This paper presents an alternative representation of a system of differential equations qualitatively showing the behavior of the biological rhythm of a crayfish during their transition from juvenile to adult stages. ...This paper presents an alternative representation of a system of differential equations qualitatively showing the behavior of the biological rhythm of a crayfish during their transition from juvenile to adult stages. The model focuses on the interaction of four cellular oscillators coupled by diffusion of a hormone, a parameter μ is used to simu- late the quality of communication among the oscillators, in biological terms, it mea- sures developmental maturity of the crayfish. Since some quorum-sensing mechanism is assumed to be responsible for the synchronization of the biological oscillators, it is nat- ural to investigate the possibility that the underlying diffusion process is not standard, i.e. it may be a so-called anomalous diffusion. In this case, it is well understood that diffusion equations with fractional derivatives describe these processes in a more realis- tic way. The alternative formulation of these equations contains fractional operators of Liouville-Caputo and Caputo-Fabrizio type. The numerical simulations of the equations reflect synchronization of ultradian rhythms leading to a circadian rhythm. The classical behavior is recovered when the order of the fractional derivative is V = 1. We discuss possible biological implications.展开更多

Liver cancers are the second most frequent cause of global cancer-related mortality of which 90%are attributable to hepatocellular carcinoma(HCC).Despite the advent of screening programmes for patients with known risk...Liver cancers are the second most frequent cause of global cancer-related mortality of which 90%are attributable to hepatocellular carcinoma(HCC).Despite the advent of screening programmes for patients with known risk factors,a substantial number of patients are ineligible for curative surgery at presentation with limited outcomes achievable with systemic chemotherapy/external radiotherapy.This has led to the advent of numerous minimally invasive options including but not limited to trans-arterial chemoembolization,radiofrequency/microwave ablation and more recently selective internal radiation therapy many of which are often the first-line treatment for select stages of HCC or serve as a conduit to liver transplant.The authors aim to provide a comprehensive overview of these various image guided minimally invasive therapies with a brief focus on the technical aspects accompanied by a critical analysis of the literature to assess the most up-to-date evidence from comparative systematic reviews and meta-analyses finishing with an assessment of novel combination regimens and future directions of travel.展开更多

Due to its significant impact, it is imperative to understand the risk stratification and preventative strategies for PHLF. The main objective of this review is to highlight the role of these strategies in a timeline centered way around curative resection. Data sources: This review includes studies on both humans and animals, where they addressed PHLF. A literature search was conducted across the Cochrane Library, Embase, MEDLINE/Pub Med, and Web of Knowledge electronic databases for English language studies published between July 1997 and June 2020. Studies presented in other languages were equally considered. The quality of included publications was assessed using Downs and Black’s checklist. The results were presented in qualitative summaries owing to the lack of studies qualifying for quantitative analysis. Results: This systematic review with 245 studies, provides insight into the current prediction, prevention, diagnosis, and management options for PHLF. This review highlighted that liver volume manipulation is the most frequently studied preventive measure against PHLF in clinical practice, with modest improvement in the treatment strategies over the past decade. Conclusions: Remnant liver volume manipulation is the most consistent preventive measure against PHLF.展开更多
