Diagnosis of both Lumbar Disc Herniation and Lumbar Spine Stenosis depends on clinical findings as well as radiological investigations. Treatment of choice of these conditions is on the basis of the patient conditions. Surgical treatment is the option only when the conservative treatment does not improve the patient’s clinical condition. Advancement and improvement of the technology have resulted in the traditional open surgical treatment into minimal invasive surgery. Intervention of the different surgical instruments with expert spinal surgeons had made percutaneous endoscopic lumbar Spine surgery as one of the preferred choices of surgery for treating Lumbar Disc Herniation and Lumbar Spine Stenosis. The concept of percutaneous endoscopic surgery for lumbar region is to provide surgical options without producing iatrogenic morbidity associated with the open surgical procedures. Conventionally, there are different approaches/techniques for Percutaneous Endoscopic Lumbar Spine Surgery, but in this review we are mainly focusing on the Transforaminal Technique. Regarding the Lumbar Disc Herniation treatment with transforaminal approach, a number of articles have been published due to which we mainly focused on those articles which were published after 2009 onwards. While fewer articles related to Lumbar Spine Stenosis treatment with Transforaminal approach were found, we tried to brief out all those articles. On the basis of comparative study of different surgeries done for Lumbar Disc Herniation and Lumbar Spine Stenosis, Percutaneous Transforaminal endoscopic Lumbar Surgery provides a substantial benefit. Transforaminal approach for treating Lumbar Disc Herniation and Lumbar Spine Stenosis is safe and effective. The Percutaneous Transforaminal Endoscopic Lumbar Surgery has advantage as it is performed under local anesthesia with shorter length of hospitalization and early 展开更多
文摘目的 比较单通道与双通道内镜技术治疗神经根型颈椎病(CSR)的临床疗效。方法 44例CSR患者分别采用经皮后路内镜下椎间盘切除术(PPECD组,21例)和单侧双通道内镜下椎间盘切除术(UBED组,23例)治疗。比较两组手术时间、术中出血量、灌洗液使用量、术后引流量。评估两组术前、术后第3天和术后3个月颈部和上肢VAS疼痛评分和颈椎功能障碍指数(NDI)以及术前和术后3个月影像学指标的变化。采用Macnab标准,比较术后3个月两组优良率。结果 UBED组手术时间短于PPECD组[(66.1±12.2) min vs.(87.9±19.9) min],术中出血量、灌洗液使用量和术后引流量均多于PPECD组[(47.4±25.6) mL vs.(14.0±6.0) mL、(9.9±1.9) L vs.(7.7±2.6) L和(26.1±12.8) mL vs.(13.8±6.7) mL](P<0.01)。与术前相比,两组术后第3天和术后3个月颈部和上肢VAS疼痛评分以及NDI均下降(P<0.05),术后3个月椎间盘角α、脊柱功能单位(FSU)角β和C_(2)~C_(7) Cobb角γ均增加,C_(2)~C_(7)矢状面轴向垂直距离(SVA)减少(P<0.05)。术后3个月,两组颈部和上肢VAS疼痛评分、NDI、椎间盘角α、FSU角β、C_(2)~C_(7) Cobb角γ、C_(2)~C_(7) SVA和优良率差异均无统计学意义(P>0.05)。结论 单通道与双通道内镜技术均能有效治疗CSR,单通道技术创伤较小,而双通道技术效率较高。
