背景:虽然关节置换围术期处置已经形成了一套标准化的流程,但是关节置换后假体周围感染仍是一个尚未彻底解决的难题,治疗方案依然没有达成共识。随着患者的不断增多,迫切需要具有指南意义的规范化治疗方案。目的:综述近年国外关节置换... 背景:虽然关节置换围术期处置已经形成了一套标准化的流程,但是关节置换后假体周围感染仍是一个尚未彻底解决的难题,治疗方案依然没有达成共识。随着患者的不断增多,迫切需要具有指南意义的规范化治疗方案。目的:综述近年国外关节置换后假体周围感染的研究进展。方法:第一作者应用计算机检索1998至2019年PubMed数据库、Springerlink数据库的相关文章,英文检索词"Joint replacement,Periprosthetic joint infection,Progress in diagnosis and treatment";共检索到400余篇相关文献,50篇文献符合纳入标准。结果与结论:(1)随着新型血清标志物α-防御素和白细胞酯酶及D-二聚体、新型核医学检验方法及利用代谢组学和蛋白质组学等新方法的提出,可以凭借早期感染的生物标志物发现和诊断假体周围感染;(2)此外可以利用新的技术来破坏生物膜、微生物繁殖过程以及定量分子方法来提高病原体鉴定的准确性;(3)随着这些假体周围感染治疗进展的出现,快速、准确、经济的方法可能使明确诊断及培养出致病菌成为一件简单的事,那么很多病例都可以选择一期翻修;如若软组织条件不好或者不满足一期翻修的条件,则可进入二期翻修;(4)明确致病微生物,可以使用敏感的药物进行治疗,为二期翻修做好充足的准备,这对于假体周围感染治疗具有划时代的意义。 展开更多

Good accuracy of the alpha-defensin lateral flow test for hip periprosthetic joint infection: A pilot study in a retrospective cohort of 52 patients

BACKGROUND The alpha-defensin lateral flow(ADLF)test is a new diagnostic tool for periprosthetic joint infection(PJI).Test accuracy for combined cohorts of hip and knee PJI has been reported to be good.AIM To assess the accuracy of the ADLF test for hip PJI,and to compare three different diagnostic criteria for PJI.METHODS A cohort of 52 patients was identified,with a painful or poor-functioning total hip-or hemi-arthroplasty,that underwent aspiration and a subsequent ADLF test.PJI was diagnosed with Musculoskeletal Infection Society(MSIS)criteria,and sensitivity,specificity,overall accuracy,positive predictive value and negative predictive value were calculated.Furthermore,test specifics were compared with the European Bone and Joint Infection Society(EBJIS)and 2018 International Consensus Meeting(ICM)criteria for PJI.RESULTS Using MSIS criteria,sensitivity was 100%(CI:54%-100%)and specificity was 89%(CI:76%-96%).Six true positives and 5 false positives were found,including one case of metallosis.Using EBJIS criteria,more PJIs were found(11 vs 6),sensitivity was lower(71%,CI:42%-92%)and specificity was higher(97%,CI:86%-100%),with 4 false negatives and one false positive result.Using 2018 ICM criteria,sensitivity was 91%(62%-100%)and specificity 100%(91%-100%).The results in this cohort are comparable to previous studies.CONCLUSION Overall test accuracy of the ADLF test was good in this cohort,with a sensitivity of 100%and specificity of 89%.Using different PJI definition criteria,sensitivity and specificity changed slightly but overall accuracy remained around 90%.Using the ADLF test in metallosis cases can result in false positive results and should be performed with caution. 展开更多

