METHODS Patients with severe...AIM To investigate the efficacy and safety of a combination of sufentanil and propofol injection in patients undergoing endoscopic injection sclerotherapy(EIS) for esophageal varices(EVs). METHODS Patients with severe EVs who underwent EIS with sufentanil and propofol anesthesia between April 2016 and July 2016 at our hospital were reviewed. Although EIS and sequential therapy were performed under endotracheal intubation, we only evaluated the efficacy and safety of anesthesia for the first EIS procedure. Patients were intravenously treated with 0.5-1 μg/kg sufentanil. Anesthesia was induced with 1-2 mg/kg propofol and maintained using 2-5 mg/kg per hour of propofol. Information, regarding age, sex, weight, American Association of Anesthesiologists(ASA) physical status, Child-Turcotte-Pugh(CTP) classification, indications, preanesthetic problems, endoscopic procedure, successful completion of the procedure, anesthesia time, recovery time, and anesthetic agents, was recorded. Adverse events, including hypotension, hypertension, bradycardia, and hypoxia, were also noted.RESULTS Propofol and sufentanil anesthesia was provided in 182 procedures involving 140 men and 42 women aged 56.1± 11.7 years(range, 25-83 years). The patients weighed 71.4 ± 10.7 kg(range, 45-95 kg) and had ASA physical status classifications of Ⅱ(79 patients) or Ⅲ(103 patients). Ninety-five patients had a CTP classification of A and 87 had a CTP classification of B. Intravenous anesthesia was successful in all cases. The mean anesthesia time was 33.1 ± 5.8 min. The mean recovery time was 12.3 ± 3.7 min. Hypotension occurred in two patients(1.1%, 2/182). No patient showed hypertension during the endoscopic therapy procedure. Bradycardia occurred in one patient(0.5%, 1/182), and hypoxia occurred in one patient(0.5%, 1/182). All complications were easily treated with no adverse sequelae. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. Online and paper documents available at the University of Technology Sydney (UTS) and the Chengdu University of Traditional Chinese Medicine (CDUTCM) were read and analyzed. In addition, in-depth interviews with academics from the two universities were conducted during 2014 to 2015.RESULTS: The two Chinese medicine programs share the common goal of providing health services to the local community, but differ in some aspects when the curricula are compared. Areas such as student profile, curriculum structure, teaching approaches and education quality assurance were found to be different. The UTS program adopts a "flipped learning" approach with the use of educational technology aiming at improving learning outcomes. On the other hand, the CDUTCM has better clinical facilities and specialist physician resources.CONCLUSION: A better understanding of the different curricula and approaches to Chinese medicine education will facilitate student learning and educational outcomes.展开更多

Methods: SCL-90, EPQ-N and cognitive behavior intervention were performed on 30 patients with PTSD, 30 patients without PTSD and 30 normal subjects, and their fasting blood was collected to analyze cortisol in the morning. Results: The scores of SCL-90 and EPQ-N in patients with breast cancer PTSD were significantly higher than those in non-PTSD patients and normal subjects (P < 0.01). After cognitive behavioral intervention, the plasma cortisol level of breast cancer PTSD patients was significantly decreased (P Conclusion: Compared with non-PTSD breast cancer patients and normal control group, breast cancer PTSD patients had more serious psychological problems, worse emotional stability and higher plasma cortisol level. This review focuses on the role and mechanism of epigenetic modification in the occurrence, metastasis and invasion of hepatocellular carcinoma (HCC), and summarizes the latest methods for the treatment of HCC by restoring dysregulated epigenetic modification. Climate change vulnerability assessment is an essential tool for identifying regions that are most susceptible to the impacts of climate change and designing effective adaptation actions that can reduce vulnerability ...Climate change vulnerability assessment is an essential tool for identifying regions that are most susceptible to the impacts of climate change and designing effective adaptation actions that can reduce vulnerability and enhance long-term resilience of these regions.This study explored a framework for climate change vulnerability assessment in the new urban planning process in Jangwani Ward,Tanzania.Specifically,taking flood as an example,this study highlighted the steps and methods for climate change vulnerability assessment in the new urban planning process.In the study area,95 households were selected and interviewed through purposeful sampling.Additionally,10 respondents(4 females and 6 males)were interviewed for Focus Group Discussion(FGD),and 3 respondents(1 female and 2 males)were selected for Key Informant Interviews(KII)at the Ministry of Lands,Housing and Human Settlements Development.This study indicated that climate change vulnerability assessment framework involves the assessment of climatic hazards,risk elements,and adaptive capacity,and the determination of vulnerability levels.The average hazard risk rating of flood was 2.3.Socioeconomic and livelihood activities and physical infrastructures both had the average risk element rating of 3.0,and ecosystems had the average risk element rating of 2.9.Adaptive capacity ratings of knowledge,technology,economy or finance,and institution were 1.6,1.9,1.4,and 2.2,respectively.The vulnerability levels of socioeconomic and livelihood activities and physical infrastructure were very high(4.0).Ecosystems had a high vulnerability level(3.8)to flood.The very high vulnerability level of socioeconomic and livelihood activities was driven by high exposure and sensitivity to risk elements and low adaptive capacity.The study recommends adoption of the new urban planning process including preparation,planning,implementation,and monitoring-evaluation-review phases that integrates climate change vulnerability assessment in all phases.展开更多

