Background Clinical practice guidelines (CPGs) play an important role in healthcare in China as well as in the world. However, the current status and trends of Chinese CPGs are unknown. The aim of this study was to ... Background Clinical practice guidelines (CPGs) play an important role in healthcare in China as well as in the world. However, the current status and trends of Chinese CPGs are unknown. The aim of this study was to systematically review the present situation and the quality of Chinese CPGs published in the peer-reviewed medical literature. Methods To identify Chinese CPGs, a systematic search of relevant literature databases (CBM, WANFANG, VIP, and CNKI) was performed for the period January 1978 to December 2010. We used the AGREE II instrument to assess the quality of the included guidelines. Results We evaluated 269 guidelines published in 115 medical journals from 1993 to 2010 and produced by 256 different developers. Only four guidelines (1%) described the systematic methods for searching and selecting the evidence, 14 (5%) guidelines indicated an explicit link between the supporting evidence and the recommendations, only one guideline used the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system. Thirty-one guidelines (12%) mentioned updates and the average frequency of update was 5.5 years; none described a procedure for updating the guideline. From the assessment with the Appraisal of Guidelines for Research and Ecaluation II (AGREE II), the mean scores were low for the domains "scope and purpose" (19%) and "clarity of presentation" (26%) and very low for the other domains ("rigour of development" 7%, "stakeholder involvement" 8%, "applicability" 6% and "editorial independence" 2%). Conclusions Compared with other studies on the quality of guidelines assessed with the AGREE instrument in other countries, Chinese CPGs received lower scores, which indicates a relatively poor quality of the guidelines. 目的:探讨葛根素注射液联合胺碘酮治疗冠心病患者行经皮冠状动脉介入治疗术(PCI)后并发心律失常的疗效和安全性。方法:回顾性分析80例PCI术后并发心律失常的冠心病患者资料,按照治疗方式不同分为观察组(40例)和对照组(40例)。所有患者... 目的:探讨葛根素注射液联合胺碘酮治疗冠心病患者行经皮冠状动脉介入治疗术(PCI)后并发心律失常的疗效和安全性。方法:回顾性分析80例PCI术后并发心律失常的冠心病患者资料,按照治疗方式不同分为观察组(40例)和对照组(40例)。所有患者围手术期及术后用药根据病情参照美国心脏病学会/美国心脏协会(AHA/ACC)制定的冠心病治疗指南。两组患者均予以常规对症治疗,与此同时,对照组患者给予胺碘酮注射液150 mg加入5%葡萄糖注射液20 ml中充分溶解,在15 min内缓慢静脉滴完,待病情得到缓解后按照胺碘酮的负荷量3 mg/kg,以1 mg/min的滴速缓慢静脉滴注,6 h后再以0.5 mg/min的滴速维持,24 h总量为1 200 mg,连续静脉滴注3 d+口服胺碘酮片0.2 g,tid,1周后减至0.2 g,bid;观察组患者在对照组治疗基础上给予葛根素注射液400 mg加入0.9%氯化钠注射液250 ml中静脉滴注,qd。两组疗程均为2周。观察两组患者临床疗效,治疗前后心率(HR)、左心室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)及不良反应发生情况。结果:观察组患者总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组患者HR、LVEF、LVESD、LVEDD比较,差异均无统计学意义(P>0.05);治疗后,两组患者HR、LVESD、LVEDD均显著低于同组治疗前,且观察组低于对照组,LVEF均显著高于同组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:葛根素注射液联合胺碘酮治疗冠心病患者PCI术后并发心律失常的疗效较好,可以改善患者心功能,且不增加不良反应,安全性较好。 展开更多

针对目前长期径流预报中物理成因考虑较少的问题,以丹江口水库为例,在分析影响径流物理背景的基础上,研究前期气象因子与水库秋汛期入库径流过程的相关关系,识别影响径流的大气环流与海温等物理因子,利用主成分分析法提取主要预报信息,... 针对目前长期径流预报中物理成因考虑较少的问题,以丹江口水库为例,在分析影响径流物理背景的基础上,研究前期气象因子与水库秋汛期入库径流过程的相关关系,识别影响径流的大气环流与海温等物理因子,利用主成分分析法提取主要预报信息,建立了包含大气环流因子、海温因子等气象物理信息以及前期降雨、径流等水文信息作为预报因子集的三层BP神经网络预报模型.利用1956~2008年秋汛期9、10月入库径流量进行模拟与试报,并与仅采用前期降雨径流的预测模型进行了比较,结果显示基于物理成因分析的预测模型稳定性良好,模拟及试报精度较高,9、10月试报精度平均提高约30%,分别达到87.5%和75%,并对预报年份中的丰枯特征有较好的体现. 展开更多

