摘要
目的探讨泌尿系统结石患者应用输尿管软镜的临床研究。方法将2019年1月至2020年6月在中国人民解放军联勤保障部队第906医院泌尿外科治疗的94例泌尿系统结石(输尿管上段及肾结石)患者随机分为对照组和观察组,各47例,对照组使用微创经皮肾镜取石术,观察组使用输尿管软镜钬激光碎石术,对比两组的结石清除率及手术指标、各项住院指标、应激指标。结果观察组手术成功率、3个月结石清除率、并发症发生率与对照组相当(P>0.05),但观察组初始结石清除率低于对照组(P<0.05);观察组手术时间、术后1d的VAS疼痛评分、住院时间均少于对照组(P<0.05);观察组术后1d的COR、Ang-Ⅱ、ET-1明显低于对照组(P<0.05)。结论泌尿系统结石患者应用输尿管软镜的临床效果显著,创伤小,手术时间短,术后疼痛轻,应激反应小,手术成功率及结石清除效果好。
Objective To investigate application of flexible ureteroscope for patients with urinary calculi.Methods The paper chose 94 patients with urinary calculi(upper ureteral and renal calculi)in urology department of our hospital from January 2019 to June 2020,and divided them into control group and observation group randomly,with 47 cases in each group.Control group was treated with minimally invasive percutaneous nephrolithotomy,and observation group with flexible ureteroscopic holmium laser lithotripsy.Stone removal rate,surgical index,length of stay index and stress index were compared between two groups.Results Success rate of operation,3-month stone clearance rate and complication rate of observation group was similar to control group(P>0.05),initial stone clearance rate of observation group was lower than control group(P<0.05);operation time,VAS pain scores and length of stay of observation group were less than control group(P<0.05);levels of COR,Ang-ⅱand ET-1 in observation group were significantly lower than control group(P<0.05).Conclusion Flexible ureteroscope can achieve significant clinical effect for urinary calculi patients,with less trauma,shorter operation time,lighter postoperative pain,less stress reaction,better surgical success rate and stone removal effect,which has positive clinical significance.