The comparative analysis shows that the regulation project of a separate reach basically has no impact on velocity distributions and variations of diversion ratios of upper and lower reaches, the variations of dynamic axes are only within the local scope of the project. The regulation project of a separate reach also has less impact on the water level in the lower adjacent reaches, but will make the water levels in the upper reaches rise. The second time was a 24 h telephone interview after a missed appointment for a dose of PEP. Results: The prevalence was 5.6 bites/10.000 people. The average age was 26.4 ± 18.3 years. Age ≤ 26 years was more concerned with 59.3% of cases (p = 0.006) in which children Conclusion: The rabies risk is potential in Boma. Fast pyrolysis of corn stover was performed at temperatures in the range of 450℃-525℃ in a fluidized bed.The chemical composition of the bio-oil acquired was analyzed by GC/MS,and its pH,kinetic viscosity and calori...Fast pyrolysis of corn stover was performed at temperatures in the range of 450℃-525℃ in a fluidized bed.The chemical composition of the bio-oil acquired was analyzed by GC/MS,and its pH,kinetic viscosity and calorific values were determined.In the pyrolysis system used in this experiment,some improvements to former pyrolysis systems were done.Two screw feeders were used to prevent jamming the feeding system,and the condenser was equipped with some nozzles and a heat exchanger to cool quickly the cleaned hot gas into bio-oil.The results showed that the bio-oil yield increased with increasing pyrolysis temperature and then declined with a further increase in pyrolysis temperature.The highest bio-oil yield of 43.3 wt.%was obtained at 500℃ with the dolomite as bed material.The char yield always decreased with the increase of temperature.The major chemical compounds of bio-oil included acetol,butanone,acetic acid,propionic acid,ethylene glycol,phenol,etc.Pyrolysis oil was completely immiscible in diesel,but homogeneous emulsions were obtained when mixing the pyrolysis oil,emulsifier with diesel in different ratios.展开更多

Five reference databases (PubMed/Embase, China BioMedical Literature Database, ClinicalKey, Wanfang Data, and China Knowledge Resource Integrated) and 7 guideline databases (Guideline International Network, National Collaborating Centre for Women’s and Children’s Health, National Guideline Clearinghouse, The National Institute for Health and Care Excellence, Scottish Intercollegiate Guidelines Network, New Zealand Guideline Group, and Canadian Medical Association infobase) were searched. Official websites of societies were searched to find relevant guidelines and consensus, and guidelines and published systematic reviews and consensus were reviewed from inception through May 2021. A literature search was performed regarding guidance offered in professional facilities, and 304 records (including 4 in Chinese) of existing guidelines for full-text review were found. Moreover, 55 guidelines were identified after a manual search. The literature review demonstrated that FP guidance and options were unevenly distributed worldwide for women who wanted to post-pone pregnancy or had impaired fertility after gonadal toxicity. With promising cancer survival rates and diversified family decisions, more attention should be paid to the improvement and update of accessible guidelines and regulatory infrastructure to inform patients about the available options and empower them to make informed choices. Restrictions to such services can be gradually eased due to the efficacy and safety of certain FP techniques for the right candidates to initiate pregnancy conception.展开更多

This review focuses on the role and mechanism of epigenetic modification in the occurrence, metastasis and invasion of hepatocellular carcinoma (HCC), and summarizes the latest methods for the treatment of HCC by restoring dysregulated epigenetic modification. It provides a theoretical basis for revealing the pathogenesis of liver cancer and developing new methods of diagnosis and treatment.
文摘Climate change vulnerability assessment is an essential tool for identifying regions that are most susceptible to the impacts of climate change and designing effective adaptation actions that can reduce vulnerability and enhance long-term resilience of these regions.This study explored a framework for climate change vulnerability assessment in the new urban planning process in Jangwani Ward,Tanzania.Specifically,taking flood as an example,this study highlighted the steps and methods for climate change vulnerability assessment in the new urban planning process.In the study area,95 households were selected and interviewed through purposeful sampling.Additionally,10 respondents(4 females and 6 males)were interviewed for Focus Group Discussion(FGD),and 3 respondents(1 female and 2 males)were selected for Key Informant Interviews(KII)at the Ministry of Lands,Housing and Human Settlements Development.This study indicated that climate change vulnerability assessment framework involves the assessment of climatic hazards,risk elements,and adaptive capacity,and the determination of vulnerability levels.The average hazard risk rating of flood was 2.3.Socioeconomic and livelihood activities and physical infrastructures both had the average risk element rating of 3.0,and ecosystems had the average risk element rating of 2.9.Adaptive capacity ratings of knowledge,technology,economy or finance,and institution were 1.6,1.9,1.4,and 2.2,respectively.The vulnerability levels of socioeconomic and livelihood activities and physical infrastructure were very high(4.0).Ecosystems had a high vulnerability level(3.8)to flood.The very high vulnerability level of socioeconomic and livelihood activities was driven by high exposure and sensitivity to risk elements and low adaptive capacity.The study recommends adoption of the new urban planning process including preparation,planning,implementation,and monitoring-evaluation-review phases that integrates climate change vulnerability assessment in all phases.