The optimal planning and design of an integrated energy system(IES)is of great significance to facilitate distributed renewable energy(DRE)technology and improve the overall energy efficiency of the energy system.With the increased penetration of distributed generation(DG),the power supply and load sides of an IES present more increased levels of uncertainties.Demand response(DR)and the energy storage system(ESS)serve as important means to shift energy supply and use across time to counter the indeterminate variations.However,the current IES planning methods are unable to effectively deal with the uncertainties of DREs and loads,and to optimize the operations of DG-DR-ESS due to the enormous possible combinations.In this paper,a new method for the optimal planning and design of an integrated energy system has been introduced and verified.The new method consists of three integrated elements.First,the method of the probability scenario has been used to model the uncertainties of the DREs and loads so as to better characterize the impact of uncertainty on the planning and design of the IES.Secondly,the optimal operation of the IES under different probability scenarios is ensured using the second-order cone optimization for quick solutions due to the simplicity of this sub-problem,serving as the bottom-level optimization.Thirdly,the optimal planning and design of IES through optimal sizing of the power generating components and ESS are performed using a special meta-model based global optimization method due to the complex,black-box,and computation intensive nature of this top-level optimization in a nested,bi-level global optimization problem.The combined approach takes full account of the interrelated operations of DG-DR-ESS under different design configurations to support a better optimal planning and design of the IES.The simulation has been carried out on an IES system modified from the IEEE 33-node distribution system.The simulation results show that the proposed method and model are effective. 展开更多

Selective laser melting (SLM), an additive manufacturing process, is capable of manufacturing metallic parts with complex shapes directly from computer-aided design (CAD) models. SLM parts are created on a layer-by-layer manner, making it more flexible than traditional material processing techniques, in this paper, Inconel 625 alloy, a widely used material in the aerospace industry, were chosen as the build material. Scanning electron microscopy (SEM), electron back scattering diffraction (EBSD) and X-ray diffraction (XRD) analysis techniques were employed to analyze its microstructure. It was observed that the molten pool was composed of elongated columnar crystal, Due to the rapid cooling speed, the primary dendrite arm space was approximately 0.5 μm and the hardness of SLM state was very high (343 HV). The inverse pole figure (IPF) indicated that the growing orientation of the most grains was 〈001〉 due to the epitaxial growth and heat conduction. The XRD results revealed that the austenite structure with large lattice distortion was fully formed. No carbides or precipitated phases were found. After heat treatment the grains grew into two microstructures with distinct morphological characters, namely, rectangular grains and limited in the molten pool, and equiaxed grains along the molten boundaries. Upon experiencing the heat treatment, MC carbides with triangular shapes gradually precipitated. The results also identified that a large number of zigzag grain boundaries were formed. The 1-year(2009-2010) measurements are analyzed of the urban surface energy balance(SEB) obtained from the sensors located at three vertical layers of a 325-m tower in downtown Beijing.Results show that:(1) The measurements from the 325-m tower represent the SEB characteristics of the cities located in semi-humid warm-temperate continental monsoon climate zone.In a typical hot and rainy summer,cold and dry winter,the measured Bowen ratio is minimum in summer and maximum in winter.The Bowen ratio measured at 140 m for spring,summer,autumn,and winter are 2.86,0.82,1.17,and 4.16 respectively.(2) At the height of 140-m(in the constant flux layer),the noontime albedo is ~0.10 for summer,~0.12 for spring and autumn,and ~0.14 for winter.The ratios of daytime sensible heat flux,latent heat flux,and storage heat flux to net radiation are 0.25,0.16,and 0.59 for clear-sky days,and 0.33,0.19,and 0.48 for cloudy days respectively.(3) Under clear-sky days,the nighttime sensible heat flux is almost zero,but the latent heat flux is greater than zero.For cloudy days,the nighttime sensible heat flux is slightly greater than the latent heat flux in winter.The nighttime upward heat flux is presumably due to the anthropogenic release(mainly latent heat for summer,while latent and sensible heat for winter). 展开更多