Background:Severe acute pancreatitis is a common diagnosis in emergency general surgery and can be a cause of significant morbidity and mortality.A consequence of severe acute pancreatitis is thrombus in the splanchni...Background:Severe acute pancreatitis is a common diagnosis in emergency general surgery and can be a cause of significant morbidity and mortality.A consequence of severe acute pancreatitis is thrombus in the splanchnic veins.These thrombi can potentially lead to bowel ischemia or hepatic failure.However,another complication of severe acute pancreatitis is retroperitoneal bleeding.At this time,it is unclear if treating patients for splanchnic vein thrombosis in the context of severe acute pancreatitis is associated with any outcome benefit.A systematic review might clarify this question.Data sources:A two-fold search strategy(one broad and one precise)looked at all published literature.The review was registered on PROSPERO(ID:CRD42018102705).MEDLINE,EMBASE,PubMed,Cochrane and Web of Science databases were searched and potentially relevant papers were reviewed indepen-dently by two researchers.Any disagreement was reviewed by a third independent researcher.Primary outcome was reestablishment of flow in the thrombosed vein versus bleeding complications.Results:Of 1462 papers assessed,a total of 16 papers were eligible for inclusion.There were no ran-domized controlled trials,2 were case series,5 retrospective single-center studies and 9 case reports.There were a total of 198 patients in these studies of whom 92(46.5%)received anticoagulation therapy.The rates of recanalization of veins in the treated and non-treated groups was 14%and 11%and bleeding complications were 16%and 5%,respectively.However,the included studies were too heterogeneous to undertake a meta-analysis.Conclusions:The systematic review highlights the lack evidence addressing this clinical question.There-fore a randomized controlled trial would be appropriate to undertake.展开更多

Radiotherapy(RT) after tumorectomy in early breast cancer patients is an established treatment modality which conventionally takes 6-7 wk to complete.Shorter RT schedules have been tested in large multicentre randomiz...Radiotherapy(RT) after tumorectomy in early breast cancer patients is an established treatment modality which conventionally takes 6-7 wk to complete.Shorter RT schedules have been tested in large multicentre randomized trials and have shown equivalent results to that of standard RT(50 Gy in 25 fractions) in terms of local tumor control,patient survival and late post-radiation effects.Some of those trials have now completed 10 years of follow-up with encouraging results for treatments of 3-4 wk and a total RT dose to the breast of 40-42.5 Gy with or without boost.A reduction of 50% in treatment time makes those RT schedules attractive for both patients and health care providers and would have a significant impact on daily RT practice around the world,as it would accelerate patient turnover and save health care resources.However,in hypofractionated RT,a higher(than the conventional 1.8-2 Gy) dose per fraction is given and should be managed with caution as it could result in a higher rate of late postradiation effects in breast,heart,lungs and the brachial plexus.It is therefore advisable that both possible dose inhomogeneity and normal tissue protection should be taken into account and the appropriate technology such as three-dimensional/intensity modulated radiation therapy employed in clinical practice,when hypofractionation is used.展开更多
Arsenic is one of the main harmful elements in industrial wastewater.How to remove arsenic has always been one of the research hotspots in academic circles.In the process of arsenic removal by traditional sulfuration,...Arsenic is one of the main harmful elements in industrial wastewater.How to remove arsenic has always been one of the research hotspots in academic circles.In the process of arsenic removal by traditional sulfuration,the use of traditional sulfurizing agent will introduce new metal cations,which will affect the recycling of acid.In this study,phosphorus pentasulfide (P_(2)S_(5)) was used as sulfurizing agent,which hydrolyzed to produce H_(3)PO_(4) and H_(2)S without introducing new metal cations.The effect of ultrasound on arsenic removal by P_(2)S_(5) was studied.Under the action of ultrasound,the utilization of P_(2)S_(5) was improved and the reaction time was shortened.The effects of S/As molar ratio and reaction time on arsenic removal rate were investigated under ultrasonic conditions.Ultrasonic enhanced heat and mass transfer so that the arsenic removal rate of 94.5%could be achieved under the conditions of S/As molar ratio of 2.1:1 and reaction time of 20 min.In the first 60 min,under the same S/As molar ratio and reaction time,the ultrasonic hydrolysis efficiency of P_(2)S_(5) was higher.This is because P_(2)S_(5) forms ([(P_(2)S_(4))])^(2+)under the ultrasonic action,and the structure is damaged,which is easier to be hydrolyzed.In addition,the precipitation after arsenic removal was characterized and analyzed by X-ray diffraction,scanning electron microscope-energy dispersive spectrometer,X-ray fluorescence spectrometer and X-ray photoelectron spectroscopy.Our research avoids the introduction of metal cations in the arsenic removal process,and shortens the reaction time.展开更多