BACKGROUND The quantitative alpha-defensin enzyme-linked immunosorbent assay(ELISA)demands a prior synovial fluid centrifugation,whereas this processing is not routinely required prior to the alpha-defensin lateral flow test.AIM To evaluate whether a prior synovial fluid centrifugation could lead the lateral flow performance to achieve comparable results to ELISA during periprosthetic joint infection(PJI)diagnosis.METHODS Fifty-three cases were included in this study:22 classified as PJI and 31 classified as aseptic cases,according to Musculoskeletal Infection Society 2013 criteria.Synovial fluid samples were submitted to centrifugation,and the supernatant was evaluated by ELISA and lateral flow tests.The sensitivity(SE),specificity(SP)and accuracy of each method were calculated as well as the agreement between those two methods.RESULTS In all of the 31 samples from aseptic patients,alpha-defensin ELISA and lateral flow tests showed negative results for infection.Regarding the 22 infected patients,the lateral flow test was positive in 19 cases(86.4%)and the ELISA was positive in 21(95.5%).Sensibility,SP and accuracy were,respectively,86.4%(95%CI:65.1%-97.1%),100%(95%CI:88.8%-100%)and 93.2%(95%CI:82.8%-98.3%)for the lateral flow test and 95.5%(95%CI:77.2%-99.9%),100%(95%CI:88.8%-100%)and 98.1%(95%CI:89.9%-100%)for ELISA.An agreement of 96.2%between those methods were observed.No statistical difference was found between them(P=0.48).CONCLUSION Alpha-defensin lateral flow test showed high SE,SP and accuracy after a prior synovial fluid centrifugation,achieving comparable results to ELISA.Considering the lower complexity of the lateral flow and its equivalent performance obtained in this condition,a prior centrifugation might be added as a valuable step to enhance the PJI diagnosis. 展开更多

Objective: To evaluate the alpha-defensin (α-DF) genes polymorphism in women with vulvovaginal candidiasis and recurrence. Methods: This observational study included clinical vaginal secretion samples collected over four years from 88 women, ranging in age from 18 to 65 years, from medical centers of Sao Paulo and Mogi das Cruzes, Brazil. Thirty-six of these women were asymptomatic (control group) and 52 presented clinical condition compatible with vulvovaginitis (38 primary or episodic as non-recurrent forms, and 14 recurrent vulvovaginal candidiasis). A portion of each sample was plated on Sabouraud dextrose agar with chloramphenicol and grown on CHROMagar Candida for presumptive characterization. The identification of the species was obtained by sequencing of the ITS1 region of rDNA. α-DF genes were amplified for subsequent evaluation of polymorphisms by endonuclease restriction assay. Results: From 88 samples were isolated 60 Candida albicans and 28 non-albicans Candida spp. Resistant C. albicans strains and non-albicans Candida spp. were more prevalent in recurrence. In all groups, the number of resistant non-albicans Candida spp. was most high than susceptible strains. α-DF1, α-DF3 and α-DF1/α-DF3 genotypes were found in 32 (36.4%), 17 (19.3%), 6 (6.8%) vaginal samples, respectively. About 33 samples were not amplified. Recurrence and severe disease were more observed in homozygous population. Conclusions: Non-albicans Candida spp. and homozygotic α-DF genotipes (α-DF1 and α-DF3) were more related with severe clinical signs and recurrence. Further studies about vulvovaginal candidiasis and α-DF genes are necessary to access the more comprehensive role of defensins in clinical manifestations. 展开更多

