Insulinomas,the most common cause of hypoglycemia related to endogenous hyperinsulinism,occur in 1-4 people per million of the general population.Common autonomic symptoms of insulinoma include diaphroresis,tremor,and...Insulinomas,the most common cause of hypoglycemia related to endogenous hyperinsulinism,occur in 1-4 people per million of the general population.Common autonomic symptoms of insulinoma include diaphroresis,tremor,and palpitations,whereas neuroglycopenenic symptoms include confusion,behavioural changes,personality changes,visual disturbances,seizure,and coma.Diagnosis of suspected cases is based on standard endocrine tests,especially the prolonged fasting test.Non-invasive imaging procedures,such as computed tomography and magnetic resonance imaging,are used when a diagnosis of insulinoma has been made to localize the source of pathological insulin secretion.Invasive modalities,such as endoscopic ultrasonography and arterial stimulation venous sampling,are highly accurate in the preoperative localization of insulinomas and have frequently been shown to be superior to noninvasive localization techniques.The range of techniques available for the localization of insulinomas means thatblind resection can be avoided.Intraoperative manual palpation of the pancreas by an experienced surgeon and intraoperative ultrasonography are both sensitive methods with which to finalize the location of insulinomas.A high proportion of patients with insulinomas can be cured with surgery.In patients with malignant insulinomas,an aggressive medical approach,including extended pancreatic resection,liver resection,liver transplantation,chemoembolization,or radiofrequency ablation,is recommended to improve both survival and quality of life.In patients with unresectable or uncontrollable insulinomas,such as malignant insulinoma of the pancreas,several techniques should be considered,including administration of ocreotide and/or continuous glucose monitoring,to prevent hypoglycemic episodes and to improve quality of life.展开更多
Rock failure phenomena,such as rockburst,slabbing(or spalling) and zonal disintegration,related to deep underground excavation of hard rocks are frequently reported and pose a great threat to deep mining.Currently,the...Rock failure phenomena,such as rockburst,slabbing(or spalling) and zonal disintegration,related to deep underground excavation of hard rocks are frequently reported and pose a great threat to deep mining.Currently,the explanation for these failure phenomena using existing dynamic or static rock mechanics theory is not straightforward.In this study,new theory and testing method for deep underground rock mass under coupled static-dynamic loading are introduced.Two types of coupled loading modes,i.e.'critical static stress + slight disturbance' and 'elastic static stress + impact disturbance',are proposed,and associated test devices are developed.Rockburst phenomena of hard rocks under coupled static-dynamic loading are successfully reproduced in the laboratory,and the rockburst mechanism and related criteria are demonstrated.The results of true triaxial unloading compression tests on granite and red sandstone indicate that the unloading can induce slabbing when the confining pressure exceeds a certain threshold,and the slabbing failure strength is lower than the shear failure strength according to the conventional Mohr-Column criterion.Numerical results indicate that the rock unloading failure response under different in situ stresses and unloading rates can be characterized by an equivalent strain energy density.In addition,we present a new microseismic source location method without premeasuring the sound wave velocity in rock mass,which can efficiently and accurately locate the rock failure in hard rock mines.Also,a new idea for deep hard rock mining using a non-explosive continuous mining method is briefly introduced.展开更多

AIM: To evaluate the efficacy of sequential blood purification therapy in the treatment of critical patients with hyperlipidemic severe acute pancreatitis.METHODS: Thirty-one intensive care unit(ICU) patients with hyp...AIM: To evaluate the efficacy of sequential blood purification therapy in the treatment of critical patients with hyperlipidemic severe acute pancreatitis.METHODS: Thirty-one intensive care unit(ICU) patients with hyperlipidemic severe acute pancreatitis treated at the Second Affiliated Hospital of Harbin Medical University were divided into either a study group(n = 15; July 1, 2012 to June 30, 2014) or a control group(n = 16; July 1, 2010 to June 30, 2012) based on the implementation of sequential blood purification therapy. The control group received continuous venous-venous hemofiltration(CVVH) on the basis of conventional treatments, and the therapeutic dose of CVVH was 30 m L/kg per hour. The study group received sequential plasma exchange and CVVH on the basis of conventional treatments. The