摘要
目的总结青岛大学附属医院近10年西弗射盾子囊霉的检出情况,明确西弗射盾子囊霉的分离培养、鉴定及药敏特征,为临床诊断及治疗提供用药依据。方法收集整理青岛大学附属医院近10年检出西弗射盾子囊霉的临床病例资料。采用常规方法接种、培养送检标本,观察分离菌的革兰染色及菌落生长特点。用VITEK 2 Compact细菌鉴定药敏仪及酵母鉴定卡和VITEK MS质谱对分离出的可疑病原真菌进行鉴定;用ATB Fungus 3进行酵母菌药敏试验。结果青岛大学附属医院2011—2021年间自临床标本中共检出8株西弗射盾子囊霉,该菌革兰染色阳性,在科玛嘉显色培养基上呈圆形、中心蓝色、边缘白色的菌落。VITEK 2 Compact和VITEK MS质谱鉴定系统均鉴定为西弗射盾子囊霉。药敏结果显示,伏立康唑敏感率100%,两性霉素B敏感率86%,氟康唑敏感率38%,5-氟胞嘧啶敏感率43%,伊曲康唑敏感率12%。结论西弗射盾子囊霉近年检出增加,其革兰染色具有念珠菌特征,在一般培养基上均可生长,在科玛嘉显色培养基上菌落生长最具特征。检出菌株对伏立康唑敏感率最高,其次是两性霉素B,对伊曲康唑敏感率最低。
Objective To summarize the detection of Stephanoascus ciferrii in the Affiliated Hospital of Qingdao University in recent 10 years,to investigate the isolation,culture,identification,and antimicrobial susceptibility characteristics of S.ciferrii,and to provide a basis for clinical diagnosis and treatment and medication.Methods The clinical data of patients in which S.ciferrii was detected in the Affiliated Hospital of Qingdao University in recent 10 years were collected.Samples from patients were inoculated and cultured by conventional methods,and the characteristics of Gram staining and colony growth of isolated fungi were observed.The isolated suspected pathogenic fungi were identified by VITEK 2 Compact Automated ID/AST Instrument and Yeast Identification Card and VITEK Mass Spectrometry(MS).ATB Fungus 3 was used in the antimicrobial susceptibility test of yeast.Results A total of eight strains of S.ciferrii were identified in clinical samples in the Affiliated Hospital of Qingdao University in 2011—2021.The strains were positive for Gram staining,and grew in round colonies with blue center and white edge on the CHROMagar colored medium.The fungus was identified as S.ciferrii by VITEK 2 Compact and VITEK MS.The results of the antimicrobial susceptibility test showed a susceptibility rate to voriconazole of 100%,to amphotericin B of 86%,to fluconazole of 38%,to 5-fluorocytosine of 43%,and to itraconazole of 12%.Conclusion The detection of S.ciferrii is increasing in recent years.Stephanoascus ciferrii shows the characteristics of candida by Gram staining,and can grow on common media.The colony grown on the CHROMagar colored medium is of typical characteristics.The detected strains show the highest susceptibility rate to voriconazole,followed by to amphotericin B,and the lowest susceptibility rate to itraconazole.
