目的探讨头颅CT在新生儿窒息脑损伤的诊断价值。方法回顾分析60例新生儿窒息脑损伤患儿的临床资料与头颅CT特征。结果临床表现为新生儿缺氧缺血性脑病(HIE)60例中轻度31例,中度24例,重度5例;而头颅CT表现正常者6例,异常者54例,轻度5例,... 目的探讨头颅CT在新生儿窒息脑损伤的诊断价值。方法回顾分析60例新生儿窒息脑损伤患儿的临床资料与头颅CT特征。结果临床表现为新生儿缺氧缺血性脑病(HIE)60例中轻度31例,中度24例,重度5例;而头颅CT表现正常者6例,异常者54例,轻度5例,中度39例,重度10例,合并颅内出血(ICH)45例,其中蛛网膜下腔出血(SAH)40例。SAH合并脑室出血(IVH)5例、脑实质出血(IPH)8例、硬膜下出血(SDH)10例,单纯硬膜下出血(SDH)6例。结论头颅CT表现与临床存在一定的差异,对HIE临床分度的判断存在不确定性,但对新生儿窒息脑损伤的诊断及判断预后有重要的价值。 展开更多

目的探讨快速检测血浆脑利钠肽(BNP)对颅脑损伤评估的意义。方法单纯颅脑损伤41例,颅脑损伤伴颅内出血28例,入院当天行头颅CT扫描,并测定入院当天、第3天、第7天血浆BNP浓度,并与对照组30例比较。结果颅脑损伤组(单纯颅脑损伤和颅脑损... 目的探讨快速检测血浆脑利钠肽(BNP)对颅脑损伤评估的意义。方法单纯颅脑损伤41例,颅脑损伤伴颅内出血28例,入院当天行头颅CT扫描,并测定入院当天、第3天、第7天血浆BNP浓度,并与对照组30例比较。结果颅脑损伤组(单纯颅脑损伤和颅脑损伤伴颅内出血)发病当天BNP水平均高于对照组,差异有统计学意义(P<0.05),颅脑损伤伴颅内出血组发病第3天BNP逐渐升高,与入院当天比较差异有统计学意义(P<0.05),第7天升高明显且达高峰,与对照组比较差异有统计学意义(P<0.01)。单纯颅脑损伤组发病第3、7天BNP均显著下降,与入院时比较差异有统计学意义(P<0.05)。结论不同时段测定颅脑外伤患者BNP均有不同程度升高,动态观察血浆BNP含量是早期诊断和监测颅脑外伤伴颅内出血的有效指标之一。 展开更多

Objective: This study aimed to describe anatomic variants of sphenoidal sinuses and adjacent structures. Methods: A retrospective and descriptive study was carried out at The National and University Teaching Hospital Hubert Koutoukou Maga (CNHU-HKM), of Cotonou in Benin from November 1st to December 31st. A review of CT scans skull was done for the subjects aged at least 16 years old. Anatomic variants of the sphenoid sinuses and adjacent structures have been investigated. Results: 225 CT scans of skull were analyzed. The sellar type was the most common type of pneumatization of sphenoid sinuses (74.7%). Pneumatization of anterior clinoid processes of greater and lesser wings of the sphenoid and of pterygoid processes was observed in 7.1%;4.6%;3.3% and 7.3%;respectively. Protrusion of carotid canals and optic canals, maxillary and vidian nerves were observed in 48.3%;13.1%;18% and 9.5%;respectively. Conclusion: Risky anatomic variants of the sphenoid sinuses and adjacent structures are also described by CT-scan among Beninese. Before any surgery and to avoid bad outcome, a precise approach of these risky anatomic variants must be carried out by using CT-scan. 展开更多