The results showed that with temperature rising, Chinese calcined flint clay based plastic refractories shrinked firstly and then expanded. The modulus of rupture (MOR) and the cold crushing strength (CCS) inereased firstly and then decreased from 110 ℃ to 600 ℃ , then increased obviously. Thermal expansion coefficient increased from 110 ℃ to 760 ℃, decreased from 760 ℃ to 1 300 ℃ , and increased from 1 300 ℃ to1500 ℃. 展开更多

Direct writing of electronics based on alloy and metal (DREAM) ink: A newly emerging area and its impact on energy, environment and health sciences 被引量:22

Electronics, such as printed circuit board (PCB), transistor, radio frequency identification (RFID), organic light emitting diode (OLED), solar cells, electronic display, lab on a chip (LOC), sensor, actuator, and transducer etc. are playing increasingly important roles in people's daily life. Conventional fabrication strategy towards integrated circuit working steps, generally (IC), requesting at least six consumes too much energy, material and water, and is not environmentally friendly. During the etching process, a large amount of raw materials have to be abandoned. Besides, lithography and microfabrication are typically carried out in "Clean room" which restricts the location of IC fabrication and leads to high production costs. As an alternative, the newly emerging inkjet printing electronics are gradually shaping modem electronic industry and its related areas, owing to the invention of a series of conductive inks composed of polymer matrix, conductive fillers, solvents and additives. Nevertheless, the currently available methods also encoun ter some technical troubles due to the low electroconduc tivity, complex sythesis and sintering process of the inks. As an alternative, a fundamentally different strategy was recently proposed by the authors' lab towards truly direct writing of electronics through introduction of a new class of conductive inks made of low melting point liquid metal or its alloy. The method has been named as direct writingof electronics based on alloy series of functional circuits, and metal (DREAM) ink. A sensors, electronic elements and devices can thus be easily written on various either soft or rigid substrates in a moment. With more and more technical progresses and fundamental discoveries being kept made along this category, it was found that a new area enabled by the DREAM ink electronics is emerging, which would have tremendous impacts on future energy and environmental sciences. Real time remaining useful life(RUL) prediction based on condition monitoring is an essential part in condition based maintenance(CBM). In the current methods about the real time RUL prediction of the nonlinear degradation process, the measurement error is not considered and forecasting uncertainty is large. Therefore, an approximate analytical RUL distribution in a closed-form of a nonlinear Wiener based degradation process with measurement errors was proposed. The maximum likelihood estimation approach was used to estimate the unknown fixed parameters in the proposed model. When the newly observed data are available, the random parameter is updated by the Bayesian method to make the estimation adapt to the item's individual characteristic and reduce the uncertainty of the estimation. The simulation results show that considering measurement errors in the degradation process can significantly improve the accuracy of real time RUL prediction. 展开更多

The traffic explosion and the rising of diverse requirements lead to many challenges for traditional mobile network architecture on flexibility, scalability, and deployability. To meet new requirements in the 5 G era, service based architecture is introduced into mobile networks. The monolithic network elements(e.g., MME, PGW, etc.) are split into smaller network functions to provide customized services. However, the management and deployment of network functions in service based 5 G core network are still big challenges. In this paper, we propose a novel management architecture for 5 G service based core network based on NFV and SDN. Combined with SDN, NFV and edge computing, the proposed framework can provide distributed and on-demand deployment of network functions, service guaranteed network slicing, flexible orchestration of network functions and optimal workload allocation. Simulations are conducted to show that the proposed framework and algorithm are effective in terms of reducing network operating cost. 展开更多

