摘要
目的分析子宫肌瘤剔除术后妊娠情况及影响妊娠的相关因素。方法回顾性分析茂名市电白区人民医院2018年6月-2019年6月收治的300例采用子宫肌瘤剔除术治疗患者的临床资料,患者术后均停止避孕,采集受孕患者临床基本情况资料、手术治疗方式资料以及切除肌瘤临床特征资料等,对可能影响其术后受孕的相关因素进行多因素Logistic回归分析。结果300例患者术后共计204例曾受孕,占比68.00%,受孕患者中,术后首次怀孕距手术6至24个月不等,均值(12.37±4.41)个月,一共妊娠210次。既往孕次>4次、年龄>35岁、有不孕不育就诊史、受教育年限≤16年的患者,其术后受孕率明显低于既往孕次≤4次、年龄≤35岁、无不孕不育就诊史、受教育年限>16年的患者(P<0.05)。肌瘤数目为多发的患者,其术后受孕率明显低于肌瘤数目为单发的患者(P<0.05),手术方式、肌瘤类型以及肌瘤最大直径和术后受孕率无明显关系(P>0.05)。多因素Logistic回归分析结果显示影响患者术后受孕的独立危险因素为年龄>35岁和有不孕不育就诊史(P<0.05)。结论对于有生育要求的子宫肌瘤患者,子宫肌瘤剔除术不失为一种安全有效的治疗选择,但是治疗时需要注意对患者年龄以及自身既往有无不孕不育就诊史等因素进行综合考虑,以尽量提高妊娠率。
Objective Analysis of pregnancy after uterine fibroids removal and related factors affecting pregnancy.Methods A retrospective analysis of the clinical data of 300 patients treated with myomectomy in our hospital from June 2018 to June 2019.All 300 patients stopped contraception after the operation.The basic clinical conditions of pregnant patients,surgical treatment methods and data were collected.The clinical characteristics of myoma were removed,and the multivariate Logistic regression analysis was performed on the related factors that may affect their postoperative pregnancy.Results A total of 204 cases of 300 patients had been pregnant after operation,accounting for 68.00%.Among pregnant patients,the first pregnancy after operation was 6 to 24 months after the operation,with an average of(12.37±4.41)months,and a total of 210 pregnancies.For patients with previous pregnancy>4 times,age>35 years old,medical history of infertility,and years of education≤16 years,the postoperative conception rate was significantly lower than that of previous pregnancies≤4 times,age≤35 years,all Patients with a history of infertility treatment and years of education>16 years(P<0.05).In patients with multiple fibroids,the postoperative pregnancy rate was significantly lower than in patients with single fibroids(P<0.05).There was no significant relationship between the operation method,fibroids type,and the maximum diameter of fibroids and the postoperative pregnancy rate(P>0.05).The results of multivariate logistic regression analysis showed that the independent risk factors that affected the patient’s postoperative pregnancy were age>35 years old and history of infertility visits(P<0.05).Conclusion For patients with uterine fibroids with fertility requirements,uterine fibroids removal is a safe and effective treatment option,but it is necessary to pay attention to comprehensive consideration of the patient’s age and his/her own history of infertility in order to try his best Improve pregnancy rate.