文摘OBJECTIVE:To explore a possible strategy of market expansion for Traditional Chinese Medicine(TCM) medical services.METHODS:Based on literature study and the data collected through a survey with a specifically-designed questionnaire,a strategy was suggested for TCM medical services entities.RESULTS:The outcome of the survey showed that 87.50% of respondents held that TCM belonged to complementary and alternative medicine or traditional medicine.CONCULSION:TCM medical services entities should be considered as market stopgap in healthcare market and a strategy of market expansion for TCM medical services should be built for them accordingly.

文摘A two-dimensional flow numerical model of the tidal reaches, which total length is more than 700 km, is established from Datong to the Yangtze River estuary. The tidal levels, velocities, diversion ratios and dynamic axes before and after the separate regulation of each reach and combined regulation of all reaches are obtained. The comparative analysis shows that the regulation project of a separate reach basically has no impact on velocity distributions and variations of diversion ratios of upper and lower reaches, the variations of dynamic axes are only within the local scope of the project. The regulation project of a separate reach also has less impact on the water level in the lower adjacent reaches, but will make the water levels in the upper reaches rise. After the implementation of the regulation projects for all reaches, the rise of water level in the upstream reaches will have a cumulative impact.

文摘Objective: To describe and analyze the epidemiological aspects as well as the observance of post-exposure prophylaxis (PEP) of people bitten by a dog and received at the Buanionzi State Health Center from 2014 to 2017. Methods: Descriptive cross-sectional study which took place in two stages. The first interview was an interrogation carried out at the health Center until surgical trimming (100% of cases) followed by PEP (70.3% of cases) according to the Essen protocol. The second time was a 24 h telephone interview after a missed appointment for a dose of PEP. Results: The prevalence was 5.6 bites/10.000 people. The average age was 26.4 ± 18.3 years. Age ≤ 26 years was more concerned with 59.3% of cases (p = 0.006) in which children Conclusion: The rabies risk is potential in Boma. Efforts in terms of public health strategies must be made to reduce it.

文摘Fast pyrolysis of corn stover was performed at temperatures in the range of 450℃-525℃ in a fluidized bed.The chemical composition of the bio-oil acquired was analyzed by GC/MS,and its pH,kinetic viscosity and calorific values were determined.In the pyrolysis system used in this experiment,some improvements to former pyrolysis systems were done.Two screw feeders were used to prevent jamming the feeding system,and the condenser was equipped with some nozzles and a heat exchanger to cool quickly the cleaned hot gas into bio-oil.The results showed that the bio-oil yield increased with increasing pyrolysis temperature and then declined with a further increase in pyrolysis temperature.The highest bio-oil yield of 43.3 wt.%was obtained at 500℃ with the dolomite as bed material.The char yield always decreased with the increase of temperature.The major chemical compounds of bio-oil included acetol,butanone,acetic acid,propionic acid,ethylene glycol,phenol,etc.Pyrolysis oil was completely immiscible in diesel,but homogeneous emulsions were obtained when mixing the pyrolysis oil,emulsifier with diesel in different ratios.
文摘To summarize fertility preservation (FP) guidelines and consensus provided by medical facilities worldwide. Five reference databases (PubMed/Embase, China BioMedical Literature Database, ClinicalKey, Wanfang Data, and China Knowledge Resource Integrated) and 7 guideline databases (Guideline International Network, National Collaborating Centre for Women’s and Children’s Health, National Guideline Clearinghouse, The National Institute for Health and Care Excellence, Scottish Intercollegiate Guidelines Network, New Zealand Guideline Group, and Canadian Medical Association infobase) were searched. Official websites of societies were searched to find relevant guidelines and consensus, and guidelines and published systematic reviews and consensus were reviewed from inception through May 2021. A literature search was performed regarding guidance offered in professional facilities, and 304 records (including 4 in Chinese) of existing guidelines for full-text review were found. Moreover, 55 guidelines were identified after a manual search. The literature review demonstrated that FP guidance and options were unevenly distributed worldwide for women who wanted to post-pone pregnancy or had impaired fertility after gonadal toxicity. With promising cancer survival rates and diversified family decisions, more attention should be paid to the improvement and update of accessible guidelines and regulatory infrastructure to inform patients about the available options and empower them to make informed choices. Restrictions to such services can be gradually eased due to the efficacy and safety of certain FP techniques for the right candidates to initiate pregnancy conception.

文摘My early understanding of the People’s Republic of China was drawn from a few books I read while growing up in a mountain village in Myanmar. I was particularly fascinated by The Other Side of the River and Red Star Over China, both written by American journalist Edgar Snow. Those books triggered great curiosity about China in me. Later, while serving in the army I was stationed in the northern and later eastern regions of Myanmar’s Shan State, both bordering China, but still never found the chance to set foot in the country. During my service at army headquarters, I was assigned to conduct research on the situation in Myanmar’s neighbors including China as well as other influential countries in the world.