The extensive Changba-Lijiagou Pb-Zn deposit is located in the north of the Xihe–Chengxian ore cluster in West Qinling. The ore bodies are mainly hosted in the marble, dolomitic marble and biotite-calcite-quartz schi...The extensive Changba-Lijiagou Pb-Zn deposit is located in the north of the Xihe–Chengxian ore cluster in West Qinling. The ore bodies are mainly hosted in the marble, dolomitic marble and biotite-calcite-quartz schist of the Middle Devonian Anjiacha Formation, and are structurally controlled by the fault and anticline. The ore-forming process can be divided into three main stages, based on field geological features and mineral assemblages. The mineral assemblages of hydrothermal stage I are pale-yellow coarse grain, low Fe sphalerite, pyrite with pits, barite and biotite. The mineral assemblages of hydrothermal stage II are black-brown cryptocrystalline, high Fe shalerite, pyrite without pits, marcasite or arsenopyrite replace the pyrite with pits, K-feldspar. The features of hydrothermal stage III are calcite-quartz-sulfide vein cutting the laminated, banded ore body. Forty-two sulfur isotope analyses, twenty-five lead isotope analyses and nineteen carbon and oxygen isotope analyses were determined on sphalerite, pyrite, galena and calcite. The δ34 S values of stage I(20.3 to 29.0‰) are consistent with the δ34 S of sulfate(barite) in the stratum. Combined with geological feature, inclusion characteristics and EPMA data, we propose that TSR has played a key role in the formation of the sulfides in stage I. The δ34 S values of stage II sphalerite and pyrite(15.1 to 23.0‰) are between sulfides in the host rock, magmatic sulfur and the sulfate(barite) in the stratum. This result suggests that multiple S reservoirs were the sources for S2-in stage II. The δ34 S values of stage III(13.1 to 22‰) combined with the structure of the geological and mineral features suggest a magmatic hydrothermal origin of the mineralization. The lead isotope compositions of the sulfides have 206 Pb/204 Pb ranging from 17.9480 to 17.9782, 207 Pb/204 Pb ranging from 15.611 to 15.622, and 208 Pb/204 Pb ranging from 38.1368 to 38.1691 in the three ore-forming stages. There are reports of osteal fat necrosis leading to periosteal reactions and osteolytic lesions following severe pancreatitis, particularly in long bones. A 54-year-old man was admitted to our hospital with acute pancretitis, who later developed spinal discitis secondary to necrotizing pancreatitis. He was treated conservatively with antibiotics and after a month he recovered completely without any neurological deficit. The ...Research and critique of unbuilt or destroyed works of architecture is traditionally carried out through the examination of surviving information such as drawings, models, photographs, biographies and monographs. The case study presented here demonstrates that this approach cannot always give a fuU-picture of the architect or designer's intentions, and may miss inconsistencies in the design and links to other precedents or antecedents in such schemes. Here, we employ strategic contemporary digital representation techniques to re-present and re-analyse the evidence available for a particular architectural project. We describe a systematic methodology, which shows that these techniques can challenge or enhance current understanding. The focus therefore is on the digital re-analysis process that has been devised. Sir Edwin Lutyens' unbuilt Liverpool Metropolitan Cathedral design, that would have delivered one of the largest cathedrals in the world, is used as a case study. The findings reveal new information about the cathedral by following structured lines of enquiry generated from the study of primary and secondary source data, as well as serendipitous results that occur as a potential by-product of the methodological process.展开更多
Aim:The aim of this study is to compare disease-free survival(DFS)and overall survival(OS)in patients with stage I cervical cancer(≤4cms,lymph node-negative)undergoing open radical hysterectomy(ORH)vs.minimally invas...Aim:The aim of this study is to compare disease-free survival(DFS)and overall survival(OS)in patients with stage I cervical cancer(≤4cms,lymph node-negative)undergoing open radical hysterectomy(ORH)vs.minimally invasive radical hysterectomy(MIRH).Methods:All patients undergoing radical hysterectomy between January 2012-December 2018 from the largest tertiary referral cancer centre were included.A 1:1 propensity matching was done based on four independent prognostic factors to compare DFS and OS with the