基于斯金纳程序教学理论的健康教育对慢性心力衰竭患者自我管理行为和心肺功能的影响

Effect of Health Education Based on Skinner’s Program Teaching Theory on Self-Management Behavior and Cardiopulmonary Function in Patients with Chronic Heart Failure

摘要 目的探讨基于斯金纳程序教学理论的健康教育对慢性心力衰竭患者自我管理行为和心肺功能的影响。方法选取2019年2月至2020年8月新乡市第一人民医院收治的80例慢性心力衰竭患者作为研究对象,采用随机数表法将其分为对照组和观察组,各40例。对照组接受常规干预,观察组接受基于斯金纳程序教学理论的健康教育。比较两组自我管理行为和心肺功能。结果干预后,观察组自我护理指数量表(SCHFI)中自我护理管理、自我护理维持、自我护理信心评分均高于对照组(P<0.05)。观察组自护能力量表(ESCA)中自我概念、自护责任感、自我护理技能、健康知识水平评分均高于对照组(P<0.05)。观察组最大摄氧量(VO_(2max))高于对照组(P<0.05),最快心率、每分钟最大通气量与运动最大通气量比值(VE_(max)/MVV)低于对照组(P<0.05)。结论慢性心力衰竭患者治疗期间,开展基于斯金纳程序教学理论的健康教育,有助于改善患者自我管理行为,促进患者心肺功能恢复。 Objective To explore the effect of health education based on Skinner’s program teaching theory on self-management behavior and cardiopulmonary function of patients with chronic heart failure.Methods A total of 80 patients with chronic heart failure who were admitted to Xinxiang First People’s Hospital from February 2019 to August 2020 were selected as the research objects,and they were divided into the control group and the observation group by random number table method,with 40 cases in each group.The control group received routine intervention,and the observation group received health education based on Skinner’s procedure teaching theory.The self-management behavior and cardiopulmonary function of the two groups were compared.Results After intervention,the scores of self-care management,self-care maintenance and self-care confidence in the self-care heart failure index(SCHFI)scale in the observation group were higher than those in the control group(P<0.05).The scores of self-concept,self-care responsibility,self-care skills and health knowledge in the exercise of self-care agency scale(ESCA)in the observation group were higher than those in the control group(P<0.05).The maximal oxygen uptake(VO_(2max))in the observation group was higher than that in the control group(P<0.05),and the fastest heart rate and maximal expiratory ventilation/maximum ventilatory volume(VE_(max)/MVV)in the observation group were lower than those in the control group(P<0.05).Conclusion During the treatment of patients with chronic heart failure,carrying out health education based on Skinner’s procedure teaching theory is helpful to improve patients’self-management behavior and promote the recovery of patients’cardiopulmonary function.
