摘要
前部缺血性视神经病变(AION)是由各种原因导致眼部供血障碍引起的急性缺血性致盲性视神经眼病,严重影响中老年患者的生活质量。中医学认为,视神经属目系,与足厥阴肝经直接相连,另通过督脉和足太阳膀胱经与肾间接相通,且在五轮学说中归于瞳神隶属于肾。因此,本文以"乙癸同源"理论为指导,从肝肾失调角度探讨AION形成的病因病机,并总结出本病多表现为以肝郁气滞、肝阳上亢致目络瘀阻为主的实证和以肝肾阴虚、精血不足致目系失养为主的虚证。提出在临床治疗过程中可应用中医辨证,肝肾同治以调其本,即疏肝气兼固肾气,滋肾阴兼补肝阴、潜肝阳、降肝火,补肾精兼益肝血,同时基于经络理论辅以针刺化瘀通络以治其标。为临床应用中医药综合防治本病提供一定的参考。
Anterior ischemic optic neuropathy(AION)is an acute ischemic blindness optic neuropathy caused by various reasons.It seriously affects the quality of life of middle-aged and elderly patients.According to the theory of Traditional Chinese Medicine,the optic nerve belongs to the ocular connectors,which is directly connected with the liver meridian,and indirectly connected with the kidney through the Du Meridian and the bladder meridian,and it belongs to the pupil god and the kidney in the theory of five orbiculi.Therefore,guided by the theory of"liver and kidney being of the same source",this paper discussed the etiology and pathogenesis of AION from the perspective of liver and kidney imbalance.And it concluded that the excess syndrome main manifestation of this disease is stagnation of liver qi and hyperactivity of liver yang,which leads to obstruction of blood stasis in the eyes and collaterals,and the deficiency syndrome mainly caused by deficiency of yin of liver and kidney and deficiency of essence and blood.It suggested that in the process of clinical treatment,Traditional Chinese Medicine should be used to treat both liver and kidney in order to regulate its origin,that was soothing liver qi and securing kidney qi,nourishing kidney yin and reinforcing liver yin,subduing liver yang,downbearing liver fire,tonifying kidney essence and benefiting liver blood;At the same time,based on the theory of meridians,acupuncture should be used to remove blood stasis and dredge collaterals;It could provide some reference for clinical application of Traditional Chinese Medicine in the comprehensive prevention and treatment of this disease.