route of surgery.Results:One hundred and ninety-nine patients were included during the study period.The median age of the cohort was 50 years.The median follow-up of patients was 47 months.Following 1:1 propensity matching,a total of 174 patients were analysed for DFS and OS in ORH(n=87)and MIRH(n=87)groups.Protective measure was used in two-thirds of the patients during MIRH.Twenty-nine patients(16.7%)had recurrences.For the matched cohort(n=174),the DFS at 36 and 60 months was 84.8%(78.1%-89.6%)and 81%(73.4%-86.6%)respectively and the OS was 96.5%(91.7%-98.5%)and 95.6%(90.3%-98%)respectively.There was no statistically significant difference in DFS or OS between ORH and MIRH.Conclusion:The present study showed no difference in oncological outcomes in MIRH compared to ORH.Retrospective audits on patient characteristics such as screening/vaccination history along with surgical technique/load and matching for crucial risk factors should be factored in future studies to eliminate the possible methodological errors.展开更多

文摘Background:Severe acute pancreatitis is a common diagnosis in emergency general surgery and can be a cause of significant morbidity and mortality.A consequence of severe acute pancreatitis is thrombus in the splanchnic veins.These thrombi can potentially lead to bowel ischemia or hepatic failure.However,another complication of severe acute pancreatitis is retroperitoneal bleeding.At this time,it is unclear if treating patients for splanchnic vein thrombosis in the context of severe acute pancreatitis is associated with any outcome benefit.A systematic review might clarify this question.Data sources:A two-fold search strategy(one broad and one precise)looked at all published literature.The review was registered on PROSPERO(ID:CRD42018102705).MEDLINE,EMBASE,PubMed,Cochrane and Web of Science databases were searched and potentially relevant papers were reviewed indepen-dently by two researchers.Any disagreement was reviewed by a third independent researcher.Primary outcome was reestablishment of flow in the thrombosed vein versus bleeding complications.Results:Of 1462 papers assessed,a total of 16 papers were eligible for inclusion.There were no ran-domized controlled trials,2 were case series,5 retrospective single-center studies and 9 case reports.There were a total of 198 patients in these studies of whom 92(46.5%)received anticoagulation therapy.The rates of recanalization of veins in the treated and non-treated groups was 14%and 11%and bleeding complications were 16%and 5%,respectively.However,the included studies were too heterogeneous to undertake a meta-analysis.Conclusions:The systematic review highlights the lack evidence addressing this clinical question.There-fore a randomized controlled trial would be appropriate to undertake.
文摘Dear Editor,Clear cell renal cell carcinoma(ccRCC)is a highly vascular tumor.Among surrogate markers that may reflect the neovasculature burden of ccRCC,prostate specific membrane antigen(PSMA)has emerged as a promising one,by virtue of its ability to be measured either immunohistochemically and transcriptionally or on positron emission tomography(PET)imaging[1,2].While the relationship between PSMA and neoangiogenesis in ccRCC is well established,the potential association between PSMA and the immune landscape of ccRCC is much less understood.
文摘Radiotherapy(RT) after tumorectomy in early breast cancer patients is an established treatment modality which conventionally takes 6-7 wk to complete.Shorter RT schedules have been tested in large multicentre randomized trials and have shown equivalent results to that of standard RT(50 Gy in 25 fractions) in terms of local tumor control,patient survival and late post-radiation effects.Some of those trials have now completed 10 years of follow-up with encouraging results for treatments of 3-4 wk and a total RT dose to the breast of 40-42.5 Gy with or without boost.A reduction of 50% in treatment time makes those RT schedules attractive for both patients and health care providers and would have a significant impact on daily RT practice around the world,as it would accelerate patient turnover and save health care resources.However,in hypofractionated RT,a higher(than the conventional 1.8-2 Gy) dose per fraction is given and should be managed with caution as it could result in a higher rate of late postradiation effects in breast,heart,lungs and the brachial plexus.It is therefore advisable that both possible dose inhomogeneity and normal tissue protection should be taken into account and the appropriate technology such as three-dimensional/intensity modulated radiation therapy employed in clinical practice,when hypofractionation is used.