anticoagulation regimen of CVVH is the regional citrate anticoagulation. Mortality rate on day 28, rates of systemic and local complications, duration of ICU, and time to target serum lipid level, as well as physiologic and laboratory indices were compared between the two groups.RESULTS: The mortality rate on day 28 was significantly lower in the study group than in the control group(13.33% vs 37.50%; P < 0.05). The duration of ICU stay was significantly shorter in the study group than in the control group(7.4 ± 1.35 d vs 9.19 ± 2.99 d, P < 0.05). The time to target serum lipid level was significantly shorter in the study group than in the control group(3.47 ± 0.52 d vs 7.90 ± 1.14 d, P < 0.01). There were no significant differences in the rates of systemic complications and local complications between the two groups(60% vs 50% and 80% vs 81%, respectively). In the comparisons of physiologic and laboratory indices, serum albumin and C-reactive protein were significantly better in the study group than in the control group after treatment(37.8 ± 4.6 g/L vs 38.9 ± 5.7 g/L, and 20.5 ± 6.4 mg/L vs 28.5 ± 7.1 mg/L, respectively, both P < 0.05). This study analyzed the characteristics of glycemic variability in type 2 diabetes mellitus (T2DM) with HbAlc 〈6.5% in duration of twice daily premixed insulin treatment and the effect of further treatment with acarbose. Methods Eighty-six T2DM patients who used premixed insulin analogue (insulin aspart 30) twice daily and had HbAlc 〈6.5% and 20 controlled subjects with normal glucose regulation (NGR) were monitored using the continuous glucose monitoring (CGM) system. The mean amplitude of glycemic excursions (MAGE), mean of daily differences (MODD) were used for assessing intra-day, inter-day glycemic variability. Hypoglycemia was defined as glucose level 〈3.9 mmol/L for at least 15 minutes in CGM. According to reference values of MAGE, T2DM patients were classified into two groups: Iow-MAGE group with MAGE 〈3.4 mmol/L (L-MAGE) and high-MAGE group with MAGE 〉3.4 mmol/L (H-MAGE). H-MAGE group received further treatment with acarbose for 2 weeks and was monitored a second time with CGM system. Results After first CGM, L-MAGE group had 41 cases, and H-MAGE group had 45 cases. The MAGE and MODD of T2DM group were all higher than those of subjects with NGR (P 〈0.01). Twenty-four percent (n=11) in H-MAGE group had a total of 13 hypoglycemic events, 10 of the 13 events occurred at night, meanwhile 5% (n=-2) in L-MAGE group had a total of 2 hypoglycemic events, which also occurred at night (hypoglycemic events: 24% vs. 5%, X2=6.40, P 〈0.01). MAGE value was correlated with hypoglycemia value and 2-hour postprandial plasma glucose value (r=-0.32 and 0.26, respectively, P 〈0.05). The effects of average high gravity factor (β), reflux ratio (R), and feedstock flux (F) on mass-transfer in distillation were examined separately. Experimental results indicated that the total number of theoretical units (NTU) of RPB increased with β, R, and F.Of the three kinds of packings, the wave thread packing of stainless steel (Packing-Ⅲ)-had the best mass transfer efficiency with the height equivalent of a theoretical plate (HETP) of approximately 7.35 mm- 23.58 ram, whereas the corrugated disk pacing of stainless steel,(Packing Ⅰ) had the worst one with the HETP of about 13.4 mm-48.07 mm.Correlations were cleveloped to describe the mass transfer efficiency for packings Comparing.experimental data with the data calculated by correlation, the average deviate obtained for each packing was 0.72%, 1.98%, and 2.7%, respectively, implying that the accuracy of correlations developed was reasonable.展开更多
The biochemical methane potentials for typical fruit and vegetable waste (FVW) and food waste (FW) from a northern China city were investigated, which were 0.30, 0.56 m3 CH4/kgVS (volatile solids) with biodegrad...The biochemical methane potentials for typical fruit and vegetable waste (FVW) and food waste (FW) from a northern China city were investigated, which were 0.30, 0.56 m3 CH4/kgVS (volatile solids) with biodegradabilities of 59.3% and 83.6%, respectively. Individual anaerobic digestion testes of FVW and FW were conducted at the organic loading rate (OLR) of 3 kg VS/(m3-day) using a lab-scale continuous stirred-tank reactor at 35°C. FVW could be digested stably with the biogas production rate of 2.17 ma/(m3-day) and methane production yield of 0.42 m3 CH4/kg VS. However, anaerobic digestion process for FW was failed due to acids accumulation. The effects of FVW: FW ratio on co-digestion stability and performance were further investigated at the same OLR. At FVW and FW mixing ratios of 2:1 and 1:1, the performance and operation of the digester were maintained stable, with no accumulation of volatile fatty acids (VFA) and ammonia. Changing the feed to a higher FW content in a ratio of FVW to FW 1:2, resulted in an increase in VFAs concentration to 1100-1200 rag/L, and the methanogenesis was slightly inhibited. At the optimum mixture ratio 1:1 for co-digestion of FVW with FW, the methane production yield was 0.49 m3 CH4/kg VS, and the volatile solids and soluble chemical oxygen demand (sCOD) removal efficiencies were 74.9% and 96.1%, respectively.展开更多

