摘要
目的探索急诊阿替普酶静脉溶栓治疗高龄急性缺血性脑卒中患者对预后的影响。方法筛选茂名市高州市人民医院收治的高龄急性缺血性脑卒中患者(2020年1月-2021年12月)为研究对象,按照盲抽签法将60例患者均分对照组与观察组,对照组开展常规保守治疗,观察组开展急诊阿替普酶静脉溶栓治疗,观察两组患者炎症应激指标、NIHSS评分、Barthel评分、血清炎症因子指标,计算治疗有效率。结果两组患者(治疗前)各项炎症应激指标(MDA、SOD、CSH-px)无对比性(P>0.05),治疗后观察组患者MDA指标更低、SOD、CSH-px指标更高,差异有统计学意义(P<0.05)。有效率观察组96.67%高于对照组83.33%,存在统计差异(P<0.05)。两组患者(治疗前)NIHSS评分、Barthel评分无差异(P>0.05)。治疗后观察组患者NIHSS评分低于对照组,Barthel评分高于对照组,差异显著(P<0.05)。两组患者(治疗前)各项血清炎症因子指标(IL-1β、TNF-α、hs-CRP)无差异(P>0.05),治疗后观察组各项指标明显优于对照组,存在统计学意义(P<0.05)。结论运用阿替普酶静脉溶栓治疗高龄急性缺血性脑卒中患者效果较为明显,可改善患者预后,降低对患者神经功能损伤,促使患者及早恢复健康,值得推广。
Objective To explore the effect of emergency intravenous thrombolysis with alteplase on prognosis of elderly patients with acute ischemic stroke.Methods Screening elderly patients with acute ischemic stroke admitted to our hospital from January 2020 to December 2021 as the research objects,60 patients were divided into control group and observation group according to blind lottery.The control group received conventional conservative treatment,and the observation group received emergency intravenous thrombolysis of alteplase.Inflammatory stress index,NIHSS score,Barthel score and serum inflammatory factor index of the two groups were observed,and the treatment efficiency was calculated.Results There was no comparison between the two groups(before treatment)in inflammatory stress indexes(MDA,SOD,CSH-Px)(P>0.05),after treatment in observation group,MDA index was lower,SOD,CSH-Px index was higher,and the difference was significant(P<0.05).The effective rate in observation group(96.67%)was higher than that in control group(83.33%),with statistical difference(P<0.05).There was no difference in NIHSS score and Barthel score between the two groups(P>0.05).After treatment,NIHSS score of observation group was lower than that of the control group,Barthel score was higher than that of the control group,and the differences were significant(P<0.05).There were no differences in serum inflammatory factors(IL-1β,TNF-αand hs-CRP)between the two groups(P>0.05),and all indexes in observation group were significantly better than those of the control group after treatment,with statistical significance(P<0.05).Conclusion Intravenous thrombolysis with alteplase has obvious effect on elderly patients with acute ischemic stroke,which can improve the prognosis of patients,reduce the neurological impairment of patients,and promote the early recovery of patients,which is worthy of promotion.