BACKGROUND Alpha-defensin has been widely studied for the diagnosis of periprosthetic joint infection(PJI).However,there is a lack of detailed information regarding the proper laboratory technique of the enzyme-linked immunosorbent assay(ELISA)method,such as sample dilution.AIM To assess the influence of dilution in the synovial fluid during ELISA for the diagnosis of knee PJI;and determine which dilution presents a better performance.METHODS Forty samples of synovial fluid from arthroplasty knees were included,17 in the infected group and 23 in the aseptic group,according to Musculoskeletal Infection Society criteria.Initially,five synovial fluid samples from each group were assessed for quantitative analysis of alpha-defensin using ELISA.Different dilution ratios(1:10,1:100,1:500,1:1000 and 1:5000)were tested based on the predetermined cutoff value of 5.2 mg/L.The dilutions that performed better were used to compare the results of all samples.RESULTS For infected cases,a gradual increase in the dilution of synovial fluid samples led to an equivalent increase in alpha-defensin level.The same was not observed in the aseptic cases.Both 1:1000 and 1:5000 dilutions presented satisfactory results to differentiate infected and aseptic cases.Further analyses were performed using 1:1000 and 1:5000 for all 40 samples.The 1:1000 dilution resulted in a sensitivity of 88.2%(95%CI,66%-98%)and specificity of 95.7%(95%CI,79%-99%),whereas the 1:5000 dilution presented a sensitivity of 94.1%(95%CI,73%-99%)and a specificity of 100%(95%CI,86%-100%).CONCLUSION The synovial fluid dilution had an important influence on the alpha-defensin ELISA results.Dilutions of 1:5000 showed the best performance for the diagnosis of knee PJI.The results of this study set the basis for a more reliable and reproducible alpha-defensin ELISA during the investigation of PJI,contributing to the expansion of this technique in different treatment centers worldwide. 展开更多

目的评价急性ST段抬高型心肌梗死(STEMI)时全身与局部血浆α防御素1-3(DEFA1-3)水平的变化。方法入选对象均为男性,冠状动脉正常[无冠状动脉粥样硬化性心脏病(CAD)]组31例、稳定型CAD组44例、STEMI组47例。全身血样经造影导管取自主动... 目的评价急性ST段抬高型心肌梗死(STEMI)时全身与局部血浆α防御素1-3(DEFA1-3)水平的变化。方法入选对象均为男性,冠状动脉正常[无冠状动脉粥样硬化性心脏病(CAD)]组31例、稳定型CAD组44例、STEMI组47例。全身血样经造影导管取自主动脉根部,局部血样(STEMI组)经抽吸导管取自罪犯血管。夹心酶联免疫吸附法(ELISA)测定血浆DEFA1-3水平。结果全身血浆DEFA1-3水平STEMI组>稳定型CAD组>无CAD组(均P<0.05)。局部与全身血浆DEFA1-3水平差异无显著性(P>0.05)。DEFA1-3诊断STEMI的最佳诊断分界点为136.3μg/L,其敏感度和特异度分别为77.2%和66.7%,受试者操作特征曲线(ROC)曲线下面积(AUC)为0.717(95%CI:0.624~0.811,P=0.000)。结论 STEMI时患者全身血浆DEFA1-3水平升高,有可能作为新型炎性标志物用于急性冠状动脉综合征患者风险或预后评估。 展开更多

目的观察肠道α-防御素-5(RD-5)、β-防御素-2(BD-2) mRNA在急性肝衰竭(ALF)小鼠造模后不同时间的动态表达,探讨其在ALF发病机制中的作用。方法将60只C57BL6小鼠按随机数字表法分为4组:正常对照组、ALF造模组、大肠埃希菌(E.coli)灌胃组... 目的观察肠道α-防御素-5(RD-5)、β-防御素-2(BD-2) mRNA在急性肝衰竭(ALF)小鼠造模后不同时间的动态表达,探讨其在ALF发病机制中的作用。方法将60只C57BL6小鼠按随机数字表法分为4组:正常对照组、ALF造模组、大肠埃希菌(E.coli)灌胃组和ALF+E.coli灌胃组。采用氨基半乳糖500 mg/kg+脂多糖10 μg/kg腹腔注射造模,另对ALF小鼠予E.coli灌胃,观察时间点为造模后6 h、12 h、24 h,每个时间点6个标本。采用实时荧光定量PCR方法检测小鼠回肠末端组织RD-5、BD-2 mRNA的表达。结果RD-5,BD-2水平在实验性ALF中呈动态变化,与正常对照组(11.25±0.74、23.86±0.39)相比,ALF造模组与ALF+E.coli灌胃组在造模后6 h显著升高(分别为14.19±0.39、26.79±0.36,12.57±0.68、26.45±0.85),差异均有统计学意义(均P<0.05);12 h达高峰(分别为15.76±0.33、29.10±0.61,12.90±0.96、27.42±0.71),差异均有统计学意义(均P<0.05);后逐渐下降;BD-2水平较RD-5水平升高更明显。结论RD-5、BD-2可能在实验性小鼠ALF肠源性内毒素血症发病机制中起一定作用。 展开更多

