Effects of BiPAP Mechanical Ventilation by Nasal Mask on Acute Cardiac Pulmonary Edema Patients

摘要 目的探讨BiPAP面罩式双向正压通气治疗急性心源性肺水肿的临床疗效。方法将80例急性心源性肺水肿患者分为两组。BiPAP组40例,在常规治疗的基础上采用BiPAP呼吸机经面罩式双向正压通气治疗;对照组40例采用常规治疗。观察治疗前后两组患者血气分析、血压、心率和呼吸频率的变化。结果治疗后BiPAP组呼吸频率、心率、PaCO2均低于对照组,PaO2和SaO2均高于对照组(t=2.69~6.89,P〈0.01);BiPAP组治疗总有效率为100.00%,对照组在相应时间治疗总有效率仅为65.54%。结论与常规治疗相比,BiPAP面罩式双向正压机械通气可迅速改善患者的心功能,并更快改善肺水肿和低氧血症,对急性心源性肺水肿有较强的适应指征,特别是对洋地黄制剂有禁忌的急性心源性肺水肿应尽早使用。 Objective To investigate the effect of bilevel positive airway pressure (BiPAP) mechanical ventilation by nasal mask on acute cardiac pulmonary edema (ACPE). Methods This study included 80 patients with acute cardiac pulmonary edema. 40 patients received BiPAP mechanical ventilation treatment (BiPAP group)and another 40 patients received only general treatments (control group). Before and after the treatment, the blood gas index, blood pressure (BP), heart rate (HR) and breathing rate (RR) were recorded. Results Breathing rate, heart rate and PaCO2 were found to be significantly lower in BiPAP group, and PaO2 and SaO2 were significantly higher (t=2.69-6.89, P〈0.01). The total effective rate in BiPAP group was 100.00%, while only 64.54% in the control group. Conclusion Comparing with conventional therapies, BiPAP mechanical ventilation can more quickly improve cardiac function, pulmonary edema and hypoxemia of patients with ACPE.
