AIM To investigate predictive and prognostic value of serum alpha-fetoprotein(AFP) level and its dynamic changes in patients with advanced gastric cancer with elevated serum AFP(AFPAGC).METHODS One hundred and five pa...AIM To investigate predictive and prognostic value of serum alpha-fetoprotein(AFP) level and its dynamic changes in patients with advanced gastric cancer with elevated serum AFP(AFPAGC).METHODS One hundred and five patients with AFPAGC were enrolled in the study, and all of them underwent at least one cycle of systemic chemotherapy at our institute and had serum AFP ≥ 20 ng/m L at diagnosis or recurrence. Clinicopathologic features, serum AFP level at diagnosis and changes during treatment, first-line chemotherapy regimens, efficacy and toxicity, and survival information were collected. A Person's χ~2 or Fisher's exact test was used to measure the differences between variables. Survival prognostic factors were investigated using the Kaplan-Meier method and Cox regression.RESULTS Median serum AFP level was 161.7 ng/m L(range, 22.9-2557110 ng/m L). Objective response rates(ORR) was significantly lower in the AFP ≥ 160 ng/m L group than in the AFP < 160 ng/m L group(30.4% vs 68.3%, P < 0.001). ORR to doublet regimens was significantly lower in the AFP ≥ 160 ng/m L group, whereas ORR to triplet regimens was similar between the two groups. Liver metastasis rate was significantly higher in the AFP ≥ 160 ng/m L group than in the AFP < 160 ng/m L(69.8% vs 50.0%, P < 0.001). Overall survival(OS) in the two cohorts did not show any significant difference(P = 0.712). Dynamic changes of AFP were consistent with response to chemotherapy, and median OS of patients with a serum AFP decline ≥ 50% and those with a serum AFP decline < 50% was 17.5 m and 10.0 m, respectively(P = 0.003). Hepatic(P = 0.005), peritoneal(P < 0.001), non-regional lymph node metastasis(P < 0.001), and portal vein tumor thrombus(PVTT)(P = 0.042) were identified as independent prognostic factors for AFPAGC. CONCLUSION Real-time examination of AFP has great predictive and prognostic value for managing AFPAGC. For those with markedly elevated AFP, triplet regimens may be a better choice.展开更多

From 15 K to room temperature, the magnetic field dependent of electroluminescence (MEL) of devices was investigated. Our observations indicated that the MEL is composed of two effects in different regimes: a low field (0≤B≤40 mT) effect and a high field (B 】 40 mT) effect. For undoped devices, the low field effect exhibits a rapid rising with the increasing field, and the high field effect shows a slow increase and gradually saturates at room temperature. For doped devices, the low field rapid increase is also present, whereas the high field effect displays a decrease with the increasing field. The larger the injection current is, the more apparent the high field decrease is. In addition, the doped device demonstrates less temperature dependence of the high field effect than undoped device, although the undoped devices also present high field decrease of electroluminescence at low temperatures (T≤150 K). Based on the energy level trapping effect due to dye doping and magnetic field modulated triplet exciton annihilation, the experimental results are carefully explained.展开更多
Background:There is no consensus on whether triplet regimen is better than doublet regimen in the first-line treatment of advanced gastric cancer(AGC).We aimed to compare the efficacy and safety of oxaliplatin plus ca...Background:There is no consensus on whether triplet regimen is better than doublet regimen in the first-line treatment of advanced gastric cancer(AGC).We aimed to compare the efficacy and safety of oxaliplatin plus capecitabine(XELOX)and epirubicin,oxaliplatin,plus capecitabine(EOX)regimens in treating AGC.Methods:This phase III trial enrolled previously untreated patients with AGC whowere randomly assigned to receive the XELOXor EOXregimen.The primary endpoint was non-inferiority in progression-free survival(PFS)for XELOX as compared with EOX on an intention-to-treat basis.Results:Between April 10,2015 andAugust 20,2020,448AGCpatientswere randomized to receive XELOX(n=222)or EOX(n=226).The median PFS(mPFS)was 5.0 months(95%confidence interval[CI]=4.5-6.0 months)in the XELOX arm and 5.5 months(95%CI=5.0-6.0 months)in the EOX arm(hazard ratio[HR]=0.989,95%CI=0.812-1.203;P_(non-inferiority)=0.003).There was no significant difference inmedian overall survival(mOS)(12.0 vs.12.0months,P=0.384)or objective response rate(37.4%vs.45.1%,P=0.291)between the two groups.In patients with poorly differentiated adenocarcinoma and liver metastasis,the EOX arm had a significantly longer mOS(P=0.021)and a trend of longer mPFS(P=0.073)than the XELOX arm.The rate of grade 3/4 adverse events(AEs)was 42.2%(90/213)in the XELOX arm and 72.5%(156/215)in the EOX arm(P=0.001).The global health-related quality of life(QoL)score was significantly higher in the XELOX arm than in the EOX arm during chemotherapy.Conclusions:This non-inferiority trial demonstrated that the doublet regimen was as effective as the triplet regimen and had a better safety profile and QoL as a first-line treatment for AGC patients.However,the triplet regimen might have a survival advantage in patients with poorly differentiated adenocarcinoma and liver metastasis.展开更多