Herbal traditional Chinese medicine(TCM) is used to treat several ailments, but its efficiency is poorly documented and hence debated, as opposed to modern medicine commonly providinge ffective therapies. The aim of this review article is to present a practical reference guide on the role of herbal TCM in managing gastrointestinal disorders, supported by systematic reviews and evidence based trials. A literature search using herbal TCM combined with terms for gastrointestinal disorders in Pub Med and the Cochrane database identified publications of herbal TCM trials. Results were analyzed for study type, inclusion criteria, and outcome parameters. Quality of placebo controlled, randomized, double-blind clinical trials was poor, mostly neglecting stringent evidence based diagnostic and therapeutic criteria. Accordingly, appropriate Cochrane reviews and meta-analyses were limited and failed to support valid, clinically relevant evidence based efficiency of herbal TCM in gastrointestinal diseases, including gastroesophageal reflux disease, gastric or duodenal ulcer, dyspepsia, irritable bowel syndrome, ulcerative colitis, and Crohn's disease. In conclusion, the use of herbal TCM to treat various diseases has an interesting philosophical background with a long history, but it received increasing skepticism due to the lack of evidence based efficiency as shown by high quality trials; this has now been summarized for gastrointestinal disorders, with TCM not recommended for most gastrointestinal diseases. Future studies should focus on placebo controlled, randomized, double-blind clinical trials, herbal product quality and standard criteria for diagnosis, treatment, outcome, and assessment of adverse herb reactions. This approach will provide figures of risk/benefit profiles that hopefully are positive for at least some treatment modalities of herbal TCM. Proponents of modern herbal TCM best face these promising challenges of pragmaticmodern medicine by bridging the gap between the two medicinal cultures. 展开更多

Urban construction land suitability evaluation (UCLSE) is a complex system engineering and the basis for rational use of the limited urban land resources in China. It has an important practical value on urban construction land use planning and management from the angle of methodology. As a widely used technique, traditional multi-criteria evaluation based on GIS (MCE-GIS), is not suitable for UCLSE. This study develops an improved MCE-GIS method which could be more suitable for UCLSE based on urban complex ecological system theory and the summary of the shortcomings of traditional MCE-GIS. The improvements include three aspects: a composite evaluation index system rather than natural indexes alone, an index weight calculated by using fuzzy Analytic Hierarchy Process (AHP) method rather than the common AHP method, and the integrated overlay rule, which includes selecting the minimum value, weighted linear combination (WLC) and simple summation. The main advantage of this improved technique is that it can make UCLSE more comprehensive, more operational and more reasonable. It can provide a scientific basis for decision making in the planning and management of urban construction land use. The improved MCE-GIS system has been adopted in the New Hefei City, Anhui Province, China. Based on the results of UCLSE in New Hefei, three functional areas including construction-appropriate areas, construction-restricted areas and construction-forbidden areas could be worked out, in which 36.90% of the total study area could be developed as urban construction land and the remaining 63.10% should be protected as reserves land or as ecological land. Furthermore, the results can orovide scientific decision suooort for spatial planning and eco-environment nrotection in New Hefei. 展开更多

With the aid of multi-agent based modeling approach to complex systems, the hierarchy simulation models of carrier-based aircraft catapult launch are developed. Ocean, carrier, aircraft, and atmosphere are treated as aggregation agents, the detailed components like catapult, landing gears, and disturbances are considered as meta-agents, which belong to their aggregation agent. Thus, the model with two layers is formed i.e. the aggregation agent layer and the meta-agent layer. The information communication among all agents is described. The meta-agents within one aggregation agent communicate with each other directly by information sharing, but the meta-agents, which belong to different aggregation agents exchange their information through the aggregation layer first, and then perceive it from the sharing environment, that is the aggregation agent. Thus, not only the hierarchy model is built, but also the environment perceived by each agent is specified. Meanwhile, the problem of balancing the independency of agent and the resource consumption brought by real-time communication within multi-agent system (MAS) is resolved. Each agent involved in carrier-based aircraft catapult launch is depicted, with considering the interaction within disturbed atmospheric environment and multiple motion bodies including carrier, aircraft, and landing gears. The models of reactive agents among them are derived based on tensors, and the perceived messages and inner frameworks of each agent are characterized. Finally, some results of a simulation instance are given. The simulation and modeling of dynamic system based on multi-agent system is of benefit to express physical concepts and logical hierarchy clearly and precisely. The system model can easily draw in kinds of other agents to achieve a precise simulation of more complex system.
