摘要
目的:研究补肾健骨活血汤对骨质疏松性人工髋关节置换术后假体周围骨密度及骨转化指标的影响。方法:选取我院86例行人工髋关节置换术患者为研究对象,按照随机分组法分为治疗组和对照组各43例,对照组予以术后常规治疗,治疗组在其基础上加以补肾健骨活血汤治疗。比较两组临床疗效、治疗前后中医证候积分、骨密度、髋关节功能、骨转化指标[血清碱性磷酸酶(ALP)、I型前胶原氨基端前肽(PINP)、骨钙素(BGP)]水平及生活质量变化情况。结果:治疗3个月后,治疗组疗效显著优于对照组(P<0.05),两组患者中医症候均明显减轻(P<0.05),治疗组患者骨密度、Harris髋关节评分表评分均显著提高(P<0.05),两组骨转化指标中,ALP、PINP、BGP水平均显著提高(P<0.05),生活质量评定量表~74(GQOL-74)评分显著提高(P<0.05),且各评估指标治疗后组间对比差异显著(P<0.05)。结论:补肾健骨活血汤可有效助益人工髋关节置换术后患者的治疗效果,提高患者骨密度及髋关节功能,改善患者生活质量,提高骨转化指标水平。
Objective:To study the effect of Bushen Jiangu Huoxue Decoction on bone mineral density and bone turnover indexes around prosthesis after artificial hip replacement for osteoporosis.Methods:Totally 86 patients undergoing artificial hip replacement in our hospital were selected as the research subjects,and they were divided into a treatment group and a controlled group according to the random grouping method,with 43 cases in each group.The controlled group received routine postoperative treatment,and the treatment group was treated with Bushen Jiangu Huoxue Decoction on the basis of the controlled group.The clinical efficacy,and TCM syndromes scores,bone mineral density,hip joint function,bone turnover indexes[serum alkaline phosphatase(ALP),aminoterminal propeptide of type I procollagen(PINP),bone gla protein(BGP)]and quality of life before and after treatment were compared between the two groups.Results:After 3 months of treatment,the efficacy in the treatment group was significantly better than that in the controlled group(P<0.05),and the TCM symptoms of the two groups were significantly relieved(P<0.05),and the bone mineral density and Harris hip scale score in the treatment group were significantly improved(P<0.05),and the levels of bone turnover indexes of ALP,PINP and BGP were significantly improved(P<0.05),and the scores of general quality of life inventory-74(GQOL-74)were significantly increased(P<0.05),and there were significant differences in the above indexes between the groups after treatment(P<0.05).Conclusion:Bushen Jiangu Huoxue Decoction can effectively help improve the treatment effects,increase the bone mineral density and hip joint function,improve the quality of life and enhance the levels of bone turnover indexes of patients after artificial hip replacement.