目的:分析消化性溃疡患者血清α防御素(alpha defensin,DEFA)、内脂素(visfatin),IL-17的表达及意义。方法:选择重庆市长寿区人民医院2016年3月至2018年3月收治的102例消化性溃疡患者,按疾病类型、幽门螺杆菌(helicobacter pylori,Hp)... 目的:分析消化性溃疡患者血清α防御素(alpha defensin,DEFA)、内脂素(visfatin),IL-17的表达及意义。方法:选择重庆市长寿区人民医院2016年3月至2018年3月收治的102例消化性溃疡患者,按疾病类型、幽门螺杆菌(helicobacter pylori,Hp)情况、病情程度分为不同组别,并同期选择95例健康体检者做为对照组。比较各组血清DEFA,visfatin,IL-17水平,并分析其相关性和诊断效能。结果:消化性溃疡组血清DEFA水平低于对照组,血清visfatin,IL-17水平高于对照组(P<0.05)。复合性溃疡组血清DEFA水平低于胃溃疡组及十二指肠溃疡组,血清visfatin,IL-17水平高于胃溃疡组及十二指肠溃疡组(P<0.05)。消化性溃疡Hp阴性组血清DEFA水平高于HP I型组及HP II型组(P<0.05);血清visfatin,IL-17水平高于HP高型组及HP及型组(P<0.05)。消化性溃疡患者活动期血清DEFA低于愈合期且低于疤痕期,visfatin,IL-17水平高于愈合期且高于疤痕期(P<0.05)。消化性溃疡患者血清DEFA和visfatin呈负相关,DEFA和IL-17呈负相关(r分别为-0.526,-0.654),visfatin和IL-17呈正相关(r为0.404)。受试者工作特征(receiver-operating characteristic,ROC)曲线显示:血清DEFA水平诊断曲线下面积(area under curve,AUC)为0.756,visfatin水平诊断AUC为0.652,IL-17水平诊断AUC为0.830,DEFA+visfatin+IL-17水平诊断AUC为0.892。结论:DEFA,visfatin及IL-17均可参与消化性溃疡发病,其水平改变可能和炎症程度有关,可作为临床重要的观察指标。 展开更多

目的探讨关节液α防御素-1(AD-1)、基质细胞衍生因子1(SDF-1)、可溶性细胞间黏附分子-1(sICAM-1)对人工肱骨头置换术术后感染的诊断价值,以及与愈合时间、肩关节功能的关系。方法选取本院42例人工肱骨头置换术术后感染患者作为研究组,... 目的探讨关节液α防御素-1(AD-1)、基质细胞衍生因子1(SDF-1)、可溶性细胞间黏附分子-1(sICAM-1)对人工肱骨头置换术术后感染的诊断价值,以及与愈合时间、肩关节功能的关系。方法选取本院42例人工肱骨头置换术术后感染患者作为研究组,另选同期86例人工肱骨头置换术术后未感染患者作为对照组。检测对比两组关节液AD-1、SDF-1、sICAM-1水平,分析关节液各指标对术后感染的诊断价值,以及关节液各指标与愈合时间、Neer评分的关系。结果研究组关节液AD-1、SDF-1、sICAM-1水平高于对照组(P<0.05);三者联合诊断人工肱骨头置换术术后感染具有较高的敏感性(73.81%)、特异性(91.86%),且AUC高达0.882;此外,关节液中的AD-1、SDF-1、sICAM-1与愈合时间存在显著正相关关系(r=0.623、0.477、0.490,P<0.05),与Neer评分存在显著负相关关系(r=-0.826、-0.794、-0.747,P<0.05)。结论AD-1、SDF-1、sICAM-1与人工肱骨头置换术术后感染的发生密切相关,三者联合诊断在人工肱骨头置换术术后感染诊断方面具有较高的临床诊断价值,且与感染患者愈合时间、肩关节功能存在密切相关性,有助于评估感染患者预后。 展开更多