Several different types of CFRTP tensile test specimens are printed and are tensile tested in the longitudinal direction to obtain an overview of the mechanical properties of 3D printed CFRTP material. These properties are compared with the literature values known for composite materials to see if these agree. The main goal of this research is to increase the knowledge of the 3D printing process of CFRTP and to later use this knowledge to further improve the 3D printing process to obtain stronger 3D printed CFRTP materials.展开更多

This study investigated...Objective Evidence suggests that type 2 diabetes (T2DM) is associated with an increased risk of dementia and that glucose variability is an independent risk factor for diabetic complications. This study investigated the relationship between glucose excursion and cognitive function in aged T2DM patients. Methods A total of 248 aged T2DM patients wore a continuous glucose monitoring system (CGMS) for 3 days in order to evaluate glucose excursion, including mean amplitude of glycemic excursions (MAGE) and mean of daily difference (MODD). All subjects were evaluated with a number of accepted cognitive function tests, including the mini-mental status examination (MMSE). The relationship between MAGE and MODD and performance on these cognitive tests was assessed. Results The MAGE and MMSE score were negatively correlated, likewise with the correlation between MODD and MMSE. Liner multivariate regression analysis showed that MAGE and MODD were also negatively related to MMSE independent of age, sex, glycemic control, hypertension, smoking, or coronary heart disease history. Conclusion Glucose excursion is related to cognitive function in aged T2DM patients. Elevated glucose excursion decreased the MMSE score, which reflects general cognitive function. Thus, therapy aimed at controlling glucose excursion may be beneficial for maintaining cognitive function in aged T2DM patients.展开更多
Background: For patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and type 2 diabetes mellitus (T2DM), the night sleep interruption and intermittent hypoxia due to apnea or hypopnea may induce glyce...Background: For patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and type 2 diabetes mellitus (T2DM), the night sleep interruption and intermittent hypoxia due to apnea or hypopnea may induce glycemic excursions and reduce insulin sensitivity. This study aimed to investigate the effect of continuous positive airway pressure (CPAP) therapy in patients with OSAHS and T2DM. Methods: Continuous glucose monitoring system (CGMS) was used in 40 patients with T2DM and newly diagnosed OSAHS. The measurements were repeated after 30 days of CPAP treatment. Subsequently, insulin sensitivity and glycohemoglobin (HbAlc) were measured and compared to the pretreatment data. Results: After CPAP therapy, the CGMS indicators showed that the 24-h mean blood glucose (MBG) and the night time MBG were significantly reduced (P 〈 0.05 and P = 0.03, respectively). The mean ambulatory glucose excursions (MAGEs) and the mean of daily differences were also significantly reduced (P 〈 0.05 and P = 0.002, respectively) compared to pretreatment levels. During the night, MAGE also significantly decreased (P = 0.049). The differences between the highest and lowest levels of blood glucose over 24 h and during the night were significantly lower than prior to CPAP treatment (P 〈 0.05 and P = 0.024, respectively). The 24 h and night time durations of high blood glucose (〉7.8 mmol/L and 〉 11.1 mmol/L) decreased (P 〈 0.05 and P 〈 0.05, respectively) after the treatment. In addition, HbA 1 c levels were also lower than those before treatment (P 〈 0.05), and the homeostasis model assessment index of insulin resistance was also significantly lower than before CPAP treatment (P = 0.034). Conclusions: CPAP therapy may have a beneficial effect on improving not only blood glucose but also upon insulin sensitivity in T2DM patients with OSAHS. This suggests that CPAP may be an effective treatment for T2DM in addition to intensive diabetes management.展开更多

Several different types of CFRTP tensile test specimens are printed and are tensile tested in the longitudinal direction to obtain an overview of the mechanical properties of 3D printed CFRTP material. These properties are compared with the literature values known for composite materials to see if these agree. The main goal of this research is to increase the knowledge of the 3D printing process of CFRTP and to later use this knowledge to further improve the 3D printing process to obtain stronger 3D printed CFRTP materials.

