丽水市1例巴贝西原虫感染实验室诊断与溯源分析

摘要 目的对本地不明原因发热患者血标本进行巴贝西原虫诊断、溯源与虫种鉴定。方法血涂片Giemsa染色镜检巴贝西原虫,基于18S核糖体RNA基因的物种特异性PCR反应检测虫属,将扩增子测序并与其他虫种溯源比较。结果在患者的薄血涂片红细胞中观察到核呈紫色,胞浆淡蓝色,点状、环状的可疑虫体,PCR在核酸提取物中检出巴贝西虫种,DNA测序和溯源显示其分类地位与田鼠巴贝西原虫(Babesia microti)同源。结论根据患者的临床表现、实验室检测结果和流行病学调查,患者为本地蜱虫叮咬而引起的田鼠巴贝西原虫自然感染。 Objective To diagnose,trace and identify Babesia from the blood specimens of a local patient with fever unknown origin( FUO). Methods The blood smear stained with Giemsa stains was detected with microscope method for Babesia,and the species specific PCR based on the 18 S ribosome RNA gene was used to detect the parasite. The amplification was sequenced,and compared with the traceability of other species. Results In the patient’s RBC of thin blood smear,suspicious insect was found to exhibit purple nucleus,light blue cytoplasm,and dot-like and ringed body. Babesia species was detected out by PCR in the extract of nucleic acid. DNA sequencing and traceability showed it was homologous to Babesia microti in classification. Conclusion According to the clinical manifestation,laboratory results and epidemiological investigation,the patient was caused by the local ticks biting.
