摘要 目的研究QT离散度(QTd)与急性心肌梗死(AMI)患者预后评估的相关性。方法对确诊的110例AMI患者(治疗组),于入院第5~10天,行经皮冠状动脉介入治疗(PCI);并与100例同期冠状动脉造影术(CAG)结果正常者(对照组)进行比较,对照分析术前1天和术后1天的标准十八导联同步心电图QTd及随访发病30天内的主要心血管事件(MACE)。结果治疗组与对照组相比,PCI前,QTd明显延长;发病30天内,MACE发生率增加(P〈0.05)。经PCI治疗,AMI患者较术前QTd缩短(P〈0.05)。AMI患者中,MACE发生组与未发生MACE组相比,术后较术前QTd降低幅度较小(P〈0.05)。结论 AMI患者QTd明显高于正常人,成功的PCI,能显著减小AMI患者QTd;QTd及其变化程度,可作为评价心肌灌注及近期预后的预测指标。 Objective To investigate the correlation between QT dispersion(QTd) and prognosis in patients with acute myocardial infarction(AMI).Methods A total of 110 patients with AMI received percutaneous coronary intervention(PCI) 5-10 days after admission.QTd was measured one day before and after PCI then compared with those of control group whose cardioangiography(CAG) was negative.Follow up major adverse cardiovascular events(MACE) within 30 days.Results QTd was longer in patients with AMI than control group,while the rate of MACE within 30 days was higher than in the control group(P0.05).Compared with before PCI,the QTd in the AMI group had shorter than after PCI.Reduction of QTd of the patients with MACE in the AMI group was less significant than that of the others in the same group(P0.05).Conclusion QTd was longer in patients with AMI.Successful PCI may notably reduce QTd.QTd and its reduction are related to myocardial perfusion and recent prognosis.
