Profound vital organ hypoperfusion contributes significantly to the dismal survival rates observed with manual CPR after cardiac arrest. This text demonstrate that the combination of standard cardiopulmonary resuscitation or active compression-decompression (ACD) CPR and an inspiratory impedance threshold valve (ITV) results in significantly increase in blood flow to the heart and brain .The beneficial effects of the ITV is the prevention in a significant decrease in the CPP or vital organ perfusion when the ITV was added to the respiratory circuit.The inspiratory impedance threshold valve (ITV) is inserted within the respiratory circuit, between the ventilation bag and the endotracheal tube, face mask, Combitube, or LMA. The ITV enhances venous return during the chest recoil phase of cardiopulmonary resuscitation (CPR) and thus primes the heart for the subsequent CPR compression. A safety check valve attached to the side of the ITV opens if the patient begins to breathe independently.Use of ACD+ITD CPR resulted in a significant overall short-term survival benefit and a strong trend toward improvement in neurological outcome. This suggest that the combination of improved perfusion during CPR with ITD coupled with hypothermia and other metabolic hibernation strategies promises to markedly increase the chances for survival in years to come and larger studies will be needed to evaluate the long-term potential benefits of ACD+ITD CPR. 展开更多
