摘要
目的了解老年精神分裂症认知损害现状并分析其影响因素,为患者认知损害的预防及康复治疗提供依据。方法选取82名老年精神分裂症患者为研究对象,采用一般资料调查表、阳性和阴性症状量表(PANSS)、蒙特利尔认知评估量表(MoCA)对其进行评估,并根据MoCA评分将研究对象分为认知损害组和认知正常组。采用单因素分析、Logistic回归分析对患者认知损害的影响因素进行分析。结果两组患者的病程、糖化血红蛋白水平、β_(2)-微球蛋白水平、同型半胱氨酸水平和一般精神病理症状评分存在统计学差异(P<0.05),在回归分析中显示病程越长、同型半胱氨酸水平越高以及一般精神病理症状评分越高的患者具有更高的认知损害风险(P<0.05)。结论老年精神分裂症认知损害与多种因素相关,患者病程越长,同型半胱氨酸水平越高以及一般精神病理症状越明显的患者认知损害程度越高。
Objective To investigate the current status of cognitive impairment in elderly schizophrenia,analyze its influencing factors,and provide basis for the rehabilitation of cognitive impairment in patients.Methods 82 schizophrenic patients in remission were selected and assessed with the general information questionnaire,Positive and Negative Symptom Scale(PANSS)and Montreal Cognitive Assessment Scale(MoCA).The subjects were divided into cognitive impairment group and cognitive normal group according to the MoCA score.Single factor analysis and Logistic regression analysis were used to analyze the influencing factors of patients'cognitive impairment.Results There are statistical differences in course of disease,glycated hemoglobin level,beta2-microglobulin level,homocysteine level and general psychopathology score(P<0.05).Regression analysis showed that long course of disease,high homocysteine level and high general psychopathology score were risk factors for cognitive impairment(P<0.05).Conclusion Cognitive impairment in elderly schizophrenia is related to many factors.The longer the course of disease,the higher the level of homocysteine and the more obvious the general psychopathology symptoms,the higher the degree of cognitive impairment.
