目的探讨无腹腔镜辅助的内镜全层切除术(EFR)治疗源于固有肌层的胃黏膜下肿瘤(SMT)的疗效和可行性。方法2007年7月至2009年8月经辅助检查和前期治疗明确诊断为起源于固有肌层的胃SMT患者20例,择期行全麻下EFR治疗:(1)黏膜下注... 目的探讨无腹腔镜辅助的内镜全层切除术(EFR)治疗源于固有肌层的胃黏膜下肿瘤(SMT)的疗效和可行性。方法2007年7月至2009年8月经辅助检查和前期治疗明确诊断为起源于固有肌层的胃SMT患者20例,择期行全麻下EFR治疗:(1)黏膜下注射生理盐水,预切开肿瘤周围黏膜和黏膜下层,显露肿瘤;(2)采用内镜黏膜下剥离术(ESD)技术沿肿瘤周围分离固有肌层至浆膜层;(3)应用Hook刀沿肿瘤边缘切开浆膜;(4)胃镜直视下应用Hook、IT刀或圈套器完整切除包括浆膜在内的肿瘤;(5)应用金属夹缝合胃创面。结果20例源于固有肌层的胃SMT中,病灶位于胃底9例、胃体11例。EFR成功切除所有病变,完整切除率为100%,未使用腹腔镜辅助;EFR时间为60—145min,平均85min。切除肿瘤最大直径1.8~3.5cm,平均2.6cm;病理诊断为问质瘤13例、平滑肌瘤4例、血管球瘤2例、神经鞘瘤1例。术后无一例出现胃出血、腹膜炎体征及腹腔脓肿,术后3d造影无一例造影剂外漏和胃排空障碍。住院天数3~8d,平均5.5d。术后随访1~12个月,平均7个月,无一例病变残留或复发。结论EFR治疗源于固有肌层的胃SMT是安全、有效的,可以切除更深的胃壁肿瘤,并提供准确的病理诊断资料,它的开展可进一步扩大内镜治疗的适应证。 展开更多

AIM: To identify whether JTE-522 can induce apoptosis in AGS cells and ROS also involved in the process, and to investigate the changes in NF-kB, p53, bcl-2 and caspase in the apoptosis process. METHODS: Cell culture, MTT, Electromicroscopy, agarose gel electrophoresis, lucigenin, Western blot and electrophoretic mobility shift assay (EMSA) analysis were employed to investigate the effect of JTE-522 on cell proliferation and apoptosis in AGS cells and related molecular mechanisms. RESULTS: JTE-522 inhibited the growth of AGS cells and induced the apoptosis. Lucigenin assay showed the generation of ROS in cells under incubation with JTE-522. The increased ROS generation might contribute to the induction of AGS cells to apoptosis. EMSA and Western blot revealed that NF-kB activity was almost completely inhibited by preventing the degradation of IkBalpha. Additionally, by using Western blot we confirmed that the level of bcl-2 was decreased, whereas p53 showed a great increase following JTE-522 treatment. Their changes were in a dose-dependent manner. CONCLUSION: These findings suggest that reactive oxygen species, NF-kB, p53, bcl-2 and caspase-3 may play an important role in the induction of apoptosis in AGS cells after treatment with JTE-522. 展开更多

