence rate,HBV-DNA negative rate,reduce the positive rate of neonatal HBsAg,HBV-DNA positive rate,improving the quality of newborn births,it is worthy of wide clinical application.