背景:研究表明关节滑液人α防御素1-3诊断假体周围感染的准确性较高,但关节穿刺对手术部位具有创伤性,不利于全膝关节置换后常规动态监测,长期重复检查可能会增加发生假体周围感染的风险,干扰骨科医生判断假体周围感染发生的原因与时间... 背景:研究表明关节滑液人α防御素1-3诊断假体周围感染的准确性较高,但关节穿刺对手术部位具有创伤性,不利于全膝关节置换后常规动态监测,长期重复检查可能会增加发生假体周围感染的风险,干扰骨科医生判断假体周围感染发生的原因与时间。外周血检测具有创伤小、标本获取容易、利于动态监控的优点,但目前国内外尚无初次全膝关节置换后血浆人α防御素1-3动态变化规律观察的相关研究。目的:观察初次全膝关节置换后血浆人α防御素1-3和外周血炎性指标的动态变化规律,为全膝关节置换后假体周围感染的早期筛查提供新的思路。方法:前瞻性纳入2015年4-12月因晚期膝骨关节炎行初次单侧全膝关节置换的患者,分别在全膝关节置换前、置换后第1,3,5天、置换后2周及置换后第1,3个月检测外周血炎性指标,并采用三文治夹心酶联免疫吸附法检测血浆人α防御素1-3水平。结果与结论:最终共27例患者纳入研究。初次全膝关节置换后外周血C-反应蛋白、红细胞沉降率呈现“单峰状”动态变化,即置换后先升高后下降;外周血白细胞计数、中性粒细胞计数、中性粒细胞百分比及血浆人α防御素1-3均呈现“双峰状”动态变化,即置换后先升高后下降,再次升高后再次下降。血浆人α防御素1-3的升降速度与幅度均较外周血白细胞计数、中性粒细胞计数、中性粒细胞百分比更加明显。突出的“双峰态势”提示血浆人α防御素1-3或许是全膝关节置换后动态监控炎症活动的良好生物标志物,可为全膝关节置换后监测并诊断关节假体周围感染提供了新思路和佐证。 展开更多

关节液α防御素-1检测在人工关节感染患者中的应用价值

目的探究关节液α防御素-1检测在判断人工关节感染(prosthetic joint infection,PJI)中的应用价值。方法选择2014年2月-2015年3月本院接诊的47例需要进行人工关节翻修术的患者进行研究,排除其他痛风、骨髓增生异常综合征等干扰因素,选... 目的探究关节液α防御素-1检测在判断人工关节感染(prosthetic joint infection,PJI)中的应用价值。方法选择2014年2月-2015年3月本院接诊的47例需要进行人工关节翻修术的患者进行研究,排除其他痛风、骨髓增生异常综合征等干扰因素,选择美国肌肉与骨骼感染协会(Muscularskeletal Infection Society,MSIS)诊断标准进行PJI诊断,将患者随机分为感染组患者24例,非感染组患者23例,将两组患者关节液α防御素-1与MSIS诊断指标的灵敏性以及特异性进行比较。结果关节液α防御素-1对PJI检测的可靠性较高,其敏感性为85.7%,特异性为92.2%;23例非感染组患者中,有20例患者确诊为非感染者;24例感染组患者中,有22例患者确诊为PJI,能够实现对86.96%患者的正确区分。结论关节液α防御素-1在进行PJI的诊断方面有着较大的发展潜力,结合体征、临床特征来进行综合诊断可以保证诊断结果的准确性。 展开更多