目的 探讨血清癌胚抗原(CEA)、唾液酸化岩藻戊糖(CA199)、胃肠道癌抗原(CA242)、甲胎蛋白(AFP)、胃癌及卵巢癌抗原(CA724)对胃癌的诊断及患者的预后评价价值.方法 回顾性分析诊断为胃癌并接受手术治疗的108例患者,应用电化学... 目的 探讨血清癌胚抗原(CEA)、唾液酸化岩藻戊糖(CA199)、胃肠道癌抗原(CA242)、甲胎蛋白(AFP)、胃癌及卵巢癌抗原(CA724)对胃癌的诊断及患者的预后评价价值.方法 回顾性分析诊断为胃癌并接受手术治疗的108例患者,应用电化学发光和酶联免疫法检测外周血清CEA、CA199、CA242、CA724、AFP水平.以108例胃癌患者标本绘制5种标志物受试者工作曲线(ROC);Kaplan-Meier 法分析胃癌患者生存时间;Cox比例风险模型分析影响胃癌患者预后的因素.结果 CEA、CA199、CA242、CA724诊断胃癌的ROC曲线AUC分别为0.735,0.714,0.598,0.576.检测敏感度、特异度、约登指数分别为40.2%、97.7%、0.379;43.0%、91.3%、0.343;35.2%、94.8%、0.300;31.3%、95.3%、0.266.CEA、CA199、CA242、CA724联合诊断胃癌的敏感度、特异度、约登指数分别为76.8%、82.3%、0.585.CEA、CA724与淋巴结转移相关(均P< 0.05);血清CEA、CA199、CA242水平与脉栓、远处转移相关及腹腔积液形成有关(均P<0.05).肿块型和导管型患者血清AFP的水平高于浸润性和溃疡型患者(P<0.05).肿瘤大小与CEA、AFP、CA724有关.Cox逐步回归分析显示,影响患者3年生存期的因素有术前CEA浓度>3 μg/ml(P=0.000)、淋巴结转移(P=0.002).随访59例患者中34例死亡,术前CEA阴性与阳性患者中位生存期分别为18.1、10.9个月(P<0.005).结论 联合检测血清CEA、CA199、CA242、CA724可提高胃癌检出率,同时对预测胃癌患者的复发、转移;术前CEA为胃癌患者唯一不良预后指标. 展开更多

目的:探讨术前联合检测血清癌胚抗原(CEA)和糖类抗原CA19-9、CA125、CA724、CA211、CA242在胃癌腹膜转移诊断中的应用价值。方法:收集2015年12月—2016年12月间在上海交通大学医学院附属新华医院收治的89例胃癌患者术前血清肿瘤标志物... 目的:探讨术前联合检测血清癌胚抗原(CEA)和糖类抗原CA19-9、CA125、CA724、CA211、CA242在胃癌腹膜转移诊断中的应用价值。方法:收集2015年12月—2016年12月间在上海交通大学医学院附属新华医院收治的89例胃癌患者术前血清肿瘤标志物检测值,其中17例患者有腹膜转移(转移组),72例无腹膜转移(无转移组),通过建立Logistic回归模型,绘制ROC曲线并计算曲线下面积(AUC)评价各血清肿瘤标志物单独与联合检测对胃癌腹膜转移的诊断效能。结果:转移组中肿瘤标志物CA125、CA724、CA211的阳性检出率明显高于无转移组相比较(均P<0.05),而其他3种肿瘤标志物阳性检出率在两组间均无统计学差异(均P>0.05);单独检测时,CA125诊断胃癌腹膜转移的AUC最大,与其他5种肿瘤标志物的差异均有统计学意义(均P<0.05);CA125、CA724、CA211联合检测与6种肿瘤标志物联合检测的AUC均明显大于各单项检测的AUC(均P<0.05),但两者间AUC差异无统计学意义(0.964 vs.0.949,P=0.866)。结论:CA125可作为胃癌腹膜转移的预测指标,且与CA724、CA211的联合检测可为临床鉴别胃癌腹膜转移提供有效的参考,但与其他肿瘤标志物联合检测的价值不大。 展开更多

