BACKGROUND The technique of percutaneous endoscopic lumbar discectomy(PELD)as a transforaminal approach has been used to treat highly migrated lower lumbar disc herniations.However,due to the different anatomic charac...BACKGROUND The technique of percutaneous endoscopic lumbar discectomy(PELD)as a transforaminal approach has been used to treat highly migrated lower lumbar disc herniations.However,due to the different anatomic characteristics of the upper lumbar spine,conventional transforaminal PELD may fail to remove the highly migrated upper lumbar disc nucleus pulposus.Therefore,the purpose of this study was to describe a novel surgical technique,two-level PELD,for the treatment of highly migrated upper lumbar disc herniations and to report its related clinical outcomes.CASE SUMMARY A 60-year-old male presented with a complaint of pain at his lower back and right lower limb.The patient received 3 mo of conservative treatments but the symptoms were not alleviated.Physical examination revealed a positive femoral nerve stretch test and a negative straight leg raise test for the right leg,and preoperative visual analog scale(VAS)score for the lower back was 6 points and for the right leg was 8 points.Magnetic resonance imaging(MRI)demonstrated L2-L3 disc herniation on the right side and the herniated nucleus pulposus migrated to the upper margin of L2 vertebral body.According to physical examination and imaging findings,surgery was the primary consideration.Therefore,the patient underwent surgical treatment with two-level PELD.The pain symptom was relieved and the VAS score for back and thigh pain was one point postoperatively.The patient was asymptomatic and follow-up MRI scan 1 year after operation revealed no residual nucleus pulposus.CONCLUSION Two-level PELD as a transforaminal approach can be a safe and effective procedure for highly migrated upper lumbar disc herniation.展开更多
The Fengcheng Formation is a crucial source rock and the primary reservoir for oil accumulation in the Mahu Sag.Crude oils are distributed throughout the Fengcheng Formation,ranging from the edge to the interior of th...The Fengcheng Formation is a crucial source rock and the primary reservoir for oil accumulation in the Mahu Sag.Crude oils are distributed throughout the Fengcheng Formation,ranging from the edge to the interior of the sag in the southern Mahu Sag.These crude oils originate from in-situ source rocks in shallowly buried areas and the inner deep sag.During migration,the crude oil from the inner deep sag affects the source rocks close to carrier beds,leading to changes in the organic geochemical characteristics of the source rocks.These changes might alter source rock evaluations and oil-source correlation.Based on data such as total organic carbon(TOC),Rock-Eval pyrolysis of source rocks,and gas chromatography-mass spectrometry(GC-MS)of the saturated fraction,and considering the geological characteristics of the study area,we define the identification characteristics of source rock affected by migrated hydrocarbons and establish the various patterns of influence that migrated hydrocarbons have on the source rock of the Fengcheng Formation in the southern Mahu Sag.The source rocks of the Fengcheng Formation are mostly fair to good,containing mainly Type II organic matter and being thermally mature enough to generate oil.Source rocks affected by migrated hydrocarbons exhibit relatively high hydrocarbon contents(S1/TOC>110 mg HC/g TOC,Extract/TOC>30%,HC:hydrocarbon),relatively low Rock-Eval Tmax values,and relatively high tricyclic terpane contents with a descending and mountain-shaped distribution.Furthermore,biomarker composition parameters indicate a higher thermal maturity than in-situ source rocks.Through a comparison of the extract biomarker fingerprints of adjacent reservoirs and mudstones in different boreholes,three types of influence patterns of migrated hydrocarbons are identified:the edge-influence of thin sandstone-thick mudstone,the mixed-influence of sandstone-mudstone interbedded,and the full-influence of thick sandstone-thin mudstone.This finding reminds us that the influence of migrated hydrocarbons 展开更多

BACKGROUND An intrauterine device(IUD)is a commonly used contraceptive among women in China.It is widely used because it is safe,effective,simple,economic,and reversible.Among the possible complications,an ectopic IUD...BACKGROUND An intrauterine device(IUD)is a commonly used contraceptive among women in China.It is widely used because it is safe,effective,simple,economic,and reversible.Among the possible complications,an ectopic IUD in the bladder is rare.It occurs insidiously,has a long course,is associated with a high risk for injury,and is difficult to treat.CASE SUMMARY A 44-year-old woman was admitted for repeated episodes of urinary frequency,urgency,and dysuria over three months.Laboratory tests revealed significantly elevated urine leukocytes and bacteria.Urine culture suggested colonization with Enterococcus faecalis.Abdominal computed tomography images suggested an abnormally positioned IUD that was protruding into the bladder.Cystoscopy revealed a metallic foreign body with multiple stones on its surface in the left posterior bladder wall.The foreign body measured approximately 1 cm.Hysteroscopy revealed the arm of a V-type metal IUD embedded in the middle and upper sections of the anterior wall of the cervical canal.The majority of the IUD was located in the uterine cavity.Cystoscopy was performed,and a holmium laser was utilized to break the stones attached to the portion of the IUD in the bladder.The IUD was then removed through hysteroscopy.CONCLUSION Ectopic IUDs in the bladder can be diagnosed with thorough imaging and safely removed through cystoscopy or hysteroscopy.展开更多

The characteristics of geomagnetic short perio...A temporary profile from Shiquan in the southern part of Shaanxi Province to Ningxian in Gansu Province for observation of geomagnetic short period variations was set up. The characteristics of geomagnetic short period variations in the profile region was analyzed through geomagnetic transfer functions at measurement sites (Fan, et al, 1998). According to the theory of multi-scale analysis of wavelet transform, the components affected by regional origin in the region is extracted in this paper. The components of regional origin is used as original data in the inversion study for investigation of 2-D conductivity structure corresponding to the regional origin. The inversion method (Zhdanov, Traynin, 1997) which is based on the minimum of the total energy of the residual field between observational electro-magnetic field and the calculated theoretical field of conductivity model and the conception of migrated residual field is introduced into our inversion of the data of magneto-variation sounding. The result of the inversion gives the underground conductivity structure in depth range of 100 km beneath the profile, and shows that a high conductivity area is located under Wugong and Qianling. The main characteristics of the inversion method is briefly discussed, that is, it can reflect the depth and the distribution of anomalous conductivity area comparatively well, and large number of parameters of inversion model is allowed to be used in the Inversion.展开更多
Amplitude versus offset analysis is a fundamental tool for determining the physical properties of reservoirs but generally hampered by the blurred common image gathers(CIGs).The blurring can be optimally corrected usi...Amplitude versus offset analysis is a fundamental tool for determining the physical properties of reservoirs but generally hampered by the blurred common image gathers(CIGs).The blurring can be optimally corrected using the blockwise least-squares prestack time migration(BLS-PSTM),where common-offset migrated sections are divided into a series of blocks related to the explicit offsetdependent Hessian matrix and the following inverse filtering is iteratively applied to invert the corresponding reflectivity.However,calculating the Hessian matrix is slow.We present a fast BLS-PSTM via accelerating Hessian calculation with dip-angle Fresnel zone(DFZ).DFZ is closely related to optimal migration aperture,which significantly attenuates migration swings and reduces the computational cost of PSTM.Specifically,our fast BLS-PSTM is implemented as a two-stage process.First,we limit the aperture for any imaging point with an approximated the projected Fresnel zone before calculating the Hessian matrix.Then,we determine whether a seismic trace contributes to the imaging point via DFZ during calculating the Hessian matrix.Numerical tests on synthetic and field data validate the distinct speedup with higher-quality CIGs compared to BLS-PSTM.展开更多