文摘Arsenic is one of the main harmful elements in industrial wastewater.How to remove arsenic has always been one of the research hotspots in academic circles.In the process of arsenic removal by traditional sulfuration,the use of traditional sulfurizing agent will introduce new metal cations,which will affect the recycling of acid.In this study,phosphorus pentasulfide (P_(2)S_(5)) was used as sulfurizing agent,which hydrolyzed to produce H_(3)PO_(4) and H_(2)S without introducing new metal cations.The effect of ultrasound on arsenic removal by P_(2)S_(5) was studied.Under the action of ultrasound,the utilization of P_(2)S_(5) was improved and the reaction time was shortened.The effects of S/As molar ratio and reaction time on arsenic removal rate were investigated under ultrasonic conditions.Ultrasonic enhanced heat and mass transfer so that the arsenic removal rate of 94.5%could be achieved under the conditions of S/As molar ratio of 2.1:1 and reaction time of 20 min.In the first 60 min,under the same S/As molar ratio and reaction time,the ultrasonic hydrolysis efficiency of P_(2)S_(5) was higher.This is because P_(2)S_(5) forms ([(P_(2)S_(4))])^(2+)under the ultrasonic action,and the structure is damaged,which is easier to be hydrolyzed.In addition,the precipitation after arsenic removal was characterized and analyzed by X-ray diffraction,scanning electron microscope-energy dispersive spectrometer,X-ray fluorescence spectrometer and X-ray photoelectron spectroscopy.Our research avoids the introduction of metal cations in the arsenic removal process,and shortens the reaction time.

文摘The extensive Changba-Lijiagou Pb-Zn deposit is located in the north of the Xihe–Chengxian ore cluster in West Qinling. The ore bodies are mainly hosted in the marble, dolomitic marble and biotite-calcite-quartz schist of the Middle Devonian Anjiacha Formation, and are structurally controlled by the fault and anticline. The ore-forming process can be divided into three main stages, based on field geological features and mineral assemblages. The mineral assemblages of hydrothermal stage I are pale-yellow coarse grain, low Fe sphalerite, pyrite with pits, barite and biotite. The mineral assemblages of hydrothermal stage II are black-brown cryptocrystalline, high Fe shalerite, pyrite without pits, marcasite or arsenopyrite replace the pyrite with pits, K-feldspar. The features of hydrothermal stage III are calcite-quartz-sulfide vein cutting the laminated, banded ore body. Forty-two sulfur isotope analyses, twenty-five lead isotope analyses and nineteen carbon and oxygen isotope analyses were determined on sphalerite, pyrite, galena and calcite. The δ34 S values of stage I(20.3 to 29.0‰) are consistent with the δ34 S of sulfate(barite) in the stratum. Combined with geological feature, inclusion characteristics and EPMA data, we propose that TSR has played a key role in the formation of the sulfides in stage I. The δ34 S values of stage II sphalerite and pyrite(15.1 to 23.0‰) are between sulfides in the host rock, magmatic sulfur and the sulfate(barite) in the stratum. This result suggests that multiple S reservoirs were the sources for S2-in stage II. The δ34 S values of stage III(13.1 to 22‰) combined with the structure of the geological and mineral features suggest a magmatic hydrothermal origin of the mineralization. The lead isotope compositions of the sulfides have 206 Pb/204 Pb ranging from 17.9480 to 17.9782, 207 Pb/204 Pb ranging from 15.611 to 15.622, and 208 Pb/204 Pb ranging from 38.1368 to 38.1691 in the three ore-forming stages. 文摘This study applies OLS,panel regression and Granger causality test to investigate the impact of the Coronavirus disease 2019(Covid-19)outbreak on the global equity markets during the early stage of the pandemic.We find that the Covid-19 outbreak has a significant negative impact on the overall equity index return of the eight economies even at 0.1%significance level.Furthermore,the pandemic has a more significant impact on the European countries than on the East Asian economies.The results have three main implications.Firstly,policy makers should react fast to mitigate the impact of a crisis.Secondly,investors should be aware of an outbreak of disease or other risks and adjust their investments accordingly.Furthermore,the Covid-19 outbreak results in a shift of power from the west to the east.
