摘要
目的研究胎儿大脑中动脉及脐动脉PI、RI、S/D值与宫内窘迫发病的关系。方法以2018年9月至2020年2月于本院治疗的产妇3038例作为研究对象,根据患者的临床指征,宫内窘迫患者395例,未发生宫内窘迫患者2643例,分析两组患者的一般资料、大脑中动脉及脐动脉PI、RI、S/D值进行比较,分析大脑中动脉、脐动脉的PI,RI以及S/D值与新生儿体重、新生儿Apagar评分的相关性。结果宫内窘迫组患者的大脑中动脉PI,RI以及S/D值显著低于非宫内窘迫组,宫内窘迫组患者的PI,RI以及S/D值显著高于非宫内窘迫组(P<0.05);宫内窘迫组患者的新生儿体重、新生儿Apagar评分显著低于非宫内窘迫组,大脑中动脉PI,RI以及S/D值与新生儿体重、新生儿Apagar评分呈现正相关,脐动脉PI,RI以及S/D值与新生儿体重、新生儿Apagar评分呈现负相关(P<0.05)。结论胎儿大脑中动脉及脐动脉PI、RI、S/D值与宫内窘迫具有显著相关性,通过对胎儿的超声诊断,可在一定程度上对于患者的宫内窘迫进行有效预测,及早开展临床干预。
Objective:To study the relationship between PI,RI,S/D of fetal middle cerebral artery and umbilical artery and intrauterine distress.Methods:3038 cases of parturients who were treated in our hospital from September 2018 to February 2020 were taken as the research objects.According to the clinical indications of the patients,395 cases of intrauterine distress and 2643 cases of non intrauterine distress were analyzed.The general data,PI,RI,S/D values of middle cerebral artery and umbilical artery of the two groups were compared.The correlation of PI,RI and S/D of middle artery and umbilical artery with neonatal weight and apagar score was analyzed.Results:PI,RI and S/D of the middle cerebral artery in the intrauterine distress group were significantly lower than those in the non intrauterine distress group,PI,RI and S/D of the intrauterine distress group were significantly higher than those in the non intrauterine distress group(P The scores of PI,RI and S/D were positively correlated with the body weight and apagar(P<0.05).Conclusion:PI,RI,S/D of fetal middle cerebral artery and umbilical artery have significant correlation with intrauterine distress.Through ultrasound diagnosis of fetus,we can effectively predict the intrauterine distress of patients to a certain extent,and carry out clinical intervention as early as possible.
