流式细胞技术监测恶性肿瘤患者免疫机能的动态变化

摘要 目的 通过对数据的组内和组间比较分析,初步评估免疫细胞监测在恶性肿瘤患者辅助个性化诊疗和预后判断方面的临床可用性。方法 选取我院38例志愿者,建立恶性肿瘤患者(n=19)、健康人群(n=19)两个小组,分别在3个时间点取外周血,检测恶性肿瘤患者T细胞亚群、NK细胞水平以及IL-6、TNF-α含量的动态变化。结果 与健康人群相比,恶性肿瘤患者的细胞毒性T淋巴细胞(CD8^(+))极显著增高(P<0.01),而辅助性T淋巴细胞(CD4^(+))、CD4^(+)/CD8^(+)、免疫总淋巴细胞极显著降低(P<0.01),T淋巴细胞(CD3^(+))两组间差异无统计学意义(P>0.05);肿瘤人群的NK细胞(CD16^(+)和CD56^(+))活性显著增加(P<0.05)。炎症因子方面,肿瘤患者的IL-6极显著高于健康人群(P<0.01),而TNF-α差异无统计学意义(P>0.05)。此外,恶性肿瘤患者的免疫力呈现出明显的个体差异。结论 利用流式细胞技术监测免疫细胞水平,跟踪评估患者免疫机能的变化情况,可以帮助医生更清晰地了解患者的免疫状态,进而为其采取更个性化的诊疗手段提供科学依据。 Objective To preliminarily evaluate the clinical value of immune cell monitoring in assisting personalized diagnosis and prognosis of patients by analyzing the data within and between groups of healthy paple and cancer patients.Methods A total 38 people whose venous blood samples were drawn from upper limbs in 3 batches at different times and divided into health group(n=19) and cancer group(n=19).The T cell,NK cell and IL-6/TNF-α levels were detected and the dynamic changes of people's immune function were described.Results The mean levels of lymphocyte,CD4^(+)T cell and CD4^(+)/CD8^(+) ratio of cancer group were lower than those of health group significantly(P<0.01),whereas the mean levels of CD8^(+) T cell and NK cell were significantly higher than those of the health group(P<0.01).The data of CD3^(+) T cell was no significant difference between two groups(P>0.05).The results of inflammatory factor detection showed that the mean level of IL-6 in cancer group was significantly higher than that in health group(P<0.01),but we didn't got similar result in the TNF-α(P>0.05).What's more and worthy of attention is data suggest that the immunity of cancer patients shows significant individual differences.Conclusion Using flow cytometry to monitor the dynamic changes of immune cells can help doctors understand the immune status of patients more clearly,and then provide scientific basis for them to adopt more personalized diagnosis and treatment methods.