AIM: To establish the role of vascular endothelial growth factor (VEGF) in the oncogenesis of human gastric carcinoma more directly. METHODS: The expression of VEGF and its receptor kinase-domain insert containing receptor (KDR) in human gastric cancer tissue were observed by immunohistochemical staining. VEGF levels were manipulated in human gastric cancer cell using eukaryotic expression constructs designed to express the complete VEGF(165) complimentary DNA in either the sense or antisense orientation. The biological changes of the cells were observed in which VEGF was up-regulated or down-regulated. RESULTS: VEGF-positive rate was 50%, and VEGF was mainly localized in the cytoplasm and membrane of the tumor cells, while KDR was mainly located in the membrane of vascular endothelial cells in gastric cancer tissues and peri-cancerous tissue. In 2 cases of 50 specimens, the gastric cancer cells expressed KDR, localized in both the cytoplasm and membrane. Introduction of VEGF(165) antisense into human gastric cancer cells (SGC-7901, immunofluorescence intensity, 31.6%)) resulted in a significant reduction in VEGF-specific messenger RNA and total and cell surface VEGF protein (immunofluorescence intensity, 8.9%) (P【0.05). Conversely, stable integration of VEGF(165) in the sense orientation resulted in an increase in cellular and cell surface VEGF (immunofluorescence intensity, 75.4%) (P【0.05). Lowered VEGF levels were associated with a marked decrease in the growth of nude mouse xenografted tumor (at 33 days postimplantation, tumor volume: 345.40 +/- 136.31 mm3)(P【0.05 vs control SGC-7901 group: 1534.40 +/- 362.88 mm3), whereas up-regulation of VEGF resulted in increased xenografted tumor size (at 33 days postimplantation, tumor volume: 2350.50 +/- 637.70 mm3) (P【0.05 vs control SGC-7901 group). AIM: To reveal the correlation between the functional differentiation phenotypes of gastric carcinoma cells and the invasion and metastasis by a new way of cell-function classification.METHODS:Surgically resected specimens of 361 gastric carcinomas(GC) were investigated with enzyme-, mucin-, and tumor-related marker immunohistochemistry. According to the direction of cell-function differentiation, stomach carcinomas were divided into five functionally differentiated types. RESULTS: (1) Absorptive function differentiation type (AFDT): there were 82 (22.7%) patients including 76 (92.7%) aged 45 years. Sixty-nine (84.1%) cases belonged to the intestinal type. Thirty-eight (46.3%) expressed CD44v6 and 9 (13.6%) of 66 male patients developed liver metastasis.The 5-year survival rate of patients in this group (58.5%) was higher than those with the other types (P【0.01). (2) Mucin secreting function differentiation type (MSFDT): 54 (15%) cases. Fifty-three (98.1%) tumors had penetrated the serosa, 12 (22.2%) expressed ER and 22 (40.7%) expressed CD44v6. The postoperative 5-year survival rate was 28.6%. (3) Absorptive and mucin-producing function differentiation type (AMPFDT): there were 180 (49.9%) cases, including 31 (17.2%) aged younger than 45 years. The tumor was more common in women (62, 34.4%,) and expressed more frequently estrogen receptors (ER) (129, 81.7%) than other types (P【0.01). Ovary metastasis was found in 12 (19.4%) out of 62 female subjects. The patients with this type GC had the lowest 5-year survival rate (24.7%) among all types. (4) Specific function differentiation type (SFDT): 13 (3.6%) cases. Nine (69.2%) tumors of this type derived from APUD system, the other 4 (30.7%) were of different histological differentiation. Sixty per cent of the patients survived at least five years. (5) Non-function differentiation type (NFDT): 32 (8.9%) cases. 目的探讨多种基因改变在癌前病变及胃癌组织中的作用,方法应用 PCR-RFLP,PCR-SSCP,RT-PCR 及免疫组化技术同时对60例肠化生,30例异型增生及52例胃癌组织中抑癌基因 APC,MCC,DCC,YNZ22,p53及癌基因 Ki-ras,Bcl-2多种变异形式进行检测。... 目的探讨多种基因改变在癌前病变及胃癌组织中的作用,方法应用 PCR-RFLP,PCR-SSCP,RT-PCR 及免疫组化技术同时对60例肠化生,30例异型增生及52例胃癌组织中抑癌基因 APC,MCC,DCC,YNZ22,p53及癌基因 Ki-ras,Bcl-2多种变异形式进行检测。结果随着肠化生粘膜向异型增生、胃癌的发展,多种基因改变的频率逐步升高,胃癌组织中 APC,DCC,YNZ22,p53,Bcl-2的改变频率分别为57.7%(30/52),43.1%(22/51),51.6%(16/31),67.3%(35/52),68.6%(35/51)显著高于肠化生上述基因的改变(APC 33.3%,DCC4.3%,YNZ22 19.4%,p5326.7%,Bcl-2 33.3%)(P<0.05,0.01),异型增生组织中DCC 基因改变为12.5%(3/24),也显著低于胃癌组织中的改变.Ⅲ型肠化中 APC 及 bcl-2基因蛋白表达率分别为61.1%,55.6%,p53突变及蛋白表达率为57.1%,27.8%,显著高于Ⅰ,Ⅱ型肠化中 APC,Bcl-2蛋白表达率(6.3%,23.8%)(P<0.01,O.05)及 p53突变及蛋白表达率(18.2%,2.4%)(P<0.05).肠型胃癌 APC,p53,Ki-ras 突变率分别为52.9%;82.4%;29.4%,显著高于胃型胃癌各基因的突变(APC 18.2%;p53 45.8%;Ki-ras 3.0%)(P<0.05).肠型胃癌 APC,Ki-ras,bcl-2基因的蛋白表达率分别为76.5%;41.2%;93.8%,胃型胃癌分别为30.3%;3.0%;54.5%,两型相比差别显著(P<0.01,0.05).APC,p53及 Bcl-2基因可能是肠化生癌变及肠型胃癌的热点基因,肠化生及异型增生阶段即可检测到基因改变的累积现象,但以胃癌组织中最显著。结论多种基因改变的累积与胃癌的发生及演进密切相关,不同类型肠化生分子改变机制不同,APC,p53及 Bcl-2基因有可能成为肠型胃癌早期诊断的分子标志。 展开更多

