摘要
目的:应用超声射频信号血管内中膜分析技术(Quality Intima-Media Thickness,QIMT),研究自然人群双侧颈总动脉内径及IMT的变化,判断危险因素对双侧颈总动脉结构功能影响的异同性。方法:参照中国动脉僵硬度评价研究方案,433名畲族人来自福建省福安市城乡自然人群,年龄15~87岁[(49.03±13.54)岁]。应用QIMT技术和常规二维超声分别观察记录静息状态下双侧颈总动脉。测量舒张末期内径、IMT,比较左右侧颈动脉管径、IMT在各年龄段之间的差异和趋势。并分析其与年龄、性别、BMI、PP、TC、TG等危险因素之间的关联。结果:(1)左右侧颈动脉管径和IMT随年龄增加而增厚,趋势检验有显著差异(P<0.01)。年龄大于35岁,左侧颈总动脉IMT较右侧总动脉IMT增厚,左右侧颈总动脉IMT间差异有统计学意义(P<0.05)。(2)左侧颈总动脉IMT与BMI、PP、臀围、TC、LDL-C和血糖呈现不同程度的正相关(P<0.05~0.01)。右侧颈总动脉IMT仅和性别、PP和血糖相关(P<0.05)。左侧颈总动脉内径与女性、臀围呈正相关(P<0.05),右侧颈总动脉内径与性别呈正相关(P<0.05)。结论:畲族自然人群双侧颈动脉IMT和管径随年龄变化存在差异,其差异与年龄、性别、脉压、体重指数、臀围、腹围、TC、LDL-C和血糖明显相关。这种血管结构和功能的改变将为临床防治心血管疾病提供重要的参考。
Objective:To study the changes of diameters and IMT in ambi-common carotid arteries (ambi-CCA) and their correlation factors by Quality Intima-Media Thickness (QIMT).Methods:According to Chinese arterial stiffness evaluation study (CASE) project,the inner diameter and IMT of (ambi-CCA) were measured by QIMT and 2-D ultrasound respectively in 433 national She people with age of 15 y 87(mean 49.03±13.54).The difference and tendency were analyzed on age,gender,BMI,PP, TC,TG,and so on.Results:(1) The significant positive correlations were found between ambi-CCA,IMT and age(P < 0.01).The left CCA IMT was thicker than right one in > 35 years old group (P < 0.05).(2) The left CCA IMT was positively associated with BMI.PP,hip circumference,TC,LDL-C,and BG respectively(P <0.05 ~0.01).The right CCA IMT was only positively associated with gender,PP,and BG(P <0.05).The inner diameter of left CCA was positive correlation with gender and hip circumference (P < 0.05).The inner diameter of right CCA was positive correlation with gender(P < 0.05).Conclusions:There were differences of IMT and inner diameter of CCA in age and position.They were correlation with age,gender,pulse pressure,body mass index,hip circumference,abdomen circumference,TC,LDL-C,and GB.The difference of ambi-CCA should be paid attention.