文摘As the global burden of diabetes is rapidly increasing,the incidence of diabetic foot ulcers is continuously increasing as the mean age of the world population increases and the obesity epidemic advances.A significant percentage of diabetic foot ulcers are caused by mixed micro and macro-vascular dysfunction leading to impaired perfusion of foot tissue.Left untreated,chronic limb-threatening ischemia has a poor prognosis and is correlated with limb loss and increased mortality;prompt treatment is required.In this review,the diagnostic challenges in diabetic foot disease are discussed and available data on minimally invasive treatment options such as endovascular revascularization,stem cells,and gene therapy are examined.
文摘Acute pancreatitis constitutes 3% of all admissions with abdominal pain. There are reports of osteal fat necrosis leading to periosteal reactions and osteolytic lesions following severe pancreatitis, particularly in long bones. A 54-year-old man was admitted to our hospital with acute pancretitis, who later developed spinal discitis secondary to necrotizing pancreatitis. He was treated conservatively with antibiotics and after a month he recovered completely without any neurological deficit. This case is reported for its unusual and unreported spinal complications after acute pancreatitis.

文摘Research and critique of unbuilt or destroyed works of architecture is traditionally carried out through the examination of surviving information such as drawings, models, photographs, biographies and monographs. The case study presented here demonstrates that this approach cannot always give a fuU-picture of the architect or designer's intentions, and may miss inconsistencies in the design and links to other precedents or antecedents in such schemes. Here, we employ strategic contemporary digital representation techniques to re-present and re-analyse the evidence available for a particular architectural project. We describe a systematic methodology, which shows that these techniques can challenge or enhance current understanding. The focus therefore is on the digital re-analysis process that has been devised. Sir Edwin Lutyens' unbuilt Liverpool Metropolitan Cathedral design, that would have delivered one of the largest cathedrals in the world, is used as a case study. The findings reveal new information about the cathedral by following structured lines of enquiry generated from the study of primary and secondary source data, as well as serendipitous results that occur as a potential by-product of the methodological process.
文摘Aim:The aim of this study is to compare disease-free survival(DFS)and overall survival(OS)in patients with stage I cervical cancer(≤4cms,lymph node-negative)undergoing open radical hysterectomy(ORH)vs.minimally invasive radical hysterectomy(MIRH).Methods:All patients undergoing radical hysterectomy between January 2012-December 2018 from the largest tertiary referral cancer centre were included.A 1:1 propensity matching was done based on four independent prognostic factors to compare DFS and OS with the route of surgery.Results:One hundred and ninety-nine patients were included during the study period.The median age of the cohort was 50 years.The median follow-up of patients was 47 months.Following 1:1 propensity matching,a total of 174 patients were analysed for DFS and OS in ORH(n=87)and MIRH(n=87)groups.Protective measure was used in two-thirds of the patients during MIRH.Twenty-nine patients(16.7%)had recurrences.For the matched cohort(n=174),the DFS at 36 and 60 months was 84.8%(78.1%-89.6%)and 81%(73.4%-86.6%)respectively and the OS was 96.5%(91.7%-98.5%)and 95.6%(90.3%-98%)respectively.There was no statistically significant difference in DFS or OS between ORH and MIRH.Conclusion:The present study showed no difference in oncological outcomes in MIRH compared to ORH.Retrospective audits on patient characteristics such as screening/vaccination history along with surgical technique/load and matching for crucial risk factors should be factored in future studies to eliminate the possible methodological errors.