One of the major topics in this field is endoscopic submucosal dissection (ESD). ESD is a new endoscopic technique using cutting devices to remove the tumor by the following three steps: injecting fluid into the submucosa to elevate the tumor from the muscle layer, pre-cutting the surrounding mucosa of the tumor, and dissecting the connective tissue of the submucosa beneath the tumor. So the tumors are resectable in an en bloc fashion, regardless of the size, shape, coexisting ulcer, and location. Indication for ESD is strictly confined by two aspects: the possibility of nodal metastases and technical difficulty, which depends on the operators. Although long-term outcome data are still lacking, short-term outcomes of ESD are extremely favourable and laparotomy with gastrectomy is replaced with ESD in some parts of therapeutic strategy for early gastric cancer. 展开更多

AIM: To investigate the effects of antisense human telomerase RNA (hTR)on the biologic behavior of human gastric cancer cell line: MKN-45 by gene transfection and its potential role in the gene therapy of gastric cancer. METHODS: The hTR cDNA fragment was cloned from MKN-45 through RT-PCR and subcloned into eukaryotic expression vector (pEF6/V5-His-TOPO) in cis-direction or trans-direction by DNA recombinant methods. The constructed sense, antisense and empty vectors were transfected into MKN-45 cell lines separately by lipofectin-mediated DNA transfection technology. After drug selection, the expression of antisense hTR gene in stable transfectants and normal MKN-45 cells was detected by RT-PCR, the telomerase activity by TRAP, the apoptotic features by PI and Hoechst 33258 staining, the cell cycle distribution by flow cytometry and the population doubling time by cell counting. Comparison among the stable transfectants and normal MKN-45 cells was made. RESULTS: The sense, antisense hTR eukaryotic expression vectors and empty vector were successfully constructed and proved to be the same as original design by restriction endonuclease analysis and sequencing. Then, they were successfully transfected into MKN-45 cell lines separately with lipofectin. The expression of antisense hTR gene was only detected in MKN-45 cells stably transfected with antisense hTR vector (named as MKN-45-ahTR) but not in the control cells. In MKN-45-ahTR, the telomerase activity was inhibited by 75%, the apoptotic rate was increased to 25.3%, the percentage of cells in the G0/G1 phase was increased to 65%, the proliferation index was decreased to 35% and the population doubling time was prolonged to 35.3 hours. However, the telomerase activity, the apoptotic rate, the distribution of cell cycle, the proliferation index and the population doubling time were not different among the control cells. AIM:To study the clinicopathological features of gastric glomus tumor and review the related Chinese literature published in 1990-2010.METHODS:A case of gastric glomus tumor was re-ported.Clinicopathological findings in 56 cases of gastric glomus tumor were analyzed.RESULTS:Gastric glomus tumor was far more common in women than in men with a female to male ratio of 1.6:1.The median age of the patients was 45 years(range 28-79 years).The patients often complained of epigastric pain and bloody stool.The tumor was located in antrum of the stomach.The greatest diameter of the tumor was 0.8-11cm.Histologically,the tumor was comprised of nests of glomus cells surrounding the capillaries.Glomus cells were small,uniform and round.Vimentin,smooth muscle actin and actin were expressed in the tumor.Other markers,including S-100 protein,CD34,CD117,desmin,CD56,synaptophysin,chromo-granin A,neuron specific enolase and cytokeratin were all negative.CONCLUSION:Gastric glomus tumor is a rare benign mesenchymal neoplasm.Its diagnosis depends on pathologic examination.Differential diagnosis includes gastrointestinal stromal tumor,paraganglioma and carcinoid tumor. 展开更多

