摘要
目的探究急性脑梗死患者血清脂联素、白细胞介素-8(IL-8)水平与颈动脉粥样硬化斑块稳定性的相关性。方法选取急性脑梗死患者198例(急性脑梗死组),均行颈动脉彩色多普勒超声和高分辨率MRI检查,根据颈动脉粥样硬化斑块形成情况进一步分为无斑块组(n=51)、稳定性斑块组(n=89)和不稳定斑块组(n=58);并选取同期健康体检者50例作为对照组。比较各组血清脂联素及IL-8水平。结果急性脑梗死组的稳定斑块和不稳定斑块所占比例明显大于对照组(P<0.05).急性脑梗死组脂联素水平低于对照组,而1L-8水平高于对照组(P<0.05)。校正其他影响颈动脉粥样硬化斑块稳定性的相关因素,经多元线性回归分析,脂联素低水平及IL-8高水平均是影响颈动脉粥样硬化斑块稳定性的相关因素(P<0.05).结论血清脂联素及IL-8水平可初步作为急性脑梗死患者颈动脉粥样硬化斑块稳定性的预估指标,血清脂联素水平的降低以及IL-8水平的增加可能预示颈动脉粥样硬化斑块稳定性下降。
Objective To investigate the correlation between the stability of carotid artherosclerotic plaque and the levels of serum adopinectin and interleukin-8(IL-8)in patients with acute cerebral infraction(ACI).Methods A total of 198 ACI patients were selected as ACI group and underwent carotid artery color Doppler ultrasound and high resolution MRI examinations.According to the formation of carotid atherosclerotic plaque,patients in ACI group were further divided into noplaque group(n=51),stable plaque group(n=89)and unstable plaque group(n=58).At the same period,50 healthy subjects were selected as control group.The levels of serum adiponectin and IL-8 were compared among the various groups.Results The proportion of stable and unstable plaques in the ACI group was significantly higher than that in the control group(P<0.05).The level of adiponectin in the ACI group was lower than that in the control group,while the level of IL-8 was higher than that in the control group(P<0.05).After correcting other factors that affect the stability of carotid atherosclerotic plaque,multivariate linear regression analysis showed that low adiponectin level and high IL-8 level were the related factors affecting the stability of carotid atherosclerotic plaque(P<0.05).Conclusions Serum adiponectin and IL-8 levels can be used as preliminary predictors of carotid atherosclerotic plaque stability in patients with ACL Decreased serum adiponectin levels and in creased IL-8 levels may indicate decreased stability of carotid atherosclerotic plaques.