The translational oscillation of the solid inner core is one of the Earth’s fundamental normal modes, which is also called Slichter mode. The normal mode should be split to form a triplet due to the Earth’s rotation...The translational oscillation of the solid inner core is one of the Earth’s fundamental normal modes, which is also called Slichter mode. The normal mode should be split to form a triplet due to the Earth’s rotation and ellipticity. In this study, according to the splitting pattern of Slichter mode, an attempt has been made to detect the possibility of Slichter triplet’s existence by using the product spectra of the long-term continuous gravity measurements from the superconducting gravimeters (SG) at 6 glob- ally-distributed permanent stations in the Global Geodynamics Projects network. The results indicate that the background noise level of the global SG observations is 0.0158 nm s–2 and the magnitude threshold of any global harmonic signals, which may be detected by the global SG, is 0.0152 nm s–2 in the subtidal frequency band from 0.162 to 0.285 cph in which Slichter triplet may occur. it implies that the signatures, related to the triplet, may be identified in the global SG observations, if they exist. It is found that there is a group of global harmonic signatures with the periods of 5.310, 4.995 and 4.344 h emerging significantly from the background noise in the global SG observations. They are in good agreement with the splitting pattern of Slichter mode. It implies that this group of signatures may be related to the inner core translational oscillations. The associated density contrast across the inner core boundary may be deduced as between the values provided in the Earth models of the PREM and 1066 A.展开更多
Based on the energy transfer process from host to dopant in an organic electrophosphorescent (EP) device, the expression of energy transfer probability (η) between the host (TPD) and guest (Ir(ppy)3) EP sys...Based on the energy transfer process from host to dopant in an organic electrophosphorescent (EP) device, the expression of energy transfer probability (η) between the host (TPD) and guest (Ir(ppy)3) EP systems was proposed. The results show that: (1) The rate of the triplet energy transfer (KHG and KGH) increases exponentially with increasing donor-acceptor molecular distance (R), whereas decreases as the intermolecular distance (RHH) increases from 0.8 to 2.4 nm. Furthermore, KGH changes more quickly than KHG. (2) The energy transfer probability (η) increases as R reduces, and the RHH changes can be safely neglected for R〈0.9 nm. The situation changes for 0.9nm〈 R 〈 1.1nm, RHH (〈1nm) plays an essential role when η changes and increases with the latter. However, if R〉1.1nm, the transfer probability will be below zero. Here, the energy transfer principle may be less important, and the high electroluminescence (EL) quantum efficiency of phosphorescent system will be attributed to the direct electron-hole recombination in phosphorescent molecules. (3) The η will increase when the Forster radius (R0) increases or Gibb's energy decreases.展开更多
Based upon seven superconducting gravimeter (SG) records of 20 000 h length after the 2004 Sumatra earthquake, four methods, namely the ensemble empirical mode decomposition (EEMD), the multi-station experiment (...Based upon seven superconducting gravimeter (SG) records of 20 000 h length after the 2004 Sumatra earthquake, four methods, namely the ensemble empirical mode decomposition (EEMD), the multi-station experiment (MSE) technique, the autoregressive (AR) method and the product spec- trum analysis (PSA) method, are chosen jointly together to detect the inner core translational modes (1S1). After the conventional pretreatment, each of the seven simultaneous residual gravity series is di- vided into five segments with an 80% overlap, and then EEMD is applied to all the 35 residual SG se- ries as a dyadic filter bank to get 35 filtered series. After then, according to different stations and dif- ferent time windows, five new simultaneous gravity datasets are obtained. After using MSE for each of the five new datasets, the AR method is used to demodulate some known harmonic signals from the new sequences that obtained by using MSE, and three demodulated product spectra are obtained. Then, according to two criterions, two clear spectral peaks at periods of 4.548 9±2.3×10^-5 and 3.802 3±3.2×10^-5 h corresponding respectively to the singlets m=-1 and m=+l are identified from various spectral peaks, and they are close to the predictions of the 1066A model given by Rieutord (2002), but no spectral peak corresponding to the singlet m=0 is found. We conclude that the selected two peaks might be the ob- served singlets of the Slichter triplet.展开更多