目的探讨胃癌组织中凋亡抑制因子survivin的表达及其与各临床病理因素的关系,评价survivin在胃癌预后判断中的价值。方法应用免疫组织化学EnVision两步法检测140例胃癌组织中survivin的表达情况,回顾性分析survivin与各临床病理因素及... 目的探讨胃癌组织中凋亡抑制因子survivin的表达及其与各临床病理因素的关系,评价survivin在胃癌预后判断中的价值。方法应用免疫组织化学EnVision两步法检测140例胃癌组织中survivin的表达情况,回顾性分析survivin与各临床病理因素及预后之间的关系。结果胃癌组织中survivin表达阳性率为46.4%(65/140)。累及浆膜层的胃癌survivin表达阳性率(53.2%)明显高于未及浆膜层者(32.6%),TNM分期Ⅲ、Ⅳ期的胃癌survivin表达阳性率(55.2%)明显高于Ⅰ、Ⅱ期者(38.4%)(P均<0.05)。分化较差的胃癌survivin表达阳性率(52.2%)与分化较好者差异无显著性意义(35.4%,P=0.059)。survivin表达阳性患者5年生存率(34.4%)明显低于表达阴性者(58.6%,P<0.006)。Cox回归模型分析显示,survivin表达状态是胃癌根治术后独立的预后因素,阳性表达提示预后不良。结论凋亡抑制因子survivin在胃癌组织中具有较高的表达率,其表达与胃癌的浸润深度和TNM分期相关。survivin的表达状态是胃癌根治术后独立的预后因素,阳性表达提示预后不良。 展开更多

AIM: To determine the effect of cis -9, trans -11-conjugated linoleic acid (c9, t11-CLA) on the cell cycle of gastric cancer cells (SGC-7901) and its possible mechanism in inhibition cancer growth. METHODS: Using cell culture and immunocytochemical techniques, we examined the cell growth, DNA synthesis, expression of PCNA, cyclin A, B(1), D(1), p16(ink4a) and p21(cip/waf1) of SGC-7901 cells which were treated with various c9, t11-CLA concentrations (25, 50, 100 and 200 micromol.L(-1))of c 9, t 11-CLA for 24 and 48h, with a negative control (0.1% ethane). RESULTS: The cell growth and DNA synthesis of SGC-7901 cells were inhibited by c9, t11-CLA.SGC-7901 cells. Eight day after treatment with various concentrations of c9, t11-CLA mentioned above, the inhibition rates were 5.92%, 20.15%, 75.61% and 82.44%, respectively and inhibitory effect of c9, t11-CLA on DNA synthesis (except for 25 micromol.L, 24h) showed significantly less (3)H-TdR incorporation than that in the negative controls (P【0.05 and P【0.01). Immunocytochemical staining demonstrated that SGC-7901 cells preincubated in media supplemented with different c9, t11-CLA concentrations at various times significantly decreased the expressions of PCNA (the expression rates were 7.2-3.0%, 24h and 9.1-0.9% at 48h, respectively), Cyclin A (11.0-2.3%, 24h and 8.5-0.5%,48h), B(1) (4.8-1.8% at 24h and 5.5-0.6% at 48h)and D(1) (3.6-1.4% at 24h and 3.7%-0 at 48h) as compared with those in the negative controls(the expressions of PCNA, Cyclin A, B(1) and D(1) were 6.5% at 24h and 9.0% at 48h, 4.2% at 24h and 5.1% at 48h, 9.5% at 24h and 6.0% at 48h,respectively)(P【0.01), whereas the expressions of P16(ink4a) and P21(cip/waf1), cyclin-dependent kinases inhibitors(CDKI), were increased. CONCLUSION: The cell growth and proliferation of SGC-7901 cell is inhibited by c9, t11-CLA via blocking the cell cycle, with reduced expressions of cyclin A,B(1) and D(1) and enhanced expressions of CDKI(P16(ink4a) and p21(cip/waf1)). 展开更多