文摘Objective Evidence suggests that type 2 diabetes (T2DM) is associated with an increased risk of dementia and that glucose variability is an independent risk factor for diabetic complications. This study investigated the relationship between glucose excursion and cognitive function in aged T2DM patients. Methods A total of 248 aged T2DM patients wore a continuous glucose monitoring system (CGMS) for 3 days in order to evaluate glucose excursion, including mean amplitude of glycemic excursions (MAGE) and mean of daily difference (MODD). All subjects were evaluated with a number of accepted cognitive function tests, including the mini-mental status examination (MMSE). The relationship between MAGE and MODD and performance on these cognitive tests was assessed. Results The MAGE and MMSE score were negatively correlated, likewise with the correlation between MODD and MMSE. Liner multivariate regression analysis showed that MAGE and MODD were also negatively related to MMSE independent of age, sex, glycemic control, hypertension, smoking, or coronary heart disease history. Conclusion Glucose excursion is related to cognitive function in aged T2DM patients. Elevated glucose excursion decreased the MMSE score, which reflects general cognitive function. Thus, therapy aimed at controlling glucose excursion may be beneficial for maintaining cognitive function in aged T2DM patients.
文摘Background: For patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and type 2 diabetes mellitus (T2DM), the night sleep interruption and intermittent hypoxia due to apnea or hypopnea may induce glycemic excursions and reduce insulin sensitivity. This study aimed to investigate the effect of continuous positive airway pressure (CPAP) therapy in patients with OSAHS and T2DM. Methods: Continuous glucose monitoring system (CGMS) was used in 40 patients with T2DM and newly diagnosed OSAHS. The measurements were repeated after 30 days of CPAP treatment. Subsequently, insulin sensitivity and glycohemoglobin (HbAlc) were measured and compared to the pretreatment data. Results: After CPAP therapy, the CGMS indicators showed that the 24-h mean blood glucose (MBG) and the night time MBG were significantly reduced (P 〈 0.05 and P = 0.03, respectively). The mean ambulatory glucose excursions (MAGEs) and the mean of daily differences were also significantly reduced (P 〈 0.05 and P = 0.002, respectively) compared to pretreatment levels. During the night, MAGE also significantly decreased (P = 0.049). The differences between the highest and lowest levels of blood glucose over 24 h and during the night were significantly lower than prior to CPAP treatment (P 〈 0.05 and P = 0.024, respectively). The 24 h and night time durations of high blood glucose (〉7.8 mmol/L and 〉 11.1 mmol/L) decreased (P 〈 0.05 and P 〈 0.05, respectively) after the treatment. In addition, HbA 1 c levels were also lower than those before treatment (P 〈 0.05), and the homeostasis model assessment index of insulin resistance was also significantly lower than before CPAP treatment (P = 0.034). Conclusions: CPAP therapy may have a beneficial effect on improving not only blood glucose but also upon insulin sensitivity in T2DM patients with OSAHS. This suggests that CPAP may be an effective treatment for T2DM in addition to intensive diabetes management.
