In 1886,Senn stated that removing necrotic pancreatic and peripancreatic tissue would benefit patients with severe acute pancreatitis.Since then,necrosectomy has been a mainstay of surgical procedures for infected nec...In 1886,Senn stated that removing necrotic pancreatic and peripancreatic tissue would benefit patients with severe acute pancreatitis.Since then,necrosectomy has been a mainstay of surgical procedures for infected necrotizing pancreatitis(NP).No published report has successfully questioned the role of necrosectomy.Recently,however,increasing evidence shows good outcomes when treating walled-off necrotizing pancreatitis without a necrosectomy.The literature concerning NP published primarily after 2000 was reviewed;it demonstrates the feasibility of a paradigm shift.The majority(75%)of minimally invasive necrosectomies show higher completion rates:between 80%and 100%.Transluminal endoscopic necrosectomy has shown remarkable results when combined with percutaneous drainage or a metallic stent.Related morbidities range from 40%to 92%.Single-digit mortality rates have been achieved with transluminal endoscopic necrosectomy,but not with video-assisted retroperitoneal necrosectomy series.Drainage procedures without necrosectomy have evolved from percutaneous drainage to transluminal endoscopic drainage with or without percutaneous endoscopic gastrostomy access for laparoscopic instruments.Most series have reached higher success rates of 79%-93%,and even 100%,using transcystic multiple drainage methods.It is becoming evident that transluminal endoscopic drainage treatment of walledoff NP without a necrosectomy is feasible.With further refinement of the drainage procedures,a paradigm shift from necrosectomy to drainage is inevitable.展开更多
Background:In March 2020,the WHO declared COVID-19 as a pandemic,and Tunisia implemented a containment and targeted screening strategy.The country’s public health policy has since focused on managing hospital beds.Me...Background:In March 2020,the WHO declared COVID-19 as a pandemic,and Tunisia implemented a containment and targeted screening strategy.The country’s public health policy has since focused on managing hospital beds.Methods:The study analyzed the bed occupancy rates in public hospitals in Tunisia during the pandemic.The evolution of daily cases and nonpharmaceutical interventions(NPI)actions undertaken by the Tunisian Government were also analyzed.The study used 3 indices to assess bed flexibility:Ramp duration until the peak,ramp growth until the peak,and ramp rate until the peak.The study also calculated the time shift at the start and peak of each wave to evaluate the government’s response efficacy.Results:The study found that the evolution of the epidemic in Tunisia had 2 phases.The first phase saw the pandemic being controlled due to strong NPI actions,while the second phase saw a relaxation of measures and an increase in wave intensity.ICU bed availability followed the demand for beds,but ICU bed occupancy remained high,with a maximum of 97%.The government’s response in terms of bed distribution and reallocation was slow.The study found that the most deadly wave by ICU occupied bed was the third wave due to a historical variant,while the fifth wave due to the delta variant was the most deadly in terms of cumulative death.Conclusions:The study concluded that decision-makers could use its findings to assess their response capabilities in the current pandemic and future ones.The study highlighted the importance of flexible and responsive healthcare systems in managing pandemics.展开更多

Axonal tracing is useful for detecting optic nerve injury and regeneration,but many commonly used methods cannot be used to observe axoplasmic flow and synaptic transmission in vivo.Manganese(Mn^2+)-enhanced magnet...Axonal tracing is useful for detecting optic nerve injury and regeneration,but many commonly used methods cannot be used to observe axoplasmic flow and synaptic transmission in vivo.Manganese(Mn^2+)-enhanced magnetic resonance imaging(MEMRI) can be used for in vivo longitudinal tracing of the visual pathway.Here,we explored the dose response and time course of an intravitreal injection of Mn Cl2 for tracing the visual pathway in rabbits in vivo using MEMRI.We found that 2 m M Mn Cl2 enhanced images of the optic nerve but not the lateral geniculate body or superior colliculus,whereas at all other doses tested(5–40 m M),images of the visual pathway from the retina to the contralateral superior colliculus were significantly enhanced.The images were brightest at 24 hours,and then decreased in brightness until the end of the experiment(7 days).No signal enhancement was observed in the visual cortex at any concentration of Mn Cl2.These results suggest that MEMRI is a viable method for temporospatial tracing of the visual pathway in vivo.Signal enhancement in MEMRI depends on the dose of Mn Cl2,and the strongest signals appear 24 hours after intravitreal injection.展开更多

The dual of the jump-diffusion risk model under a threshold dividend strategy is discussed. The authors derive a set of two integro-differential equations satisfied by the expected total discounted dividend until ruin. The cases where profits follow an exponential or mixtures of exponential distributions are solved. Applying the key method of the Laplace transform, the authors show how the integro-differential equations are solved. The authors also discuss the conditions for optimality and show how an optimal dividend threshold can be calculated as well.展开更多

Results showed that rate of egg production (F, eggs/(,d)) ...Reproductive rate of a marine planktonic copepod Labidocera euchaeta Giesbrecht in Xiamen Harbor was studied during the period from April 1986 to August 1987. Results showed that rate of egg production (F, eggs/(,d)) was positively correlated with temperature (T,℃) ) and body weight of spawning female (We,ugC ) as: F=0.0637 (T-7)0.7445Wc. Each month a maximum rate of egg production occurred at one of the five experimental temperatures (10°, 15°, 20°, 25°, and 30℃) near the monthly mean field temperature. The available food density (P, ugC/ml) had significant effect on specific rate of egg production (Fc, %C/d) as :Fc=0.1732(P-0.0179)/P. The gross efficiency of egg production of ca 62% in carbon was independent of food density snd ingestion rate. Observations on in situ egg production and hatching rate demonstrated that this species reproduces continuously all the year round, with summer as its high reproductive season and winter a low one.