目的:检测胃癌患者血清中17种胃癌相关细胞因子的水平,研究其与miR-10b水平的相关性,筛选在实验诊断中对胃癌诊断有意义的肿瘤标志物并探讨其对胃癌诊断的应用价值。方法:基于病例对照研究设计,选取病理组织学明确诊断为胃腺癌的患者血... 目的:检测胃癌患者血清中17种胃癌相关细胞因子的水平,研究其与miR-10b水平的相关性,筛选在实验诊断中对胃癌诊断有意义的肿瘤标志物并探讨其对胃癌诊断的应用价值。方法:基于病例对照研究设计,选取病理组织学明确诊断为胃腺癌的患者血清标本34例为胃癌组,选取体检中心经体检合格的健康人血清标本80例为对照组。采用Luminex检测技术检测2组研究对象血清样本中胃癌相关细胞因子的水平,选择巨噬细胞炎性蛋白3α(CCL20)、白细胞介素2(IL-2)、白细胞介素4(IL-4)、白细胞介素5(IL-5)、白细胞介素6(IL-6)、白细胞介素10(IL-10)、白细胞介素17A(IL-17A)、肿瘤坏死因子α(TNF-α)、干扰素γ(IFN-γ)、单核细胞趋化因子(CCL13)、干扰素诱导T细胞alpha趋化因子(CXCL11)、B淋巴细胞趋化因子(CXCL13)、骨形成蛋白4(BMP4)、骨形成蛋白7(BMP7)、肝细胞生长因子(HGF)、粒细胞集落刺激因子(GM-CSF)和血小板衍生因子BB(PDGF-BB)共17种细胞因子,从中筛选对胃癌诊断有意义的肿瘤标志物。采用实时荧光定量PCR(qRT-PCR)方法检测胃癌组患者血清miR-10b水平,采用电化学发光法检测胃癌组患者血清肿瘤标志物癌胚抗原(CEA)、糖链抗原19-9(CA19-9)和糖链抗原72-4(CA72-4)水平。结果:与对照组比较,胃癌组患者血清中CCL20、IL-2、IL-4、IL-6、IL-17A、TNF-α、CCL13、BMP-4和HGF水平明显升高(P<0.05);Logistic回归分析,血清中CCL20、IL-2、IL-4、IL-6、IL-17A、TNF-α、CXCL11、CCL13、BMP-4、BMP-7和HGF水平与胃癌患病风险有关联(P<0.05);受试者工作特征曲线(ROC曲线)分析,CCL20对胃癌诊断的曲线下面积(AUC)明显优于3种肿瘤标志物CEA、CA19-9和CA72-4的联合诊断;将6.71 ng·L-1 CCL20作为诊断临界值时,CCL20的诊断特异度为76.5%、灵敏度为91.3%、阳性预测值为78.8%、阴性预测值90.1%、阳性似然比为8.74、阴性似然比为0.26、诊断准确度为86.84%;Spearman相关性分� 展开更多

蓝激光成像结合放大内镜对内镜医生诊断上消化道早癌的价值研究 被引量:27

目的探讨蓝激光成像(Bu)结合放大内镜对不同年资内镜医生进行上消化道早癌诊断的价值。方法采用我院上消化道可疑病变病例行BLI精查,分别留取病灶处白光、BLI、BLI放大的图像,采用单盲法将图片分别给两组不同年资内镜医生独立观察... 目的探讨蓝激光成像(Bu)结合放大内镜对不同年资内镜医生进行上消化道早癌诊断的价值。方法采用我院上消化道可疑病变病例行BLI精查,分别留取病灶处白光、BLI、BLI放大的图像,采用单盲法将图片分别给两组不同年资内镜医生独立观察,首先观察所有病灶的白光图片,然后再观察所有病灶的BLI和BLI放大图片,分别记录经白光观察和BLI观察的病灶边界,浸润深度、腺管结构和血管走形,再对病灶性质做出诊断。所有结果以术后病理诊断结果为金标准,与病理结果吻合的认为判断正确,否则认为是判断错误。最后统计分析两组医生用白光或BLI对病变诊断的差别。结果共18例患者纳入研究且均进行了ESD治疗,16例为肿瘤性病变。仅通过白光观察,低年资组医生对病灶边界判断的正确率为57%,而高年资组的准确率为74%,两组差异有统计学意义(X2=4.997,P=0.032),而通过BLI放大观察,低年资组和高年资组对病灶边界判断的准确率分别为90%和96%,两组差异无统计学意义(r=1.985,P=0.229)。使用白光观察,高年资组和低年资组医生对病灶性质判断的准确率分别为82%和66%(X2=5.431,P=0.022),而使用BLI放大观察后,两组医生对病变性质的判断均有所提高,准确率分别为97%和84%(X2=7.337,P=0.007),两组差异有统计学意义。但不论是使用白光观察还是BLI放大观察,两组医生对病变浸润深度的判断差异均无统计学意义。结论BLI结合放大内镜行胃镜精查可帮助内镜医生尤其是低年资内镜医生提高上消化道早癌的诊断水平。 展开更多
